There are three Questionnaires in this document:
1. The main Questionnaire is set out on pages 3 - 33 and is
for completion by or on behalf of all residents aged
18 and over. Don't forget to fill in the Prize Draw
form on page 33!
2. If you are aged between 11 and 17 please complete the
Questionnaire set out on pages 35 - 36. If you want
to be included in the Prize Draw then please fill in
the form at the end of page 36.
3. If you are under 11 and would like to give your views as
well as enter the Picture Competition, please go to
pages 37 and 38.
