Section 1 IntroductionThis directory provides a list of Keystone 65 Preferreds network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.
How should you use this directory?You should use this directory to select a primary care physician. You must choose a network physician to be your primary care physician. Your primary care physician will coordinate all specialty care for you, and will refer you to a specialist for all specialty care visits. Your primary care physician will give you a referral if you need to seek care from a network specialist.
How does a PCP work in an HMO plan? You will have to choose one of our network providers who are listed in this directory to be your Primary Care Physician (PCP). The term PCP will be used throughout this directory. Generally, you must get your health care services from your PCP. When you become a member of Keystone 65 Preferred, you must choose a network physician to be your PCP. Your PCP is a physician who meets state requirements and is trained to give you basic medical care. For information on how PCPs are paid, see the How are providers reimbursed? section.
The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non-network providers.
In most situations, your network PCP must give you approval in advance before you can use other providers in the plans network, such as specialists, hospitals, skilled nursing facilities, or home health care agencies. This is called giving you a referral. Your PCP arranges referrals to other providers within the network. Members have a choice of specialists, with PCP referral when required; however, members must receive care for routine X-ray facilities, laboratory, physical therapy, and occupational therapy at the PCPs designated site. Covered services that need prior authorization (approval in advance) are marked in the Benefits Chart in your Evidence of Coverage.
How do you get care from a non-contracting provider?Please refer to Chapter 4 of the Evidence of Coverage for more information on covered health services that you get from non-contracting providers. If a non-contracting provider submits a bill directly to you and asks you to pay for covered services you received from the non-contracting provider, you should not pay the bill. Instead, please submit the bill to Keystone 65 Preferred for processing and determination of your liability, if any.
KeywordsNetwork providersProvider is the general term we use for doctors, hospitals, and other health care professionals and facilities that are licensed or certified by Medicare and by the state to provide health care services. We call them network providers when they accept our payment as payment in full, and in some cases to coordinate as well as provide covered services to members of our plan.
Evidence of Coverage (EOC) and Disclosure InformationThis document, along with your enrollment form and any other attachments, riders, or other optional coverage selected, explains your coverage, what we must do, your rights, and what you have to do as a member of our plan.
Primary Care Physician (PCP)Your primary care provider is the doctor or other provider you see first for most health problems. He or she also may talk with other doctors and health care providers about your care and refer you to them. In many Medicare health plans, you must see your primary care provider before you see any other health care provider.
Covered servicesThe general term we use to mean all of the health care services and supplies that are covered by our plan.
ReferralA written or electronic order provided by your primary care physician that approves you to see a plan specialist or to obtain additional services. A referral is used in advance of the services received or the specialist visited.
Prior AuthorizationApproval in advance to get services or certain drugs that may or may not be on our formulary. Some in-network medical services are covered only if your doctor or other network provider gets prior authorization from our plan.

It is best to ask a non-contracting provider to bill Keystone 65 Preferred first, but if you already paid for the covered services, we will pay you for our share of the cost. If you get a bill for the services, you may send the bill to Keystone 65 Preferred for payment. We will pay the non-contracting provider for our share of the bill and will let you know what, if anything, you must pay. You wont have to pay a non-contracting provider any more than what the non-contracting provider would have received from you if you had been covered under the Original Medicare plan.
What is the process for obtaining emergency services?You may get covered emergency medical care whenever you need it, anywhere in the United States or its territories. For more information, see the Medical Benefits Chart in Chapter 4 of the Evidence of Coverage. Call the plan for details if traveling outside the United States. If you have an emergency, we will talk with the doctors who are giving you emergency care to help manage and follow up on your care. The doctors who are giving you emergency care will decide when your condition is stable and the medical emergency is over. After the emergency is over, you are entitled to follow-up care to be sure your condition continues to be stable.
Sometimes it can be hard to know if you have a medical emergency. For example, you might go in for emergency care thinking that your health is in serious danger and the doctor may say that it wasnt a medical emergency after all. If it turns out that it was not an emergency, as long as you reasonably thought your health was in serious danger, we will cover your care.
Urgently needed care is a non-emergency, unforeseen medical illness, injury, or condition that requires immediate medical care. Urgently needed care may be furnished by in-network providers or by out-of-network providers when network providers are temporarily unavailable or inaccessible. The condition could, for example, be an unforeseen flare-up of a known condition that you have. When you are outside the service area and cannot get care from a network provider, our plan will cover urgently needed care that you get from any provider at the in-network cost-sharing amount. Our plan also covers urgently needed care outside of the United States and its territories. See Chapter 4 of your Evidence of Coverage (Medical Benefits Chart (what is covered and what you pay)) for more information. Call the plan for details if traveling outside the United States. Please note that non-urgently needed care is not covered outside the United States.
You must use plan providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers, neither Medicare nor Keystone 65 Preferred will be responsible for the costs.

