DEFINITIE
• ulcer = localized defect of the surface of the oral mucosa in which the covering epithelium is destroyed, leaving an inflamed area of exposed connective tissue
• ulcus = door weefselverval ontstaan oppervlaktedefect van de huid of de mucosa met geringe neiging tot genezing
(van der Waal, 2013)

Erosion is a clinical term that describes a soft tissue lesion in which the skin or mucosa is denuded (i.e., the epithelium is worn away or destroyed).
Erosions are moist and slightly depressed and often result from a broken vesicle, epithelial break· down, or trauma. In the eroded area, the epithelium above the basal cell layer (layer above the connective tissue or dermis) is lost. Healing rarely results in scarring because the basal layer of epithelium remains intact. Pemphigus, erosive lichen planus (desquamative gingivitis), and erythema multiforme are diseases that produce mucocutaneous erosions

An ulcer is a craterlike lesion of the skin or oral mucosa. It is the term used to describe an uncovered wound of cutaneous or mucosal tissue that exhibits gradual tissue disintegration and necrosis. The border of a mucosal ulcer is often round but can be irregular. Ulcers extend deeper than erosions, from beyond the basal layer of the epithelium into the dermis (connective tissue). Scarring may follow healing of an ulcer. Ulcers may result from trauma; aphthous stomatitis; infection by viruses, such as herpes simplex, variola (smallpox), and varicella-zoster (chickenpox and shingles); cancer; or granulomatous disease. Ulcers are usually painful and often require topical or systemic drug therapy for effective management.

A bulla is a fluid-filled blister > 1 cm in size. This condition develops from the accumulation of fluid in the epidermal-dermis junction or a split in the epidermis. Accordingly, the surface is smooth and dome shaped and easily ruptured by the slightest of trauma. Because of their size, bullae represent a more severe disease than do conditions associated with vesicles. Bullae are commonly seen in pemphigus, pemphigoid, burns, frictional trauma, Stevens-Johnson syndrome, and epidermolysis bullosa. bullous lichen planus: rare condition
pustula: vesicle filled with purulent exudate
A pustule is a circumscribed elevation filled with pus- a purulent exudate consisting of a mixture of inflammatory cells and liquidresulting from an infection.
Pustules are < 1 centimeter in diameter and may be preceded by a vesicle or papule.
They are creamy white or yellowish and are often associated with an epidermal pore (i.e., pimple) or sweat gland.
Within the mouth, a pustule is represented by a pointing abscess or parulis. Herpes zoster also produces pustules that eventually ulcerate and cause intense pain.

– antistoffen op cellen van orale mucosa • normale spiegel ANF en complement
– een lokale immunologische afwijking • verhoogde titer antistofen tegen streptococcen
– bevorderende factoren • trauma • psychische : stress ( stud 55% ) • hormonale invloed: afname zwangerschap / pil • toename premenstreel • familiaal / allergie/ asthma/ hooikoorts
RECIDIVERENDE ULCERATIES: AFTEUZE STOMATITIS • symptomen
– pijnlijke erosie van 3 mm. – rode halo / scherp omgrensd – geelgrijs beslag – vesikel nooit waargenomen – pijnlijk – solitair of multieple – geneest na één week – geen litteken
• behandeling – geen ideaal product – caustische agentia: trichloorazijnzuur – corticosteroïd : locaal, regionaal, algmeendspeoling
met tetracyclinen oplossing – vit B complex

RECIDIVERENDE ULCERATIES: syndroom van Behçet
• ooglaesies :
– slechts in een deel van de gevallen
– vele manifestaties:
• scleritis, keratitis, conjunctivitis
• anterior of posterior uveitis
• hypopion
• iridicyclitis
• optische neuritis
– dit kan tot blindheid leiden
RECIDIVERENDE ULCERATIES: syndroom van Behçet

– gastro-intestinaal

• arthritis is een tamelijk constante manifestatie bij 50 – 60 % van de patiënten met Behçet

verhemelte en verhemeltepijler – algemene symptomen
• keelpijn dysfagie • lichte koorts • hoofdpijn
– geneest na enkele dagen • behandeling: niet nodig: R/symptomatisch
ERYTHEMA EXSUDATIVUM MULTIFORME (EEM) syndroom van STEVENS JOHNSON
• etiologie pathogenese: huidziekte : meerdere uitlokkende factoren – medicaties – sulfamiden e.a. – infecties: vooral herpes – zeldzame factoren:
• radiotherapie, • maligne tumoren • idiopathisch
• acute cutane vasculaire aandoening
– type III immuuncomplex pathologie – type IV overgevoeligheid – trage

