The green checkmark indicates that this diplomate’s Medical License is valid and up to date. The Cognitive Examination does not require MOC Activities. The diplo-mate’s next MOC examination for the Subspecialty in Sleep Medicine is due in 2021. The MOC Activity Requirements and Annual MOC Registration Fees are in-complete.

On this page you can either add a new license record or update your license. You can proceed to add a new license record by clicking the “Add a New LicenseRecord” button. To update a license, click on the button under the “Select” column heading and then click the “Update Selected License” button.

The Total column refects the current progress in each category for Continuous Medical Education, Self Assessment, PIP Clinical Activity, PIP Feedback Activity.The green arrow will grow as more CME credits/PIP Activities are earned in the three year block.

You can also resort information in each column by selecting the up and down arrows in the column heading. Another feature is to select the drop down menu to sort by year.

You can proceed to add a new record by selecting the “Add New Record” button. To update a record, click the button under the “Select” column heading and click the “Update Record” button. To delete a record, click the button under the “Select” column heading and click the “Delete Record” button.

Page Summary: This is a general overview of Maintenance of Certification - Continuous Pathway to Lifelong Learning (CP-MOC). The “MOC Records” tab is high-lighted in yellow indicating where you are in the system. This page allows you to add or update MOC activity records. When adding an MOC activity record, you are required to complete the Activity Title, Sponsor Name and Date Completed. When updating a record, all the fields will already be populated giving you an opportunityto edit the information. You are also required to indicate the MOC Program component met by this activity.

The next step is to Indicate the MOC Program Component(s) met by this activity by entering the CME Credits and checking the appropriate check box(es) for SelfAssessment, PIP - Clinical Module or PIP - Feedback Module.

At the bottom of the page in the grey section, you can also View Tips for Managing MOC Activity Records, viewing Important Information about MOC Activity Dates, andviewing Important Information about PIP Activity Qualifications.

Page Summary: This is a general overview of Maintenance of Certification - Continuous Pathway to Lifelong Learning (CP-MOC). The “Registration Fee Payment” tabis highlighted in yellow indicating where you are in the system. This page allows you to view the Annual MOC Registration Fee for your three year MOC RequirementCycle. You can see in this example for Year 1 - 2013, the diplomate has an Annual Fee of $175.00 with a payment received on 1/7/2013. For Year 2 - 2014 and Year 3- 2015, the same annual fee will be due in each cycle year.

The Annual MOC Registration Fee is due by December 31 of the cycle year. Payment must be received by the due date or you will be charged a late fee. Payment for multiple years within a cycle can also be made.
