BACKGROUND

• The Catalan health care system has been characterized by itsability to promote and adapt new organizational andmanagement practices and by being a source of constantinnovation.

• The knowledge generated from the practices carried out by theprofessionals of the Catalan health care system is highly valuableand sharing it is a priority in order to increase the quality andefficiency of the system

OBJECTIVE

• The aims of the Observatory of Innovation in HealthcareManagement in Catalonia are to collect, validate, organize andvalue, mainly, the knowledge of the Catalan health system, frommany innovative experiences that are being developed today.

• It provides a forum for compiling some of the best practicesobserved in the system, drawn up based on the identification ofinnovative initiatives.

• It offers an innovation community web page fostering interactionamong more than 450 health professionals who exchange theirexperiences.

What is it?Consists of the planned and standardised withdrawal of chronic medication in elderly and poly-medicated patients.

What advantages does it offer? Greater patient satisfaction Improvements in functionality and in quality of life Reduction of the risk of side effects and interactions Lower economic expenditure for the patient and for the community

Economic impact?• Primary Healthcare Teams (EAPs) have estimated an annual saving of

4.2 million euros, resulting from the withdrawal of medication from 31,000 patients.

RESULTS

What is it?Virtual appointment with a professional who reviews the tests previously conducted on the patient through the Shared Medical History. Next, the nursing staff contacts the patient to check if the information is correct.

What advantages does it offer? Reduces time and costs for patients by avoiding unnecessary face-to-

face appointments; Allows professionals to prioritize their time Contributes to improving the quality of patient care avoiding

unnecessary complementary exploratory tests.

Economic impact?• A saving of 25 euros per scheduled procedure taking into account

only the saving in complementary tests was estimated.• Taking into account the cost associated with missing a day at work:

the global economic saving was estimated at 120 euros per patient

RESULTS

What is it?Consists of a procedure that begins when the primary health care physician photographs a questionable skin condition on a patient and attaches it to the patient’s medical history. Next a virtual visit is scheduled in the dermatologist’s diary and the dermatologist replies to the physician with the diagnosis and the treatment.

What advantages does it offer?• Reduces time and costs for patients by avoiding trips to the hospital• Contributes to reducing the time between diagnosis and treatment

allowing resolution to be optimized in primary health care.

Economic impact?• Reduction has occurred in the waiting lists for specialist care and a

reduction in referrals to specialist care (average days on the waiting list for specialist care has fallen from 30 to 16).

• The Observatory offers a public forum for posting and showcasing innovative management experiences and therefore constitutes a display cabinet for organisations to publish their experiences and it also forges ties with other institutions and creates synergies and joint projects.

• Key drivers to identify the best practices are not only based on the reduction of costs of healthcare but also on the benefits of innovation in terms of improvement access and scalability to other settings.

• The three best practices offer advantages for patients and professionals in terms of greater satisfaction for patients (improvement in quality of life, reduction of risk of side effects) and for professionals (prioritisation of time and optimisation of primary health care resources).

Best practice work best when they are contextual. Therefore, toidentify what works and what does not work in different contextswill be crucial for the adoption and spread of best practices in thefuture.

Rethinking about the exercise of Best Practice in not being a uniqueexchange of information in a community web page. Identifyingbarriers and drivers for the implementation, sharing problems,acknowledging failures and finding solutions will be a significantstep forward in the future.
