Backgroud• It has been estimated that Finland is one of the fastest ageing countries within the EU• In 2020 one-fifth (20%) of the population will be over 65 years old• The number of Finns over 75 years of age is expected to increase by 50% by the year
2020• The entire age structure and distribution of Finnish population is on its way to
permanent change• According to surveys, most Finns who are over 60 years view ageing mainly in a
positive way• The vast majority of over 75-year older people live in their own homes (86%) • One-third of over 60-year olds receive outside help in their homes in the form of long-
term help or care provided by spouses, children or by municipal home help or home nursing
• If we want elderly people to stay longer at their homes we have to create services which endorse living at home.

Messaging application• Attendo and Sonera started to develop in cooperation messaging
application
• Messaging application goals were:• Improving the quality of service• reducing the sense of urgency of home care workers • Increases visibility for close relatives on delivery of services• Improving flow of information between nurses

Pyhtää home care pilot• The pilot was carried out Attendo Pyhtää home care, which has:
• 12 nurses • approx. 65 regular home care customers • approx. 40 temporary client • 4 nurses participated in the development of the service • 10 home care client's relatives
• Eventually took part in the pilot:• 8 home care clients • 17 relatives • All pyhtää home care nurses and physiotherapists
The pilot started 09.11.2015 and ended on 25.02.2016

Interviews• Before we started the pilot there were interviews:
• 4 home care workers • 11 home care client´s relatives
• The purpose of the interviews was to find out what are the biggest problems in home care
• We wanted to get solutions for communication between client, relative and nurse