ERYTHEMA EXSUDATIVUM MULTIFORME (EEM) syndroom van STEVENS JOHNSON
• symptomen:
– papulaire letsels op de rugzijde van de handen & voeten
– letsels worden groter en talrijker en breiden uit naar proximaal
– soms acuut met onstaan van koorts, spierpijn, infectie van de bovenste luchtwegen
ERYTHEMA EXSUDATIVUM MULTIFORME (EEM) syndroom van STEVENS JOHNSON
• symptomen:
– idiopatische vorm verloopt over 4 tot 6 weken en is zelf-limiterend, maar met soms frekwente recidieven gedurende jaren vooral in lente en in herfst
– veelheid van huid- en mucosalaesies: bullae, maculae, erosies, papels
ERYTHEMA EXSUDATIVUM MULTIFORME (EEM) syndroom van STEVENS JOHNSON
• symptomen:
– in de huid een wielvormig letsel met verheven perifere rand, een ingedrukte binnenste ring, en centraal een erythemateuze vlek, of een met vocht gevulde blaar: de zogenaamde” iris”, of “cocarde” of “schietschijf” laesie
– intra-oraal barsten de blaren vlug: erosies met onderliggend erytheem
ERYTHEMA EXSUDATIVUM MULTIFORME (EEM) syndroom van STEVENS JOHNSON
• symptomen: de meest ernstige vorm is gekend als “Stevens Johnson” / “Leyel” syndroom – bij jonge personen en mannen – mucosaletsels staan op de
voorgrond – gans het mondslijmvlies is
aangetast met erythemateuze laesies: de lippen zijn bedekt let een hemorhagisch exsudaat, en dit gaat over naar zwarte bekorsting. Uitgebeide erosies van de tong, het wangslijmvlies, en de pharynx maken inname van voedsel uitermate moeilijk
ERYTHEMA EXSUDATIVUM MULTIFORME (EEM) syndroom van STEVENS JOHNSON
• symptomen:
– ook andere mucosae zijn aangetast: bulbaire en palpebrale conjunctiva, uitwendige genitalieën,( glans penis, vulva) en neusgaten
• zonder behandeling is het beloop fataal

SYNDROOM VAN REITER
• etiologie : nog onzeker
– gelijkenis met gonorroea
– abnormale immunreactie tegen micro-organismen
– bij 75 à 95 % - is HLA B27 antigen aanwezig
– correlatie is hoger bij doorgemaakte shigella infectie: bij HLA- B27 patienten met zulke infectie is Reiter aanwezig in 25 %
– seksueel overgebrachte Chlamydia en mogelijk Mycoplasma spelen mogelijk een rol.

– mannen : 18-45 jaar
– blanken > rassen met donker huidtype

– binding van een fluorescerend A.L. op een AG-AL complex.
– toepassing:
• Pemphigus vulgaris,
• Pemphigoïd,
• Lymfoma’s (T- en B-cel)
PEMPHIGUS VULGARIS
• blaarvormende dermatose
• dodelijk vòòr de tijd van de corticosteroïden
• vroegtijdige diagnose door tandarts en MKA: in 50% van de gevallen zal de orale aantasting éérst optreden
• de huid is bij deze patienten ook uitvoerig aangetast
• de dosis steroïden kan bij vroegbehandeling lager blijven
PEMPHIGUS VULGARIS
• etiologie
– is een auto-immuunziekte
• symptomen
– grote blaren of diffuse gelatineuze plaques
– blaren barsten snel
– grote pijnlijke erosies & erytheemvlekken
– teken van Nikolsky: wrijven op
aangetaste mucosa of huid doet blaren ontstaan en vergroten
PEMPHIGUS VULGARIS
histopathologie
• immuunfluorescentieonderzoek toont immuuglobulines ( antistoffen) tegen de intercellulaire substantie
• blaarvormingen zijn intra-dermaal
• zeer moeilijk om een gave blaar te biopteren om dit aan te tonen
• immuunfluorescentie toont een typische honigraat patroon in het epiderm veroorzaakt door het vastzetten van antistoffen op de intercellulaire brugjes
PEMPHIGUS VULGARIS

voorkomen:
• op oudere leeftijd
• vrouwen > mannen

• symptomen
– slijmvliezen > huid
– weinig aantasting van de huid
– vooral mondmucosa en conjunctiva
– erosies op de gingiva: desquamatieve gingivitis
– littekenvorming
– fibreuze banden in de conjunctiva
BENIGNE SLIJMVLIES PEMPHIGOID = cicatricieel pemphigoïd= parapemphigus
• evolutie:
– niet dodelijk / slepend verloop
– entropion kan tot bindheid leiden
cicatricieel pemphigoïd ernstige oogletsels
BENIGNE SLIJMVLIES PEMPHIGOID = cicatricieel pemphigoïd= parapemphigus
histopathologie:
• sub- epitheliale blaren
• met intacte basale cellaag als dak
• immuunfluorescentie : antistoffen IgG en C3 tegen de basale membraan: lijnvormige tekening aan de onderzijde van het epiderm.
• bij serologisch onderzoek : geen circulerende antilichamen tegen de basale membraan aantoonbaar.
BENIGNE SLIJMVLIES PEMPHIGOID = cicatricieel pemphigoïd= parapemphigus
behandeling:
• steroïd toegediening tot de laesies onder controle zijn ; nadien geleidelijke afbouw en stop
• desquamatieve gingivitis: topische steroïden
• weinig aantasting van de huid

BULLEUS PEMPHIGOID
• heeft minder belang in de mondpathologie
• bij 1/3 patiënten bestaan afwijkingen in de mond
• man/vrouw = 1/1
• auto-antilichamen tegen basale membraan bij 70 %: +
• direct immunofluorescentie onderzoek idem als bij cicatricieel pemphigoïd.
