This is a story about different groups coming together to change lives.
Let me tell you about how NHS Halton CCG, Halton Council and Community Integrated Care transformed a traditional dementia care home into a center of excellence…
But first let us take you back to the late 90’s – early ‘noughties’
…here’s how the relationship often worked

the pooling of over 42m of
local statutory provision.

• Dad of 2 perfect girls-A Director- A
musician and a life long punk rocker!

• Passion about getting care right and also customer service –commissioners, people we support and their families
• Mum to two grown up boys and a

– the lot!)
Background to St Luke’s
Successful home for many years
Recent years began to experience occupancy
issues.
Hands-off relationship between the home and
the Local Authority
Losses for many years, becoming untenable…
…and then in 2012
NHS reforms – introduction of the CCG and integration
Worked together to create a true Centre of Excellence
David Lyon, CCG Clinical Lead and specialists became
involved
NHS Halton CCG Clinical Specialists work in the service, a
transformation of its living environment and a focus on
providing true relationship-centred care - delivered by
highly trained staff.
Aims continued…
Improve reputation of St Luke’s
Sharing our learning nationally
Create a better living environment of an old
home, to make it more dementia friendly
and person-centred….more of that later

Aligning targets to off-set the prospect of
closure
Creating a Dementia Centre of Excellence
Bringing in Clinical Specialists to the service,
transforming staff skills and forging an
ambitious vision.

Environment - cinema, pub and indoor garden, as well
as reminiscence
Home is now full

facility
