Academic Clearance

• Clearance to your program area – signed by program area advisor

• Medical Clearance – TB reading appointment

• Clearance to College of Education – placed in binder in Lobby of Dean’s Office – College of Education

• Sign clearance form in the Dean’s Office

• Give form to Laura Anne or slide under HER office door –404F

• Make copies of all forms to keep in your professional folder.

Criminal Background

Check

• Complete Criminal Background Check Form

• Give form to Laura Anne or Jean Edwards

• Complete on-line portion of CBC when prompted

• Complete county criminal background checks if

required
