• From now on do not use this client profile form.
• The redetermination form you should all have experience with filling out, if not don’t hesitate to contact me with questions about it.
• This client profile has the data that has been uploaded from your Health Center. This is the preferred way to attach a redetermination form to client profile. Now, we will walk though the steps to do just that.
Health Center Records
Search for the child within your records. To do this you can search first and last name or date of birth. Any personal identifiers are good.

To change the columns that you see above click on properties and choose “Show/Hide Columns Window.”

Attaching a Redetermination Form to Client Profile
Click on box to open the profile
Then you will choose add a record and the redetermination form will a appear. File out the form. Once you have finished the form click update.
