TRIGR stands for Trial to Reduce Insulin-Dependent Diabetes in the Genetically at Risk. It is a multinational clinical trial aimed at reducing the incidence of Type 1 diabetes in children. More than 40 centers in the U.S., Canada, Europe, and Australia will be involved in TRIGR. They are recruiting about 6000 families with a history of type 1 diabetes and expecting a baby. When babies are weaned from breastfeeding, they will receive infant formulas that contain no, or a reduced amount of, intact foreign food proteins typically found in baby formulas. TRIGR will enroll babies from across 17 countries. Recruitment will span two years, with follow-up for 10 years. It will investigate potential environmental factors, specifically the early introduction of cow's milk into the baby's diet. Participating institutions in the U.S. are located in Los Angeles, New York, Pittsburgh, Ponce, Puerto Rico, Seattle, and St. Louis.

The Genetics of Kidneys in Diabetes (GoKinD) Study is a JDRF research program being conducted in both the U.S. and Canada. The study will help scientists better understand how to prevent and better treat diabetic nephropathy in people with type 1 diabetes. The study is enrolling adults ages 18-54 who have type 1 diabetes. Half of these individuals will have diabetic kidney disease and half will be free of kidney disease. The GoKinD Study also seeks to enroll both parents of these individuals. If parents are unavailable, adults with type 1 diabetes may join the study on their own.

Amyloid diabetes is the term that M.R. (Pete) Hayden, M.D., of Camdenton, Missouri, uses for what we usually call type 2 diabetes. Islet amyloid (a scar tissue) forms in the islets of people with type 2 diabetes. This scar tissue creates a diffusion barrier and causes an insulin secretory defect as well as an insulin absorptive defect around the vessels within the islets of the pancreas, Dr. Hayden tells me. He is a family physician on sabbatical to pursue his interest in islet amyloid.

The approach involves making use of the diabetes simulator as a computer-based interactive blackboard during diabetes education lessons. A randomized controlled trial approach is being used where half a group of patients receive lessons with the diabetes simulator and the other half receive lessons with more standard (non-computer based) lectures. So far the approach has only been tested with 24 patients, however a larger scale study is planned, and any diabetes educators or endocrinologists who might be interested in running such a study locally in their own clinic(s) are invited to contact the AIDA authors via the AIDA Web site.
