The Blue Tie Affair:

Recognizing Our Children’s Heroes 2016 Nomination Form

Do you know someone who:

Serves as a volunteer coach?

Reads books to children at the local childcare center?

Wakes up extra early or stays super late to make sure your child is safe?

Organizes community efforts that support the well-being of children?

The Joint Base Myer-Henderson Hall Family Advocacy Program recognizes individuals in the National Capital Region who have gone above and beyond in the call of duty in their efforts to prevent child abuse and neglect in our community. Please nominate an individual for consideration. A committee from the installation will select final recipients from the list of nominees. Honorees will be announced at the Annual Blue Tie Affair, April 27, 2016, Spates Community Club (214 McNair Road (BLDG 407), Fort Myer, VA) at 1:30 PM. NOMINATION GUIDELINES

Deadline for nominations is April 8, 2016.

Nominations are open to the entire Community—all branches of the Military, Family members, DoD Civilians, Retirees, and community partners.

You may nominate more than one individual. Please complete a nomination form for each nominee.
