21st Century Assessment.

Meet with your

case manager.

Follow the instructions in your

packet of orientation materials.

If you agree to participate in the study, your case manager will walk you through the required steps.

This assessment is used to help identify your skill level.

To be considered for participation in the EPIC Pilot Program you will need to enroll in the study. Here’s how:

Complete all required fields. Rank the training programs that interest you.

*If the information that has been filled in for you is incorrect please update your information.

If there is a suitable training provider available, you will be randomly assigned to the EPIC or SNAP E&T Services group.

After You Apply

ReviewYour application & assessments will be reviewed to determine if you qualify for EPIC services.

MeetBefore you leave today, schedule a follow up appointment with the DHS case worker.

AssignedWith your case worker, sign the consent form, and complete the registration for the EPIC study. This process may take up to 30 minutes.

Tammy, we have to fix this because this is the wrong order of things. You would have to complete the forms earlier for random assignment. the complete forms would come before the assigned box.

I'm a bit worried by this language. It might signal to theSNAP recipient that they will get training. There are a few problems here. If they area assigned to SNAP E & T group, they are not likely to get training. And if they are assigned to the community services group in the counties with no SNAP E & T they are really unlikely to get training.I might change this to:Review: your application and assessments will be reviewed.Meet: Before you leave today, schedule a follow-up appointment with the DHS case worker.Consent: Sign the consent document for the study. Complete the registration document with your case worker.Assigned: If there is suitable training available, you will be randomly assigned to the EIPIC or SNAP E & T services group (obviously this will need to be tailored for those counties w/o SNAP E & T). This process will only take a few minutes.
