• Learn how one hospital implemented, measured and evaluated a medication reconciliation initiative.

• Learn innovative approaches to have patients become advocates for themselves and others regarding safe medication management and communication with healthcare providers.

• Learn how to implement a community awareness campaign to effectively educate the community, providers and organization on the benefits of having an accurate medication list.

North Bay Regional Health Centre
