2014 Controversies of prostate cancer. We know many things about the prostate cancer, but we still have not found answers to critical questions! Here are 7 questions or controversies described in an excellent post published by @NewPCaInfoLink. 1. P as PSA! Who should we do a PSA as a screening? This is the biggest controversy! At what age, with or without symptoms, abnormal DRE of the prostate, men with a hereditary risk ?! Other factors may contribute to discriminate a population at risk? Baldness, for example? 2. P as PREVENTION! Is there a way to prevent prostate cancer? The answer is definitely no! There is nothing. 3. L as LOW RISQUE. The cancers classified as low risk :  when to treat ? How to identify patients? I say again: we do not know reliably. But I know that we need to find more predictive factors to improve the classification in risk classe. Maybe using nomograms to reduce the range of predictions for a given individual. This is the challenge for the XXIst century urology. 4. C as CURE! Can we CURE a prostate cancer in a men with a high-risk form? 5. C as Chess Game. Medical treatment decisions for prostate cancer at low-risk is like a chess game: think carefully before choosing! Think about the disease, evolution, and sexual and urinary consequences. 6. Q as in Quantity and Quality. The Hormone therapy is not the vitamin therapy! When need to start a androgen deprivation therapy? 7. I as Intermittent! What kind of Androgen Deprivation therapy? Continuous or I n t e r m I t t e n t? I must admit once again that we do not know exactly. « The more you know, the more you realize you know nothing. » Says Socrates Finally, we know more things about #prostatecancer more we realize our ignorance.
