CMS Transforming Clinical Practice Initiative: What is it?

• The TCPI is designed to help clinicians achieve large-scale health transformation • support >140,000 clinician practices over the next four

years in sharing, adapting and further developing their comprehensive quality improvement strategies.

• ACEP is one of 39 health care organizations selected to participate in the CMMI TCPI• One of 10 Support and Alignment Networks (SAN)

Support and Alignment Networks (SAN): What is it?

• SANs formed by professional associations and others that align their memberships, communication channels, continuing medical education credits and other work to support the PTNs and clinician practices.

• 4-year, $3 million grant to ACEP• Funds support project staff, clinical experts, data

infrastructure and analytics

A. Savings lives through improved sepsis care

B. Reducing avoidable care by supporting clinicians and patients around ACEP’s Choosing Wisely™ recommendations:1. reducing use of high-cost imaging for low back pain, 2. head CT scan after minor head injury, 3. chest CT for pulmonary embolus,4. abdominal CT for renal colic

C. Improving value of ED chest pain evaluation by reducing avoidable testing and hospitalization

CMS’s TCPI Goals for SANs• To assist clinicians in transforming their practices and commit themselves and their

communities of practice to quantifiable outcomes aligned with the overall aims of the TCPI model, including:

1. Support more than 150,000 clinicians in work to achieve practice transformation;

2. Build the evidence base on practice transformation so that effective solutions can be scaled, if successful;

3. Improve health outcomes for 5 million Medicare, Medicaid and CHIP beneficiaries.

4. Reduce unnecessary hospitalizations and other overutilization of other services for 5 million Medicare, Medicaid and CHIP beneficiaries;

5. Sustain efficient care delivery for Medicare, Medicaid and CHIP beneficiaries by moving at least 75% of clinicians that complete the TCPI phases of transformation to participate in incentive programs and practice models that reward value upon completion of TCPI;

6. Generate savings to the federal government over a period of 4 years through reduced Medicare, Medicaid and CHIP expenditures. In addition, we believe commercial payers may experience savings as a result of this model.
