Altogether Better have developed an award winning, evidenced based approach to engaging people and communities Developed Health Champion models Community Health Champions, Practice Health Champions, ED Champions, Pregnancy & Early Years Champions Our ambition is to unlock the potential within communities to improve health and wellbeing And.using a whole system approach we create the conditions for a new relationship between citizens and services which has the potential to transform services Who are we? 3. Altogether Better have Worked with more than 18,000 volunteer community health champions who have supported more than 100,000 people in their communities to live better And the numbers continue to grow.. 4. Community Health Champions are people who voluntarily bring their life experience and their ability to relate to people to promote health in different communities. 5. COMMUNITY HEALTH CHAMPIONS 6. Whats special about Altogether Better? Gives a VOICE to CHCs a national platform to tell their stories Strength in numbers Building and sharing the evidence base Making the case 7. Empowering citizens gives people VOICE Strength in numbers?? Voice means little if no one is LISTENING We want to be in RELATIONSHIP with people We need to create the conditions to change the relationship between citizens and services What have we learnt about citizen voice and shifting the boundary? 8. The consultation and the right conversation The core of health care lies in the conversations that patients have with clinicians and other health care workers A productive way to improve the whole health service is to transform the way the NHS does business by supporting really good conversations with people patients and communities. 9. Information days We started out offering one day but will adapt these looking at 2 days or a range of half days It keeps the process personal; meet people explain the project Find out why people want to be a champion CV Builder? Unemployed? Retired? Wants to give something back to the NHS/Practice? 16. Training Day One Getting to know each other What is health? Models & theory A good lunch!
