Instructions: Weekly check the battery status indicator, verify pads are present and in good condition, and first responder kit is

attached (gloves, pocket mask/faceshield, razor). Sign and date to verify items have been checked. Indicate any missing or damaged

equipment in the comments section. If the battery is no longer working, or pads are missing please contact AED Coordinator (details

below) IMMEDIATELY..
