M.P. Shah Hospital is a Social Service league Hospital which has been in existence for the last 83 years, as a ‘not for profit’ organization. The Hospital has one single social objective of providing quality, affordable health care services to the community at affordable costs. It is a multi-super-specialty, state-of-the art, tertiary care hospital with a current capacity of 210 fully operational beds, 30 dedicated critical care beds, 10 renal dialysis units and 12 chemotherapy units. It provides all the modern out-patient and in-patient facilities with dedicated pavilions/ wards, specialty clinics, advanced radiology, pathology and Cath lab operations. We are going through exciting times of development and redefining the way health care is delivered in East and Central Africa region.

Dear Participants,On behalf of the M.P. Shah Hospital we wish to warmly Welcome you to the M. P. Shah Hospital 5th Annual Symposium. This is a great opportunity for us to host you, as you attend, participate and network at this symposium. I wish to sincerely thank and appreciate, Dr. S.T. Patel for his leadership and vision for making this symposium truly international.Previous symposiums have seen many hospitals positively impact the community with information that has helped improve healthcare practices throughout the country. We note with concern that infectious disease are still a major healthcare burden in the developing world. Evolving resistance and a profound lack of new antibiotics further compound the situation. These concerns were the driving force behind the introduction of the M. P. Shah Hospital INFECTIOUS DISEASES Symposium, a forum that would begin to address the aforementioned challenges and increase awareness on current best practice, trends and technologies.Over eighty years have passed since M. P. Shah Hospital began as a simple nursing home to provide quality, affordable healthcare services to the community. We have since grown into a fully-fledged hospital providing a variety of health services. This high level of service has been our hallmark for the past eight decades, and has remained consistent as we touch lives and bring hope to many in the region. The past few years have seen significant progress for the Hospital, both in terms of its physical infrastructure as well as its systems and processes. These changes have continued to align the hospital as a center of excellence for quality healthcare in the region. A key development is the commencement of complete Paediatric services in the newly built state-of-the-art Suryakala Maganlal Chandaria Children’s Medical Center. This brand new children’s hospital is a 5 storey center offering both Outpatient and Inpatient Paediatric services including Neonatal and Paediatric ICU as well as private rooms for children separated by age category. A resident team of Paediatrically-trained doctors and nurses run the service, including a Paediatric Intensivist. In addition, the hospital has completed an additional 5 storey tower as part of its long-term development. The Dr. Kanubhai Babla Medical Center, houses our brand new ICU, Theatre, Cardiac Diagnostic Center and Cath Lab, along with executive private patient rooms. The Hospital is going through an exciting phase of development and it has launched a brand new Cardiac Center focused towards improving lifestyle diseases related to heart like hypertension, stroke, angina, hyperlipidemia, diabetes etc. The Dr. Rasik Kantaria Cardiac Center is located on the 5th floor of the Dr. Kanubhai Babla Medical Center. Once again it’s August and it’s time for the 5th infectious diseases symposium, and On behalf of the Organizing Committee, I would like to warmly welcome you all to the M.P. Shah Hospital’s 5th infectious diseases symposium.

Sub-Saharan Africa has borne the major brunt of infectious disease, including HIV-AIDS, Ebola and now Zika is with us. Keeping up with this theme we now have the 5th edition of the Infectious Diseases Symposium. This symposium is one of the Hospital’s largest annual events and it serves as a forum to facilitate interdisciplinary scientific discussions on various aspects of infectious diseases management. It is recognized by the Medical Practitioners & Dentist Board for CME accreditation and we are confident that it shall provide a unique Educational experience, through the distinguished presenters. They will focus on the impact of change on clinical and laboratory aspects of infectious diseases as well as the current burden of infectious diseases in global public health as well as the changing pathogenic landscape due to emerging and re-emerging infections. These sessions will also cover current challenges and future trends in infectious disease diagnostics.

