This is a very interesting article which holds potential for publication but needs revision. Please consider the following: 

There is a missed opportunity here to clearly explain and develop the background information about why this research is important.

“A number of contributing or confounding factors are also associated with pressure ulcers, the significance of these factors has yet to be elucidated.” The author(s) should

build on this, that even two important pressure ulcer organizations agree that the contributing or confounding factors are still unknown.

This is why this research is important- we are still looking at WHY pressure ulcers occur and their research is trying to provide evidence to support that microclimate

(skin temperature and moisture) are important contributing or confounding factors.

Microclimate is developing as a factor in pressure ulcer incidence and this manuscript represents one of the very few clinical studies that have

reported temperature changes as predictors of skin changes.

However the manuscript would be strengthened by adding more details of the investigation of the sheet used to control microclimate - this only appears in the discussion section and the experimental design and results

should be placed in the appropriate sections of the paper.

Do you have a photo of the ulcers on teh lower sacrum?

Are you describing the buttocks?

The sacrum is a bony prominence and your conclusion about the superficial ulcers not on bony prominences does not follow. 

Examine the influence of the synthetic sheet on moisture, temp, Braden subscales. This is an important component and needs more attention in the

paper.

Also describe the underpad, was it plastic? rubber? and correlate it to the outcomes.

• Comments to the Author This interesting manuscript reports microclimate changes and superficial skin damage. The manuscript could be strengthened through providing greater detail of the support surface and special sheet used in the study.

Answer:• Thank you very much for your suggestions,These beds

dimensionsare 190 x 90 x 13 cm (length, wide, and thick) construct from foam matress and covered by plastic. The hospital bed was covered by 100 % cotton sheet. In this study we also introduced synthetic fiber sheet randomly, theses sheet made by cotton 42 % and polyester 58 %.

• Comments to the Author Discuss how turning the patient was avoided during the data collection, turning would alter microclimate and your data was collected over 4 hours.

Answer:• Thank you very much for your review. In this study we didnt

avoid turning position for patient during the data collection, patient allowed to turning naturally or pasively by family or nursing. But if patient had been turned lateral we delay the measurement for two hours after supine position. Average collecting data for each patients about 30 minutes not 4 hours. 08.00 am until 12.00 am was our measurement interval time to avoid circadian rhytm.
