In Sunderland in 2009.…
• 25% of adults smoke
• 24% of mothers smoke during pregnancy
• 14% of adults estimated to be at risk of CVD (3% actual)
• 31% of adults report weekly binge drinking
• 24% adults report having a limiting long term condition
In Sunderland in 2009.…• 10% reception children
21% year 6 – obese
• 26% adults obese
• 9.6 yr variation in life expectancy between wards - 1.5 miles apart
• Gap in life expectancy -
narrowed for females……widened for males
• What are we doing ?
Wellness Service
Targeting those individuals who are not yet engaged in physical
activity, ensuring there are a range of preventative services, targeted interventions and specialist support services at a local
level to improve individuals health & well-being
2 examples :
• Community Wellness Programme
• Sunderland Exercise Referral & Weight Management Programme
Community Wellness Programme • 9 sites
• All community based venues
• Targeting those individuals not yet engaged in physical activity
• Ensure a new range of preventative services are available at local level
• Developing & delivering a number of new pathways into programme
• Ensuring a robust monitoring & evaluation framework is in place

Exercise Referral & Weight Management Programme
• Multi agency, multi component
– Sunderland's Wellness Service
– City Hospitals Dietetic Department
– STPCT
• Adults referred in by GP’s & HCP’s
• 15 week supportive programme
• Reasons for referral incl - obesity, arthritis, hypertension, diabetes, anxiety, depression, stress….
• Activities include Gym based, Cycling, Exercise Classes, Walking & Swimming….on referral
• How we do it ?

• Pooling of skills & resources to achieve mutual gain

• What we have achieved so far ?
• Community Wellness Programme
– 31 weekly sessions delivered by CWC– 20 weekly supervised sessions delivered by a volunteers from
venues– 1,300 individuals enrolled on the programme – Over 14,000 attendances – 84 staff/volunteers received training to supervise sessions
• Exercise Referral Programme – 10 dedicated ER staff commissioned by PCT – All 59 GP practices refer – Over 125 regular referring agents– 6 months of programme - 1514 referred– Over 800 began 15 week support programme