The symposium will provide you with a unique opportunity to learn about the global trends and most significant developments in infectious diseases, as well as to network with a vast group of diagnostics experts, clinicians and opinion leaders. This year we expect over 500 Senior Medical Professionals to attend the symposium. Today is a result of months of planning by the dynamic, young, organizing committee who have seen this event through to the end while adding quality to every aspect.

SPEAKERS PROFILES & ABSTRACTS I Session I: Paediatric Infections

Dr. J Wachira

Senior Consultant Paediatrician - M.P Shah HospitalFellow of the Royal College of Paediatrics & Child Health-UKM.Med Paediatrics & Child Health/MBChB- University of NairobiExecutive Board member- Kenya Paediatrics Association- (KPA)Ex- Temporary advisor World Health Organization (WHO) in neonatal and pediatrics acute care management

Ambrose is a Kenyan paediatrician with Masters training in epidemiology, based at the KEMRI-Wellcome Trust Research Programme in Nairobi. Working closely with the Kenya Ministry of Health in 2009, his early research involved conducting systematic reviews for a national exercise to adapt the World Health Organization paediatric clinical practice guidelines using the GRADE methodology. Following this, Ambrose was invited to support similar exercises in Uganda and Rwanda. More recently, he was the principal investigator on a large pragmatic clinical trial comparing antibiotic treatments for childhood pneumonia. The findings of this study contributed towards a recent major revision in the Kenyan guidelines, and are likely to eventually influence practice in the region. As he approaches the end of his PhD studies, he is currently considering future areas of work that build on his interest in conducting pragmatic trials for interventions to improve the care and outcomes of hospitalized children in sub-Saharan Africa.

Case Management of Childhood Pneumonia: Revised WHO and Kenya Ministry of Health Classifications and TreatmentChildhood pneumonia claims over one million lives annually and is a leading cause of morbidity worldwide. The case management guidelines for childhood pneumonia currently in use in Kenya and many other countries in sub-Saharan Africa were developed by the World Health Organization (WHO) have been credited for contributing towards substantial reductions in pneumonia deaths. There is however uncertainty over the effectiveness of the guidelines in the current era against a background of the introduction of vaccines targeting Haemophilus influenzae type B and Streptococcus pneumoniae - the leading causes of bacterial pneumonia in children. Results from clinical trials conducted in predominantly Asian populations demonstrating clinical equivalence between amoxicillin and benzyl penicillin led to a revision of the WHO pneumonia guidelines in 2013. However, Kenyan policy makers declined to support a similar change in the local guidelines in the absence additional supportive evidence from African populations citing concerns of high mortality in Africa. We conducted conducting a multi-centre pragmatic clinical trial to compare the effectiveness of the two antibiotics. A total of 527 children aged 2–59 months with severe pneumonia as defined in the 2005 WHO guidelines were recruited from 6 public hospitals across Kenya. The risk of treatment failure was 7.7% in the amoxicillin arm and 8.0% in the benzyl penicillin arm at 48 hours in per-protocol analyses (risk difference, −0.3% [95% CI, −5.0% to 4.3%]), indicating non-inferiority within the pre-specified margin of 7% at 48 hours. Similar results were obtained in intention-to-treat analyses. Mortality was low (0.8%) and comparable to reported findings of the previous trials conducted in Asia that informed the WHO technical updates in 2013.

The findings of this study offer important evidence for policy makers in sub-Saharan African considering the local adaptation of the recent global recommendations for the management of childhood pneumonia. This work has been used to inform a 2014 national guideline panel meeting that revisited the Kenyan guidelines on treatment of pneumonia. The panel reviewed the available evidence, including an updated systematic review that included the Kenyan trial results. Draft recommendations to change policy based on this trial and other evidence were presented to the Ministry of Health and accepted as new national treatment guidelines in early 2015.