Original Medicare Original Medicare is offered by the government, and not a private health plan like Medicare Advantage Plans and prescription drug plans. Under Original Medicare, Medicare ser-vices are covered by paying doctors, hospitals, and other health care pro-viders payment amounts established by Congress. You can see any doctor, hospital, or other health care provid-er that accepts Medicare. You must pay the deductible. Medicare pays its share of the Medicare-approved amount, and you pay your share. Original Medicare has two parts: Part A (Hospital Insurance) and Part B (Medical Insurance) and is avail-able everywhere in the United States.
EmergencyA medical emergency is when you, or any other prudent layperson with an average knowledge of health and medicine, believe that you have medi-cal symptoms that require immediate medical attention to prevent loss of life, loss of a limb, or loss of function of a limb. The medical symptoms may be an illness, injury, severe pain, or a medical condition that is quickly getting worse.
Emergency CareCovered services that are: 1. rendered by a provider qualified to
furnish emergency services; and2. needed to treat, evaluate, or stabilize
an emergency medical condition.
Urgently needed servicesUrgently needed services are care provided to treat a non-emergency, unforeseen medical illness, injury, or condition that requires immediate medical care. Urgently needed ser-vices may be furnished by network providers or by out-of-network provid-ers when network providers are tem-porarily unavailable or inaccessible.
Out-of-network providerA provider or facility with which we have not arranged to coordinate or provide covered services to members of our plan. Out-of-network providers are not employed, owned, or operated by our plan or are not under contract to deliver covered services to you.

What is the service area for Keystone 65 Preferred?The counties in our service area are: Bucks, Chester, Delaware, Montgomery, and Philadelphia counties in Pennsylvania.
How do you find Keystone 65 Preferred providers in your area? The directory contains a list of primary care physicians (PCPs), specialists and pharmacies in the Keystone 65 Preferred network in alphabetical order by county then by city. You should use the Table of Contents to locate the type of provider you are looking for. If you already know the name of the provider, you can locate the provider in the Alphabetical Index. The Alphabetical Index will then refer you to the page on which the provider is listed. If you do not know the name of the provider, you can begin your search by checking the full list of providers in your area. Be sure to ask the provider if they participate in the Keystone 65 Preferred plan.
You must choose a network physician to be your PCP. Your PCP will coordinate all specialty care for you. If you already know the name of the PCP you would like to choose, you can locate the PCPs name or group practice name in the Alphabetical Index. The Alphabetical Index will then refer you to the page on which the PCP is listed. If you do not know the name of the PCP you would like to select, you can begin your search by checking the list of PCPs in your area. Before selecting your PCP, please call the providers office to see if the provider is accepting new patients. Once you have chosen a PCP, include the PCPs name or group practice name and provider identification number (also referred to as group ID) on the enrollment form.
How do you find a pharmacy?This booklet provides a list of Keystone 65 Preferreds network pharmacies. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and Keystone 65 Preferred Rx (HMO)s formulary.
When this pharmacy directory refers to we, us, or our, it means Keystone Health Plan East. When it refers to plan or our plan, it means Keystone 65 Preferred Rx (HMO).
We call the pharmacies on this list our network pharmacies because we have arranged with them to provide prescription drugs to plan members. In most cases, your prescriptions are covered under Keystone 65 Preferred only if they are filled at a network pharmacy or through our mail order pharmacy service. Once you go to one pharmacy, you are not required to continue going to the same pharmacy to fill your prescription but can switch to any other of our network pharmacies. We will cover prescriptions filled at non-network pharmacies under certain circumstances as described in your Evidence of Coverage.
Not all network pharmacies may be listed in this directory. Pharmacies may have been added or removed from the list after this directory was printed. This means the pharmacies listed here may no longer be in our network, or there may be newer pharmacies in our network that are not listed. This list is current as of June 2016. Service areaA geographic area where a health plan accepts members if it limits membership based on where people live. For plans that limit which doc-tors and hospitals you may use, its also generally the area where you can get routine (non-emergency) services. The plan may disenroll you if you permanently move out of the plans service area.

