Burning Mouth Syndrome

Definition
• Burning Mouth Syndrome (BMS) is an intra-oral burning sensation for which no medical or
dental cause can be found. *

Epidemiology
• BMS is fairly common worldwide, and affects up to five individuals per 100,000 general population. People with BMS are more likely to be middle aged or elderly
• females are three to seven times more likely to have BMS than males.
• Asian and native American people have considerably higher risk
Clinical Features
• Pain in the mouth that is burning, scalding, or tingling which affect the
tongue, lips, or roof of the mouth
• Xerostomia .
• Erythema
• Edema of papillae on the tip of the tongue.
Clinical Features
• Altered taste sensation such as bitter or metallic taste
• Geographic tongue
• Difficulty in speaking, nausea .
Diagnosis
• BMS is hard to diagnose , but we have
to review your medical history and make
intra-oral examination , beside some tests like :• Blood tests
• Oral swab tests
• Salivary flow test
• Biopsy of tissue
• Allergy test
Classification • Primary BMS: If tests do not reveal an underlying
medical problem, the diagnosis is primary BMS
• Secondary BMS: Certain medical conditions can cause BMS. Treating the medical problem will cure the secondary BMS.
Treatment • Low dose amitryptiline
• chlordiazepoxide 10-30 mg/day (start with 5 mg and increase the dose to 5 mg
every 4-7 days until oral burning is relieved )
• Clonazepam 0.5-1 mg. 2-3 times daily, for 4 days
• Alpha lipoic acid – as mouth rinse