Dr. A Laving

Dr. A Laving is the Program Coordinator for undergraduate students, Department of Paediatrics & Child Health, University of Nairobi. Responsible for making the undergraduate teaching program, from lecture schedule, tutorial topics to clinical teachings in the wards as well as co-ordinating their ward, newborn unit and child health rotations.Participate in teaching paediatric residents, medical and nursing students, supervising research protocols of residents, development of undergraduate and postgraduate programs and preparing examinations for the Department of Paediatrics and Child Health at the University of Nairobi. Participate in running of the paediatric gastroenterology and general paediatric services at Kenyatta National Hospital. Conduct major teaching ward rounds, a general paediatric outpatient clinic and a paediatric gastroenterology/liver clinic once a week, as well as weekly clinical bedside teaching sessions with fourth year medical students and paediatric residents. Consulted for all paediatric GIT issues at the national referral and teaching hospital. Perform diagnostic and therapeutic upper and lower endoscopies and supervise percutaneous liver biopsies on children.

Dr. R. Shah

Dr. Reena Shah is the Interim Chair of the Department of Medicine, AKUHN. She is an Assistant Professor of Medicine and Section Head for Infectious Diseases. Dr. Shah completed her MBBS at the University of Southampton, UK and had been a member of the Royal College of Physicians (MRCP) London since 2000. She obtained a Diploma in Infectious Diseases from the University of London as well as a Diploma in Tropical Medicine from the London School of Hygiene and Tropical Medicine. Following this she obtained a Masters in Infectious Diseases from the University of London. She was awarded Fellowship to the Royal College of Physicians Edinburgh FRCPE in 2014. Her clinical research interest is primarily in the area of HIV/AIDS and hepatitis B. She is currently Chair of the Antibiotic Sub-Committee of the Pharmacy and Therapeutics Committee and also the Vice-Chair of the Kenya HIV Clinicians Association and member of the British HIV Association.

Dr. L Achieng’

Dr. L Achieng’ Is a Physician and Infectious Disease Specialist. She Is a Lecturer in Internal Medicine at the University of Nairobi and an Adjunct Assistant Professor of Medicine at the University of Texas Medical Branch. She heads the Infectious Diseases Unit at the Kenyatta National Hospital and is a member of several Antibiotic stewardship committees and the National Antimicrobial Stewardship Advisory Group.Research interests include improving outcomes in sepsis, antimicrobial stewardship and in cardiovascular disease in HIV

Antibiotics are among the most commonly prescribed medications and this has led to an increase in development of drug resistance. Increasing resistance makes selection of appropriate empiric antibiotics more challenging. As the global community grapples with the emergence of multi drug resistant organisms, it is imperative that all health facilities develop appropriate antibiograms to guide the activities of antibiotic stewardship committees. The onus remains on clinicians to prescribe antibiotics only when necessary and for the shortest duration possible. In this session we will look at the science of antibiograms and how these should guide our practice as clinicians

Dr. Omonge is qualified in MBChb (Nairobi) and MMed (Nairobi)He is a leading authority in clinical therapeutics and pharmacology.He has an interest in HIV medicine and a respected member of the faculty at the University of Nairobi.He is the general secretary of society of AIDS in Africa and the Kenya HIV Clinicians AssociationHe has authored numerous research articles in the field of HIV and Diabetes.

Dr Enoch Omonge is a lecturer in the department of Clinical Medicine and therapeutics, thematic unit of Clinical Pharmacology. His special interest is HIV Medicine and Rational Antimicrobial use . Has contributed to developing Antibiotherapy protocols and HIV care at the KNH. Current research interest is HIV associated coinfections and comorbidities

Dr. G Omuse

Dr. Geoffrey Omuse is an Assistant Professor and Consultant Clinical Pathologist at the Aga Khan University Hospital Nairobi. He has very diverse research interests including several publications on infections due to Staphylococcus aureus in Kenya. In 2013 he completed a month long training on molecular typing of Staphylococcus aureus as part of a technology transfer grant at Tygerberg Hosital, Stellenbosch University, South Africa.

Title: MRSA in Kenya-Filling the gapSeveral studies looking at the prevalence of MRSA in Kenya have been published. However, there is a paucity of knowledge on the molecular epidemiology of MRSA in Kenya and indeed East Africa. Understanding the molecular epidemiology is necessary in order to better assess the relationship between circulating strains and propensity to cause disease. This also helps when investigating MRSA outbreaks and in evaluating the effectiveness of infection control measures put in place to prevent and control MRSA outbreaks.

