Make sure you have the following materials on hand before you start packaging the test booklets and answer documents.

PREPARING MATERIALS1. Place Booklet 1 on top of

Booklet 2 for each student (Make sure the student identification label/barcode is affixed in the proper location on the back cover of each test booklet)

2. Alphabetize the class set of test booklets

3. Place the class roster sheet on top of the complete class set

4. Place the complete class set of test booklets with class roster in a large clear poly bag (Do not use rubber- bands or paper clips)

5. Alphabetize the class set of constructed response answer documents and place the purple header sheet on top

6. Place the alphabetized class set of answer documents with purple header sheet in a small clear poly bag (Do not use rubber-bands or paper clips)

7. Fill out the Test Security Sheet and School Group List

PLEASE NOTEAll math test booklets with student answers in languages other than English must be handled as follows:

– Each language must be in its own “pack” and appropriately labeled

– These “packs” should be placed on top of the class set

PACKING THE CARTON

FOR DELIVERY TO THE SCORING SITE

PACKING THE CARTON

1. Place the large empty manila DAA envelope in the bottom of the white/red shipping carton

2. Place the completed Security Sheet in the carton

3. Place the alphabetized class set/s of test booklets and answer documents in the carton

4. Place the School Group List on top of the test materials and close the carton (Do not seal with tape)

5. On the carton, fill in the Borough, District, School number and the name of the test (example: Borough: Q District: 84 School: 123 Test: NYS ELA Gr. 3 2010)

Place the test booklets and answer documents for only one grade in each carton.

The process for packaging the Grades 4, 6 and 8 Math and ELA test booklets and answer documents is similar to the steps outlined above.

Repeat the directions in slides 3-21.

When packaging the test booklets for these grades make sure you put Test Booklet 2 on top of Test Booklet 3.

School personnel must remain at the Crowne Plaza Hotel until the school’s scoring carton(s) are checked in by the scoring leader. They must also be available to correctly package the scoring materials if during the in-take process it has been determined that the materials were not properly packaged.