You can go to all the pharmacies on this list, but your costs for some drugs may be less at pharmacies in this list that offer preferred cost-sharing. We have marked these pharmacies with P to distinguish them from other pharmacies in our network that offer standard cost-sharing.

How are providers reimbursed?Our HMO reimbursement programs for health care providers are intended to encourage the delivery of quality, cost-effective care for our members. Below is a general description of our HMO reimbursement programs, listed by type of participating health care provider. Please note that these programs may change from time to time, and the arrangements with particular providers may be modified as new contracts are negotiated. If after reading this material you have any questions about how your health care provider is compensated, please speak with the provider directly, or contact us.
Primary Care Physicians Most PCPs are paid in advance for their services, receiving a set dollar amount per member per month for each member selecting that PCP. This is called a capitation payment, and it covers most of the care delivered by the PCP. Services not included under capitation are paid fee-for-service according to the HMO fee schedule. Many PCPs are eligible to receive additional payments for meeting certain quality performance and medical cost management measures relative to other qualifying participating PCP offices.
In addition, primary care physicians can earn supplemental reimbursement through an incentive program called Quality Incentive Payment System (QIPS). Independences QIPS functions as a critical component to the Independence primary care compensation offering and gives eligible participating primary care practices incentives for providing quality and efficient care. Through the QIPS program, by performing well against the program measures, primary care physicians can earn an incentive. These meaningful and significant incentives empower and incent primary care physicians to provide superior quality and efficient care to their patients. The QIPS program includes financial incentives for specific HEDIS and HEDIS-like quality measure performance, total cost efficiency, generic prescribing, utilization, and care efficiency. In addition, the program provides a direct financial incentive for practices that achieve recognition of a Patient-Centered Medical Home (PCMH) as defined by NCQA.
SpecialistsMost specialists are paid on a fee-for-service basis, meaning that payment is made according to our HMO fee schedule for the specific medical services that the specialist performs. Obstetricians are paid global fees that cover most of their professional services for prenatal care and for delivery.

Independence has specialty provider incentive programs designed to promote quality improvement and medical cost management. For example, we have a program that focuses on hip and knee replacement procedures. We have additional incentive programs in place with certain cardiology, oncology, urology, and womens health providers.
Designated Specialty SitesFor a few specialty services, PCPs are required to select a designated site to which they refer all of our HMO members for those services. The specialist services for which PCPs must select a designated site could include, but are not limited to, laboratory, radiology, occupational therapy, and physical therapy. Specialists in designated sites usually are paid a set dollar amount per member per month (capitation) for their services based on the PCPs who have selected them. Before selecting a PCP, HMO members may want to speak to the PCP regarding the designated specialty sites that PCP has chosen.
HospitalsFor most inpatient medical and surgical covered services, hospitals are paid case rates, which are set dollar amounts paid for a complete hospital inpatient stay related to a specific procedure or diagnosis (e.g., transplants). Some hospitals are also paid per diem rates, which are specific amounts paid for each day a member is in the hospital. These rates usually vary according to the intensity of the services provided. For most outpatient and emergency-covered services and procedures, most hospitals are paid specific rates based on the type of service performed. Hospitals may also be paid a global rate for certain outpatient services (e.g., lab and radiology) that includes both the facility and physician payment. For a few covered services, hospitals are paid based on a percentage of billed charges. Most hospitals are paid through a combination of these payment methods for various covered services.
Some hospitals participate in a quality incentive program. The program provides increased reimbursement to these hospitals when they meet specific quality criteria. Other criteria are directed at improved patient outcomes and electronic submissions. These incentive programs are expected to evolve over time.
The Integrated Provider Performance Incentive Plan (IPPIP) is an accountable care organization (ACO) payment model, which is designed to encourage provider collaboration and care coordination between primary care physicians, hospitals, and specialists. This ACO payment model offers an opportunity for primary care practices participating in the ACO to earn additional incentives based on ACO performance against nationally-recognized quality measures and a population-based, total cost gain-sharing program.
Skilled Nursing Homes, Rehabilitation Hospitals, and Other Care FacilitiesSkilled nursing and other special care facilities are paid per diem rates, which are specific amounts paid for each day a member is in the facility. These amounts may vary according to the intensity of services provided.
Hospital inpatient stayA hospital stay is when you have been formally admitted to the hospital for skilled medical services. Even if you stay in the hospital overnight, you might still be considered an outpa-tient.

