INATTENTIVE SYMPTOMS
1) Six (or more) of the following have persisted for at least 6 months to a degree that is inconsistent with developmental level and negatively impacts directly on social and academic/occupational activities:
a. Fails to give close attention to details or makes careless mistakes.
b. Has difficulty sustaining attention.
c. Does not appear to listen.
d. Struggles to follow through on instructions.
e. Has difficulty with organization.
f. Avoids or dislikes tasks requiring a lot of thinking.
g. Loses things.
h. Is easily distracted.
i. Is forgetful in daily activities.

HYPERACTIVE/IMPULSIVE SYMPTOMS
2) Six (or more) of the following have persisted for at least 6 months to a degree that is inconsistent with developmental level and negatively impacts directly on social and academic/occupational activities:
a. Fidgets with hands or feet or squirms in chair.
b. Has difficulty remaining seated.
c. Runs about or climbs excessively in children; extreme restlessness in adults.
d. Difficulty engaging in activities quietly.
e. Acts as if driven by a motor; adults will often feel inside like they were driven by a motor.
f. Talks excessively.
g. Blurts out answers before questions have been completed.
h. Difficulty waiting or taking turns.
i. Interrupts or intrudes upon others.

Developmental considerations
Always think: relative to same-age peers!
• Preschool – normal is very broad. What else could be
going on?? Need a good developmental assessment.
Rule out sensory issues, language delays, etc. But it IS
possible to diagnose in preschool.
• Elementary – early years – what are the demands? Are
they realistic and developmentally appropriate?
• High-School – Can be complicated by other things.
Don’t overlook mood issues – not everything is ADHD!
Watch for more inattentive, more restless. For individuals
17 and above only 5 or more symptoms are needed.
• Adulthood – executive skills, restless. Yes, it persists in the
majority of cases, but the functional impact may be
less.7

• Oral instructions simple and short
• Lessons brief
• Allow time for processing and rehearsal
• One task at a time (not notetaking and listening)
• Quiet environment
• Direct instruction with repetition. Repetition. Repeat
• Nonessential, irrelevant or confusing info omitted
• Present steps just before they are needed
• Present visually and verbally
• Be organized and clear.

• Frequently misspelled words
• Classroom rules

Classroom Accommodations
Barkley’s 9 principles
1. Rules and instructions provided to children with ADHD must
be clear, brief, and often delivered through more visible and
external modes of presentation than is required for the
management of non-ADHD children.
2. Consequences used to manage the behavior of children
with ADHD must be delivered swiftly and more immediately
than is needed for non-ADHD children.
3. Consequences must be delivered more frequently, not just
more immediately, to children with ADHD in view of their
motivational deficits.
4. The type of consequences used with children with ADHD
must be of a higher magnitude, or more powerful, than that needed to manage the behavior of non-ADHD children

5. An appropriate and often richer degree of incentives must be provided within a setting or task to reinforce appropriate behavior before punishment can be implemented.
6. Those reinforces or particular rewards which are employed must be changed or rotated more frequently with children with ADHD than with non-ADHD children given the penchant of the former for more rapid habituation or satiation to response consequences, apparently rewards in particular.
7. Anticipation is the key with children with ADHD.
8. Children with ADHD must be held more ‘publicly’ accountable for their behavior and goal-attainment than non-ADHD children.
9. Behavioral interventions, while successful, only work while they are being implemented and, even then, require continued monitoring and modification over time for maximal effectiveness.

Behavior Management
• Behavior modification; NOT traditional talk therapy
or play therapy
• Goal is to get child doing more desired behavior
and less undesired behavior – but do NOT use to
bully the child
• Use rewards and punishments – but mostly rewards
• Put in terms of ABC’s
– Antecedents – things that happen before
– Behaviors – things the child does
– Consequences – things that happen after the behavior
• Adults learn to change A’s and C’s

• Parent and teacher interventions should be carried
out at the same time with communication between
the two
• Five points to incorporate into both components:
– Start with goals that the child can achieve in small steps
– Keep it simple – don’t try to change too much at once
– Be consistent – across times of day, settings, people
(specials teachers, too)
– Implement behavior interventions over the long haul – not just a few months
– Expect to have to freshen-it-up regularly

• Target behaviors can be either negative behaviors
that need to stop OR new skills that need to be
developed
• The areas targeted will typically not be the
symptoms of ADHD – but rather the specific
problems that those symptoms cause in every-day
life
– “completes assigned work with 80% accuracy”
– “plays well with siblings”
– “does what he is told first of second time told it”

Conclusions
“The treatment of ADHD requires a comprehensive behavioral, psychological, educational, and sometimes
medical evaluation followed by education of the individual or their family members as to the nature of the
disorder and the methods proven to assist with its management. Treatment is likely to be multidisciplinary,
requiring the assistance of the mental health, educational, and medical professions at various points in its course. Treatment must be provided over long time periods to assist those with ADHD in the ongoing management of
their disorder. In doing so, many with the disorder can lead satisfactory, reasonably adjusted, and productive lives.”

• Medication is most effective stand-alone treatment
• Evidence stronger for parent or teacher administered behavior therapy in school-age children than in teens
• Appropriate school-based accommodations, special education services as indicated
• Need to address other comorbid conditions
• Behavior therapy recommended as initial treatment in preschoolers

• Restrictive diets: helpful for subset of kids with ADHD and food allergies

Implications of Arnold Study:
• Patients with uncomplicated ADHD should try another stimulant from the alternate category if they fail an initial trial.
• Sub-optimal responders (improved, but not normalized) to a stimulant may also benefit from a trial of a different stimulant from the alternate category.

Dosing issues:
• Start with small doses, increase in small, weekly increments
• One week sufficient to assess response
• Poor ”community care” response in MTA often attributed to insufficient dosing
• Beneficial effects, side effects are dose-dependent
• Differences in response common with generics considered “equivalent” by FDA
• Stimulants aren’t dosed on the basis of weight…non-stimulants are.
• Response to stimulants is highly individualized

ADHD and Substance Abuse
• ADHD is a significant risk factor for substance abuse disorders in teens, adults
• Medication during adolescence may reduce risk, but protective effects disappear by age 25

• Recommended by many pediatricians, especially during summer months
• Driven by concerns that kids aren’t eating enough, achieving expected growth
• Often driven by difficulty tolerating stimulant-related side effects
• Does ADHD significantly impact functioning in one or more major life domains (school/work, family, friends, community) during weekends, vacation?

Managing stimulant-related side effects:
• Appetite/weight: • Eat Breakfast• Encourage small snacks throughout day • Bedtime snacks replace missing lunchtime calories
• Sleep: • Limit access to electronics before bed• Melatonin (3-6 mg, two hours before bedtime)• Consider adjusting duration of medication coverage• Adjunctive clonidine
• Irritability: • Evaluate for comorbid conditions• Consider “dose-sculpting” in late afternoon• Consider alternative medication
• Most stimulant related side effects resolve within three months of treatment initiation