Dr David Silverstein is a Consultant Cardiologist and an Internist at the Nairobi Hospital for the last 30 years. He came to Kenya in 1974 as a lecturer and then senior lecturer in the University of Nairobi and subsequently Chief Cardiothoracic physician for the government of Kenya. He entered private practice in 1981 and has served on the Medical Advisory Committee and was elected member of the Board for 17 years.He got his MD degree for the University of Chicago and then went to the University of Washington for his internal medicine residency and Cardiology fellowship. He is a Fellow of the American College of Cardiology (FACC) and the American college of Physicians (FACP).Dr. D Silverstein

Dr. Oundo graduated in 2013 from Moi University with M.B.Ch.B after which he joined Tenwek Hospital in 2014 for internship. In 2015, he worked in casualty department in Longisa County referral hospital in 2015. He then moved to MPSHAH hospital where he currently works as a medical officer in HDU/ICU department. He has a deep interest in internal medicine.

Problem; candidiasis is a common and important problem affecting critical care set ups in our facilities. Cause: the most common species is C. albicans, but there are increasing isolation of C. glabrata, C. krusei and C. parapsilosis. There are multiple risk factors, including an ICU patient, immunocompromised, neutropenia, recent surgery, and central catheters. These factors also increase the risk of contracting fluconazole-resistant strains.Manifestation: infection is usually from GIT penetration, biofilms, localized focus, and colonization, which lead to hematogenous spread. It can then affect multiple organs and organ systems: osteoarticular, endophthalmitis, periotonitis and abdominal infections, pneumonia and empyema, mediastinitis, hepatosplenic involvement, and candidemia.Diagnosis: the main diagnostic modalities are blood culture, biopsy, beta-D-glucan assay, PCR, colonization index and candida score. Susceptibility testing is important for streamlining therapy, de-escalation and following of resistance patterns.Treatment mainly involves adequate source control and proper choice of antifungals. Antifungal choice will depend on resitance patterns, culture results, and the underlying status of the patient. The arsenal is azoles, echinocandins, and amphotericin B.Mortality ranges from 30-60% depending on time of initiation of antifungals and severity of the infection. Outcomes are worse with tumors, neutropenia and higher APACHE II scores.

Treatment  mainly  involves  adequate  source  control  and  proper  choice  of  antifungals.  Antifungal  choice  will  depend  on  resitance  patterns,  culture  results,  and  the  underlying  status  of  the  patient.  The  arsenal  is  azoles,  echinocandins,  and  amphotericin  B.  

Mortality  ranges  from  30-­‐60%  depending  on  time  of  initiation  of  antifungals  and  severity  of  the  infection.  Outcomes  are  worse  with  tumors,  neutropenia  and  higher  APACHE  II  scores.

Update on Visceral Leishmaniasis ( Kala-azar)Visceral Leishmaniasis(VL) is a protozoal infection transmitted by Sand flies ( phlebotomus sp). The disease is characterized by high fever, weight loss, hepato Splenomegaly and Bone Marrow suppression. It is the second largest parasitic killer after Malaria.500,000 visceral leishmania cases occur annually and 90% are from India, Brazil and Sudan.Visceral Leishmaniasis is recognized as one of the neglected tropical diseases. It is endemic in the East African region. In Kenya new foci of the disease have come up. In 2003 Leishmania East African Platform (LEAP) was created which serves as a base for training and evaluation of new treatment. Sodium Stibogluconate (SSG) for 28 days has been the main stay of treatment of VL since 1940. A short course of SSG and Paramomycine(PM) for 17days was evaluated and is now the first line treatment in the East African region. The other drugs are liposomal Amphotericine B and Miltefosine the first highly active oral drug for VL.Diagnosis of VL has been by demonstration of parasites in the bone marrow, splenic aspirate and lymph glands. Serological tests are also available. More recently a rapid diagnostic test K39 dipstick has been recommended in endemic areas where facilities are limited.Work on development is also going on but there is no vaccine yet.

