NURSING DIAGNOSISDefinition: It is a complex decision making process,
making conclusions from collected data and involves analysis of this data.
In ND both subjective and objective data is gathered together, shuffled, sorted, scrambled, spit out in composite form. - By Braden

PLANNINGThe Nurse has to derive the necessary
outcomes from the Nursing Diagnosis and the Nursing Interventions.
Decide the input from the patient and family when performing interventions.
Select a standardized care plan and create an individualized care plan.
Select the outcomes , write the goals.
Purpose of PlanningTo determine changes in patients health
status PrioritazationFocus on a particular problem in 1 shiftCo-ordinate so that more than one problem
can be handled at a given time
IMPLEMENTATIONPerform the action ( Preparation of
Nurse, Patient and Article)Delegation ( Transfer of both
responsibility and accountability), Supervision.
Recording ( Action performed, time, patient status/response)
EVALUATION Definition: Deliberate, systematic process in
which judgement, value of something is made against standard criteria/ value.
Standard: Usually established by authority with customer census against what should be ideally done.
Criteria: Observable, Measurable, Achievable.
