Examples of unlawful disclosure of PHI include:
• Positioning PHI in the subject line of an email
• In the absence of encrypt an e-mail message containing PHI that is sent outside
of the pro? s office
• Asking you to leave the PHI information in a common space
Keep an open ear to your doctor’s office, in a hospital laboratory, hipaa compliant
