Patient is required to sign the MCAF [Medical Claim Authorization Form] form. The hospital will facilitate the submission on patient’s behalf to the insurance company upon their discharge.
AXA: AXA Shield
An Integrated Shield Plan has two parts
• Additional coverage above MediShield Life (e.g. for Class A/ B1 wards in public hospital, or in private hospital)
• Managed by private insurer
• MediShield Life forms the foundation of all IPs
• Targeted at large hospital bills in Class B2/C in public hospitals
• Non-profit, managed by CPF Board
All Integrated Shield Plan policyholders are covered by MediShield Life. MediShield Life is part of your Integrated Shield Plan. There is NO duplicate coverage.
