• August 1, 2009 for returning students

Plan Year

• Returning students will have a 13 month plan– August 1, 2009 – August 31, 2010

• First plan year only

• New students for 2009-2010 school year– September 1, 2009 – August 31, 2010

Eligibility:All Residential Students Who…

• Are taking 7 or more credit hours

• Are on a vicarage

• Deaconess students during internship

• Are in an approved foreign exchange program

Enrollment

• Enrollment is mandatory for all residential students unless they qualify as an eligible opt-out

• Dependent enrollment is optional

Payment

• Coverage is paid by the student– Will be billed by the seminary

Rates

• Self only $ 227 / month

• Self & spouse $ 535 / month

• Self & children $ 512 / month

• Family $ 819 / month

Coverage Continuation

• Can continue up to 6 months

• Can continue up to 12 months if the student is writing a dissertation

• Can continue up to 12 months if the student is waiting for a call

• Enrollment and coverage continues over the summer months whether or not the student is attending classes

• Plan Year Deductible – $ 500 individual / $ 1,000 family

• Coinsurance: 20%• Plan Year Coinsurance maximum

– $ 3,000 individual / $ 6,000 family

• Preventive Care: $ 0• Office Visit: $ 25• Hospital: 20% after deductible is met• Emergency Room visit: $100 copay (waived if admitted)

Short-term drugs Retail – up to 30-day supply• Generic $ 10• Formulary $ 25

• Non-formulary $ 50

Long-term drugs Preferred Home Delivery Maintenance – up to 90-day supply• Generic $ 20• Formulary $ 25• Non-formulary $100

• Preventive Care (covered 100%) $ 0• Diagnostic Care (covered 100%) $ 0• Annual Deductible $ 50 / person• Maximum Benefit in Plan Year $ 1,500• Basic Restoration in Plan Year 20% after deductible• Major Restoration in Plan Year 50% after deductible• Orthodontia for 50% with lifetime

dependent children maximum of $ 1,250