Dr. Jennifer Verani MD, MPH leads the Global Disease Detection, Epidemiology and Surveillance Program for the US Centers for Disease Control and Prevention in Kenya. She is a pediatrician, epidemiologist, and a Captain in the United States Public Health Service. She obtained her medical degree from Harvard Medical School, MPH in International Health from the Harvard School of Public Health, and undergraduate degree in International Development from Brown University. Dr. Verani completed her residency training at the Children’s Hospital of New York-Columbia, where she served in the community pediatrics track. Prior to her career in medicine, Dr. Verani worked for several years on public health programs in Latin America. In 2006 Dr. Verani came to CDC as an Epidemic Intelligence Service (EIS) officer in the Parasitic Diseases Branch, where she conducted research on schistosomiasis, lymphatic filariasis, soil-transmitted helminths, guinea worm, Chagas disease, and Acanthamoeba. Following EIS, she joined the Respiratory Diseases Branch to work on the prevention of pneumonia and neonatal sepsis. In that position Dr. Verani collaborated on studies of the effectiveness and impact of vaccines against Streptococcus pneumoniae and Haemophilus influenzae type B in several countries in Latin America and Africa, and provided technical assistance for respiratory disease surveillance in low-resource settings. She led the revision of CDC guidelines for the prevention of neonatal group B streptococcal (GBS) disease and a multi-state investigation of the remaining burden of early-onset GBS disease in the United States. In her current position Dr. Verani leads the Population-Based Infectious Disease Surveillance platform in Kibera and Asembo, and is establishing a network of Acute Febrile Illness sentinel surveillance sites across Kenya. Dr. Verani is also an Adjunct Assistant Professor of Pediatrics at Emory University School of Medicine, where she helps to train interns who serve patients at an urban primary care clinic.

Title Antibiotic Stewardship in the Private Healthcare settings: Lessons LearntAuthor: Christina Mwachari MBChB, MSC, MPH, PhDIntroduction Introduction: Antibiotic use is the major driver of antimicrobial resistance (AR) worldwide. The inappropriate use of antibiotics has fuelled the emergence of AR in resource limited settings. Antimicrobial stewardship has been demonstrated to slow down the spread of AR. This evaluation demonstrates antimicrobial stewardship experience in the private settings.Methods: Antimicrobial resistance trends were obtained from selected hospitals from the inception of antibiotic stewardship committees in 2008. Lessons learnt since the inception of AS for the rational use of antimicrobials were documented. Results: Low prevalence of MRSA despite increasing trends of resistance to Staphylococcus aureus. Increased prevalence of extended spectrum beta-lactamase producing Gram negative organisms among hospital acquired infections. There was emerging antimicrobial resistance to Carbapenems. Conclusion: Effective antimicrobial stewardship was associated with reduced hospital acquired infections, improved selection of empirical antibiotics, reduced overuse of reserved antibiotics through continuous medical education.

Prof Godfrey Lule currently works as professor of medicine in the Department of clinical medicine and therapeutics, University of Nairobi medical school and he is a practicing consultant Physician, gastroenterologist and thematic head of the infectious disease unit, Kenyatta National hospital. He is an adjunct professor of infectious diseases, University of Texas Medical Branch,Galveston since 2010 and University of Maryland, Baltimore since 2013. Dr. Adam received a Doctor of Medicine degree from the University of Illinois followed by training in internal medicine and infectious diseases at the University of Arizona followed by molecular parasitology training at the National Institutes of Health. He joined the faculty of the University of Arizona College of Medicine where he worked as a Professor of Infectious Diseases and Microbiology / Immunology until 2011. He was a Fulbright scholar at the University of Nairobi in 2011 and then joined the AKUHN faculty in April 2012 as a Professor of Pathology in clinical microbiology. He is currently leading the effort to expand molecular diagnosis at AKUHN and is chair of the research committee and the infection control committee. He has been involved in numerous basic and clinical research related projects primarily in molecular parasitology and fungal infections.

