Cardio-Respiratory and Critical Care Program

• The Coronary Care • The Pacemaker Program• The Heart Function Clinic• The Sleep Diagnostic Centre• The Pulmonary Function Lab• Respiratory Services

Maternal Newborn and ChildcareProgram• First hospital in Ontario to establish a

home-like birthing unit. • Family-centered model - 5000

deliveries between the General and Birchmount Campus

• Partnership with Toronto Public Health ensuring access to breastfeeding services

• Paediatrics – Supporting children and families with child life specialists including inpatient, day surgery and day clinic units.

• Paed Link – The implementation of a fast-track process for children and their families seeking care in the emergency department resulting in decreased wait times for the paediatric population.

Medicine and Geriatrics Program

• Social Work – Provides in-patient support, linkages to community services, discharge planning for patients leaving hospital.

• Geriatric Assessment and Intervention Network (GAIN) Clinic – Cross-discipline team assessment to provide support to frail seniors living in the community

• Palliative Care (focuses on relieving and preventing suffering of patients)

Oncology Program

• Provides cancer care treatment and consultation to Scarborough residents.

• Five highly-trained medical oncologists and a team of specialty-trained nurses provide high-quality care.

• The provision of oncology services at TSH means that Scarborough residents, some of whom currently have to travel downtown for their care, can receive the same level of care close to home.

Mental Health Program

• 82,000 visits per year in a range of languages.• Provides diagnosis, treatment, community reintegration and

psychosocial rehabilitation for patients suffering from severe and persistent major mental illness.

• Inpatient/outpatient, community programs and supportive housing service.

• Accessed by the Emergency department or through a psychiatrists affiliated with TSH.

Nephrology and Dialysis Program(Kidney)

• The Scarborough Hospital is home to the largest Nephrology Program in North America.

• Over 6,000 patients receiving care.

• It is one of 26 Regional Chronic Kidney Disease programs in Ontario

• The program has links with St. Michael’s Hospital for transplantation and linkages with various community program.

Diabetes Education Centre

• Goal - Enable patients to cope positively and gain independence in living with diabetes.

• Interdisciplinary team including, certified diabetes educators, nurses, dietitians, social workers, endocrinologists and chiropodists

• Program Components include: individual assessments and follow ups, group education program, gestational diabetes classes, diabetes-related equipment instruction

• Booking an appointment:Referrals from all physicians and self-referrals are accepted.

Emergency and Urgent Care• Two full-service Emergency Departments (General and

Birchmount campuses) offer around-the-clock urgent care. • Approximately 100,000 people are treated every year —

making TSH one of the busiest emergency departments in the country.

• In 2009, TSH opened its new Emergency Department at the General campus.

• Upon arriving in Emergency, all patients are assessed by a nurse, who places patients in priority based on the severity of their condition.
