• We were deeply saddened to hear the news of the death of Dr Kate Granger this year.
• Kate inspired us to promote and support the #hellomynameis… campaign
• We encouraged our providers to do this and provided training in compassion awareness, practices and skills
• All of our providers have adopted #hellomynameis… as has Lancs Constabulary and Boots
• We continue to promote compassion in practice as testament to her life and work
Compassion in practice
Prevention – the context People in Pennine Lancashire have some of the worst health in the country and
die earlier than people living elsewhere Economic deprivation is driving health inequalities Unhealthy lifestyle choices are causing higher rates of preventable health
problems, e.g. diabetes; heart disease and respiratory problems
• 12% of our population have experienced 4 or more preventable adverse childhood experiences (ACEs), increasing their risk of poor health and social outcomes in adulthood
• Lower than national uptake of bowel, cervical and breast screening – screening is important to detect cancer early and so improve outcomes
• Children and young people are not getting the best start in life that they could be
• Economic deprivation is driving health inequalities for all people
• People are not accessing services early enough• Health and care services need to be more person centred and
coordinated, especially for those with complex needs• Some people spend longer in hospital than they need to and
more care and support could be provided outside of hospital• Services could be run more efficiently • There are difficulties in recruiting and retaining staff at all
levels

• Patient co-production has driven the redesign of MSK services in the area
• Patients love the self-referral based spinal drop-in clinics established in each of our 5 boroughs for lower back pain
• A redesigned website with self-care videos and guidance is helping patients manage their pain and reduce worry
• Clinicians across the primary/secondary care interface have forged strong relationships with effective clinical leadership.
• Patient care pathways have improved as a result

• New technology brings with it efficiencies, value for money and new ways to put patients at the heart of services
• We invested in telemedicine for 49 nursing homes across 5 boroughs
• GP triage has transformed patient care, workload and outcomes
• Nursing homes receive instant medical advice and support reducing unnecessary attendances at A&E and other services
Telemedicine
• We commission psychological therapies from Lancashire Care Foundation Trust (LCFT) – East Lancashire’s mainstream mental health trust provider
• We wanted to go further to support more people who need psychological therapies
• We sought a solution that added value and patient choice
• The Third sector, embedded in communities, offered a creative, responsive and experienced solution
• Access into treatment has more than doubled in the 1st 12 months and remains on a upward trajectory
• We continue to develop services and build capacity
Improving mental health services

• Increased access to urgent investigations• Increased use of fast track referral• Patients designed patient information leaflet• Increased uptake bowel screening by GPs -
encouraging non-participants• Coordinating test and appointments to speed up
process• More early diagnosis, less late emergency
presentations• We worked together to create a Macmillan
recovery package which supports those with cancer, and we supported the ‘Move More’ exercise programme for recovery
• Cancer targets achieved• Previously - very late diagnosis of pancreatic and
other cancers, now much faster process• The whole health service united for patients
Improving Cancer Services

Palliative and End of Life Care
• Patient-led and inspired strategy for palliative, and end of life care across Pennine Lancashire
• Key worker model developed based on patient feedback to co-ordinate care and encourage
• Pathways reviewed and developed with clinical leadership and patient involvement
• Patients involved in creation of information and promotional materials
• Strong collaboration of providers, patients and commissioners

• We are dementia friendly and promote and support “dementia friendly” activities
• We listened to patients who use hospital eye services who wanted to have increased community eye services, and we have enabled this to happen
• We supported the British Lung Foundation (BLF) to launch a new Breathe Easy group in Burnley and co-funded the costs of running the group
• We developed a new model for diabetes services in East Lancashire which we believe will improve clinical outcomes and quality of life for patients by bringing services closer to home
• Patients told us that they want to see improved access to a GP. We listened and we have engaged with patients, receiving over 2,000 responses to our consultation. The feedback will be used to create the final model