Prof Gunturu Revathi is Associate Professor of Microbiology at the Aga Khan University Hospital, Nairobi. She is also a visiting faculty at the University of Nairobi and the Jomo Kenyatta University of Science and Technology. She has 25 years of professional experience having worked in the Cytology Research Center at the Maulana Azad Medical College, New Delhi, Dept. of Microbiology, the University College of Medical Sciences, New Delhi before moving to Kenya. The Paediatric Intensive Care Unit (PICU) / NeonatalIntensive Care Unit (NICU) and Paediatric High Dependence Unit (HDU) are staffed by highly trained doctors and nurses who specialize in caring for critically ill patients. This ICU has a higher staff-to-patient ratio and access to advanced medical resources. Common conditions that are treated within this ICU include- Acute respiratory distress syndrome, trauma, multiple organ failure and sepsis. Patients may be transferred directly to an intensivecare unit from an emergency department if required, or from a ward if they rapidly deteriorate, or immediatelyafter surgery if the surgery is very invasive and the patient is at high risk of complications. The new ICU is equipped with the latest emergency response monitors, defibrillators, ventilators etc. Each patient has a bedside and centralized cardiac monitoring system. The new ICU beds are designed to keep patients both safe and comfortable. The New Dr. Kanubhai Babla Intensive Care UnitThe M. P. Shah Hospital’s Dr. Kanubhai Babla Intensive Care Unit (ICU) is located on the ground floor of the Dr. Kanubhai Babla Medical Centre. It is a specially designed 7 bed rooms (of which 2 are isolation rooms with negative pressures) that deals with patients with high risk for actual or potential life threatening health problems. Working in ICU requires a lot of focus and stamina but very satisfying. Dealing with critically ill patients requires continuous monitoring of every detail of the patient’s hemodynamic parameters, physiological processes like voiding and breathing to their medication and interacting with their relatives and keeping themup to date and reassured at all times. Empathizing with their relatives and understanding the constant worriesand anxiety concerning the patients and giving them all the available options. Being there for the critically ill and helping them overcome the stress related to disease responses, pain free and still maintaining their dignity and decency. Wound Management ClinicM. P. Shah Hospital now has a Wound Care Center which is one of its kind in the region. The M. P. Shah Hospital Wound Care Center was inaugurated on 13th may 2016 by Prof. Khainga.MP Shah Hospital is the first hospital to have this high standard facility in the country. The department is located in the accident and emergency department. It is equipped with high quality dressing materials, suitable for all kinds of wounds. The unit is headed by the nurse in charge who coordinates the operation of the unit.Dressings are done on appointment basis to enhance proper and early preparations for the patients and materials to be used. A schedule is in place to accommodate the patients’ needs to enhance patient delight. Dressing of the wounds is done by the specialty nurses in A&E in collaboration with the A&E doctors and in constant consultation with the surgeons where need arises.The materials we use are as follows; 1. For debridement – intrasite gel for autolytic debridement, iruxol for conservative sharp enzymes, and adsorb for chemical debridement and versa jet for surgical debridement. 2. For bacterial balance – acticoat, allevyn and bactigrass 3. For inflammation reduction – monocrystalline silver which helps reduce the bacterial load in the wound which is causative of prolonged inflammation 4. For moisture maintenance – elect foam, allevyn, and opsite flexi grid and opsite post –op.This maintains the appropriate moisture levels to help facilitate healing 5. For advance closure – opsite flexigrid and secura.We look at at our patients comprehensively by identifying the factors that delay proper wound healing for example their nutritional status, any underlying medical conditions e.g. cancer or diabetics, age, cause of the wound, compliance to antibiotics and other medications, environment and materials used for dressing. In view of all this, we give our patients a detailed health education based on assessment and identified individual problems. We not only address patient wounds but also give psychosocial support to enable a holistic approach to the healing process.
