What are chemical peels?

A chemical peel, also called chemexfo-liation or derma peeling, is one of the least invasive ways to improve the appearance of skin.

Chemical peels are a method of regenerating and resurfacing the skin by inducing a controlled wound to the skin. They remove the top layers skin to help induce collagen remodelling and therefore improve sun-damaged skin, skin pigmentation problems, wrinkles,

skin texture, and the overall appearance of the skin. The strength of the chemical peel will determine its depth. Deeper peels will have a greater number of complications and a longer recovery, but also will result in a greater improvement in the skin.

What types of chemical peels are there?

There are several types of chemical peels each suited to different skin problems and types. Chemical peels also vary in strength and are classified as superficial, medium, and deep according to the level of skin they reach.

Are there any medical problems that doctor should be aware of prior to having a chemical peel?

Patients with a history of, or a current infection of herpes simplex virus (cold sores) and , If there is an active infection present, may be asked to wait until it has passed prior to having a chemical peel. Also if Patient have a history of cold sores, needs anti-viral medication as a prophylaxis to an outbreak during the treatment.

If patient have a history of keloid (thick, pigmented scars) may also be excluded from all but the most superficial of peels.

Patients with HIV/AIDS or immunosuppression should avoid chemical peels because this may impair wound healing and increase the likelihood of infection and scarring.

Also those who have recently had a course of oral isotretinoin or Roaccutane should avoid chemical peels for at least 6 months before undergoing medium or deep chemical peels. Similarly, patients who have had a recent face or browlift should wait at least 6 months.

HOW ARE CHEMICAL PEELS PERFORMED?

Prior to surgery, instructions may include the elimina-tion of certain drugs and the preparation of the skin with topical pre-conditioning medications. The patient may be advised to clean the area with an antiseptic soap the day before surgery.

A chemical peel can be performed in a doctor's office or in a surgery center as an out-patient proce-

dure. At the time of treatment, the skin is thoroughly cleansed with an agent that removes excess oils, and the eyes and hair are protected. One or more chemical solutions - an alpha hydroxy acid, such as glycolic acid, salicylic acid, or lactic acid; trichloroacetic acid (TCA); or carbolic acid (phenol) - are used. Dermatologic surgeons/ Therapist are well qualified to select the proper peeling agent based upon the type of skin damage present. During a chemical peel, the physician applies the solution to small areas on the skin. These applications produce a controlled wound, enabling new, refreshed skin to appear. Most patients experience a warm to somewhat hot sensation which lasts about five to ten minutes, followed by a stinging sensation. A deeper peel may require pain medication during or after the procedure.

Chemical peeling is often used to treat fine lines under the eyes and around the mouth. Wrinkles caused by sun damage, aging and heredi-tary factors can often be reduced or even eliminated with this proce-dure. However, sags, bulges, and more severe wrinkles do not respond well to peeling and may require other kinds of cosmetic surgical proce-dures such as a face lift, brow lift, eye lift or soft tissue filler.