Ambulatory Surgical Centers (ASCs)Most ASCs are paid specific rates based on the type of service performed. For a few services, some ASCs are paid based on a percentage of billed charges.
Physician Group Practices and Physician AssociationsCertain physician group practices and independent physician associations employ or contract with individual physicians to provide medical services. These groups are paid as outlined previously. These groups may pay their affiliated physicians a salary and/or provide incentives for primary care providers based on efficiency, quality, service, or other performance standards. We also may have arrangements with physician practice management (PPM) entities, IPAs (independent practice associations), or IDS (integrated delivery systems) in which the IPAs or IDS are paid a global fee to cover the cost of all services, including hospital, professional, and ancillary services provided to members who choose a PCP in such IPA or IDS. Such IPA or IDS would be at risk for the cost of these services. The PPM, IPA or IDS may also provide incentives to its affiliated physicians for meeting certain quality, service, and performance standards and may encourage members either directly or through providers who participate with them to use certain providers based on quality and cost-effectiveness.
Ancillary ProvidersSome ancillary providers, such as those providing durable medical equipment and home health care, are paid fee-for-service payments according to our HMO fee schedule for the specific medical services performed. Other ancillary providers, such as those providing laboratory services, are paid a set dollar amount per member per month (capitation). Capitated ancillary vendors are responsible for paying their contracted providers and do so on a fee-for-service basis. Independence has ancillary provider incentive programs designed to promote quality improvement and medical cost management. For example, we have programs in place with certain skilled nursing, and home health providers to encourage quality improvement and cost-efficiency.
Behavioral Health/Substance AbuseA behavioral health/substance abuse management company administers most of our behavioral health benefits, provides a network of participating behavioral health care providers, and processes some of the related claims. For some members, the behavioral health management company is paid an administrative fee for the behavioral health management services it provides, and we process and pay the related claims.

Skilled Nursing Facility (SNF) CareSkilled nursing care and rehabilita-tion services provided on a continu-ous, daily basis, in a skilled nursing facility. Examples of skilled nursing facility care include physical therapy or intravenous injections that can only be given by a registered nurse or doctor.
Rehabilitation servicesThese services include physical thera-py, speech and language therapy, and occupational therapy.
Ambulatory Surgical CenterAn ambulatory surgical center is an entity that operates exclusively for the purpose of furnishing outpatient surgical services to patients not requiring hospitalization and whose expected stay in the center does not exceed 24 hours.
Durable medical equipmentCertain medical equipment that is ordered by your doctor for medical reasons. Examples are walkers, wheelchairs, or hospital beds.

Primary Care PhysiciansThis section is arranged alphabetically by county, city, and then by practice or provider name. Each entry includes the following information: practice or provider name, provider identification number, address, phone number, type of practice, and the participating physicians within the practice (if applicable).
This listing also identifies which primary care physicians have been recognized as Patient-Centered Medical Homes (PCMH) by the National Committee for Quality Assurance (NCQA). PCMH is a new model of primary care delivery to improve access to and the coordination of care for members. A PCMH is intended to provide more personalized, coordinated, effective and efficient care. This directory identifies the primary care practices in the Keystone 65 Preferred network that are recognized by NCQA as a PCMH. You may visit the NCQA website at www.ncqa.org to learn more about these recognition programs. PCMH practices are identified by an asterisk next to their office address as either 2011 or 2014 Patient-Centered Medical Home accreditation.
Certified Nurse PractitionersThese providers are credentialed as primary care providers and may also be selected as a primary care physician. This listing also identifies facilities that are recognized for their expertise. This list is current as of June 2016. These facilities are called Blue Distinction Centers. Through Blue Distinction Centers, Keystone 65 Preferred members can identify participating facilities that have extensive experience, meet rigorous quality standards, and consistently demonstrate positive results. Currently, Blue Distinction Centers have been established for bariatric surgery, cardiac care, complex and rare cancers, knee and hip replacement, maternity care, spine surgery, and transplant services. All of the centers meet specific, evidence-based criteria on provider qualifications, best practices, and outcomes. Blue Distinction Centers+ not only meet the same quality criteria as Blue Distinction Centers, but they also go a step further. Hospitals receiving a Blue Distinction Center+ designation are also measured on how efficiently they deliver this high-quality patient care. These nationally established criteria were developed in collaboration with medical experts and organizations at the local and national level. The facilities within our provider network that have been recognized as Blue Distinction Centers are identified by the logo on the right.
