But the stairway is blocked…
• Extended length of stay: patients decondition:
a never-ending cycle begins
• Bed block: decreased access to hospital
services where do we transfer too?
• Patients not admitted to home wards, lack of
understanding of needs
• Emergency Department over capacity
• Unnecessary re-admissions, poor clinical
handover and transfer planning
• Lack of accommodation options
• Lack of transfer champions
• Limited communication between acute, sub-
acute and community sectors, who should be
doing what?
• Poor communication/handover
• Ownership: who’s responsibility is it?
