Intake, Information & Referral If you need help finding the right service in your community or within the Clermont County Board of Developmental Disabilities, contact our Information and Referral Division: Intake/Eligibility Clermont County Board of DD 2040 US Highway 50 Batavia, OH 45103 (513) 732-4831 [email protected] If you are specifically interested in services for a child under the age of 3, you may also contact the Early Intervention office:

Clermont Early Intervention 2040 US Highway 50 Batavia, OH 45103 (513) 732-5030 (513) 732-5031 (fax) [email protected]

Early Intervention (EI) Services are provided by Clermont County Developmental Disabilities Services for children under the age of three. This program follows guidelines and/or rules from the Ohio Department of Developmental Disabilities, from Part C of I.D.E.A., as well as rules from the Ohio Department of Health Help Me Grow system.

Once your child is determined eligible for services, an Early Intervention Team will support you and your family. The team includes a Service Coordinator, Physical Therapist, Occupational Therapist, Speech Therapist, Early Intervention Specialist and if needed specialist in hearing and vision.

One member of the team will be assigned as a Primary Service Provider (PSP). This professional will visit you and your child regularly at home or other community settings. At each visit, your PSP will help to address your questions and priorities related to your child’s development. Together you will find ways to support your child during everyday activities. The PSP will consult other team members as needed, and they may join the PSP on visits with you.

Is my child eligible for services?

An infant or toddler under the age of three is eligible for services from Clermont County DD Services if he or she has developmental delays in one of the following areas:

• self-help skills

• physical development skills (fine gross and motor skills)

• language, speech, hearing or communication skills

• cognitive skills

• social/emotional development An infant or toddler may also be eligible if he or she is at risk for developmental delay due to medical issues.

School Age Services

Pre-Employment Transition Services

Clermont County Board of Developmental Disabilities School Age Program provides educational services to students (ages 6 through 21) with intensive cognitive/medical/physical needs, multiple disabilities, significant behaviors, and autism spectrum disorders.

Student placement is determined by the IEP team along with the LEA (Local Education Agency) as an educational option on the continuum of services offered in Clermont County. Students must be enrolled in their local school district.

Instruction includes functional daily living skills, adaptive behavior, vocational skills and recreation/leisure skills. Additionally, students may receive related services such as speech and language therapy, occupational therapy, physical therapy and/or nursing services as determined by the IEP. All students participate in adapted physical education which includes aquatics.

The vision of the School Age Program is to provide a safe learning environment for students to actively participate in the educational process and provide students the opportunity to achieve their fullest potential.

Jody Bailey, Principal (513) 732-4970 [email protected]

Pre-Employment Transition Services is a partnership between the Ohio Department of Developmental Disabilities and Vocational Rehabilitation. Those services are: Job Exploration/Counseling, Work Based Learning, Counseling on Post Secondary, Work Readiness Training, Instruction on Self Advocacy.

The mission is to enhance students’ vocational knowledge and skills, better enabling them to become productive and as work-ready as possible upon graduation.

Transition Summer Work Youth programs include working at businesses such as: Clermont County YMCA, Coney Island, Big Lots, Tractor Supply Company, Grants Farm, UC Clermont. These of course are paid work experiences.

Our summer Career Exploration Program focuses on soft skills for students who are new to learning about vocational expectations and responsibilities. Gift Of Time (GOT) Respite Cooperative

Community Support Services

Kim Wynn, SSA Supervisor 2040 US Highway 50 Batavia, OH 45103 (513) 732-4850 [email protected]

Community Support Services assists individuals and their families to choose services that will best meet their needs within a given budget. A Service and Support Administrator or SSA is assigned to individuals receiving Medicaid waiver services and/or services through local funds to help coordinate their services. The SSA helps individuals and others close to them create a service plan based on their needs and goals. The service plan identifies each service the individual will receive along with the name of the service provider they have chosen. Services may include day services, community employment, residential supports, self-advocacy activities, community activities and transportation. The SSA can help the individual or family access information about service options and available service providers.

The SSA is responsible for getting the written plan and any updates to the individual, guardian or family and all service providers in a timely manner. The SSA maintains contact with the individual, family and providers throughout the year to ensure that services are delivered according to the plan. The plan may be changed or adjusted at any time and is reviewed at least annually.

Monica May, SSA Supervisor 2040 US Highway 50 Batavia, OH 45103 (513) 732-4844 [email protected]

Clermont DD has incorporated the Gift of Time Respite Cooperative Program into its many valuable and outreaching programs. The “co-op” is essentially families helping families. Participating families or caregivers become respite members and volunteer their time. This “gift of time” allows members to earn respite points.* Individuals qualified for Clermont DD services and their siblings may attend respite.

Respite events are held twice per month, giving caregivers the opportunity to run errands, grocery shop, or spend time with others in the household.

The GOT Respite Cooperatives is funded through Clermont DD’s yearly fundraisers, as well as through grants, foundations, and other fundraisers. Time is your only expense.

Self Advocacy at Clermont DD

Resources, Websites and Telephone Numbers

Clermont County Turn Up the Voices is the self advocacy group that works in partnership with the Clermont County Board of Developmental Disabilities.

Self advocates exercise their rights as citizens by communicating and representing themselves. They have a say in decision-making in all areas of their daily lives and in public policy decisions that affect them.

The October Podcast from Clermont DD was all about self advocacy.
