REPLACING ALL THE TEETH

Replacing all the teeth using dental implants has historically been a challenge due to the lack of bone available for implant placement or the proximity of vital structures that need to be avoided (the sinus cavities in the upper jaw and the nerve canal in the lower jaw).

Many patients are told they are not implant candidates or that they will need bone grafting prior to having dental implants placed. Bone grafting can be painful, costly and take several months or longer to heal.

Traditional implant techniques often call for the placement of 8-10 (or more) implants to replace an entire set of upper or lower teeth, and because the bridge only replaces teeth (not lost gum tissue) it can look unnatural and have a “horse tooth appearance”.

Will my teeth look natural?Absolutely! The All-on-4 dental implant technique gives you permanent teeth that look, feel and function just like real teeth … for life!! No one will know you have implants unless you tell them.

Why I get my permanent set of teeth only 3 days after my implants are placed?As with any surgery, you need time to heal. However, you will leave the country after 9 days of the surgery with your permanent teeth! Once the healing process is complete 3-days after, a multilayered set of teeth will be placed on a titanium bar and on your implants after three – four days of the surgery

What if I just need implants on my bottom arch? What if I someday need All-on-4 dental implants on both arches?All-on-4 implants can be placed on just the upper or lower arch or both. -- Dr. Freer and Dr. Saenz, your implant specialists, will evaluate the circumstances and if they feel you might eventually need replacement teeth on both the upper and lower arches, they might suggest performing the procedure on both arches at the same time. This provides better results while also saving time and money. They will work with you to determine what best meets your needs both health wise and cost wise.

What type of cleaning is required after an implant is placed? Dental implants should be treated like your natural teeth, daily brushing and flossing is recommended. Our team of specialists and dental hygienists will review oral hygiene instructions with you and will recommend the most appropriate oral hygiene treatment plan to fit your needs.

Are there any additional costs/fees to expect at the end of the treatment?It’s not our policy to add extra costs to our final quotation, however if during the first consultation show up some different oral situation, will be discussed during the consultation appointment with the patient.

FAQ - PROS and CONS

ACRYLIC:

Description: Typically a Titanium Milled (CAD/CAM) Bar Connects the four implants. Acrylic denture teeth are set on the bar, tried in for approval of esthetics, and the teeth are processed (connected to the bar) with a pink acrylic. So in essence, there are 3 layers: the bar, the teeth, and the pink acrylic gum that connects everything.

Prosthodontists needs to set the necessary room for all layers (14-16mm), otherwise the bridge will not fit in the space provided. Repairs /breaks on the acrylic hybrid All-on-4 bridge are a result of the minimized space.

PROS: Cost is Less (denture teeth come "stock,“ fabricated in a specialized factory).Easier to repair - Acrylic is softer, so there is a "give" to the bite, for grinders (Bruxism) they will slowly wear away the acrylic, similar to natural teeth. However, if the material is more stiff (Ceramic) a grinder can chip the restoration (too brittle). For this grinders must wear a night protection to sleep.

CONS: Staining more frequentDelamination (Debonding) of tooth from acrylic. Wear is faster, Higher repair; in a study in the Journal of Prosthodontics, 255 patients were followed up for an average of 15 years, and approximately 58% had one or more prosthetic events (i.e, chipped/broken tooth, loose implants screw) during that recall period. Overall survival rates at 20 years were 86% for prosthesis (bridge), 15% survived free of any event.

FAQ - PROS and CONS

ZIRCONIA:Description: A monolithic, high-strength material that is directly fitted to the 4 implants (without a titanium bar).

PROS:- Zirconia is Harder than Acrylic (for grinders they will wear away slower the zirconia compared to acrylic).- Can layer porcelain for more aesthetics, natural look (should be as minimal layering as possible)- No chance for tooth delamination/debonding (All one piece)- Can be used in limited space situations (12-16 mm of space)

CONS: - Cost is Higher (requires expensive CAD/CAM milling/sintering machinery, custom hand finishing and finset detailing)- Harder to repair (since the material is all one piece, if a "chip" occurs, the entire bridge needs to be replaced, OR a ceramic veneer can be glued to the front surface of the bridge as an alternative)- Because of the incredibly stiff nature of the zirconia there is a potential for failure from fracture of the entire zirconia bridge if used improperly.

• Zygomatic is the most advanced technique that exists nowadays in the market for UPPER JAW. It is a treatment normally done under special conditions and there is very few Maxillofacial Surgeons doing this kind of treatments, there is only a handful of them.

•The Zygomatic implant is an alternative to bone grafting in extremely resorbed maxilla.
