PLANNINGThe Nurse has to derive the necessary

outcomes from the Nursing Diagnosis and the Nursing Interventions.

Decide the input from the patient and family when performing interventions.

Select a standardized care plan and create an individualized care plan.

Select the outcomes , write the goals.

Standard: Usually established by authority with customer census against what should be ideally done.

Criteria: Observable, Measurable, Achievable.
