Who can be part of the study?
You can be part of the study if you
● live in Australia
● have a disability
● are 18 years or older.
You can also be part of the study if you are
● a family member of a person with a disability
● a support person of a person with a disability
● an advocate of a person with a disability.
Advocates help people with disability to understand
information and make choices.

If you want to be part of the study you must
● read all the information in this book
● ask for more information if you need it
● give your consent.
Consent means you understand and say yes.
Things to think about before you say yes to the study
When you do the survey, your ideas will help
make our services better.
Survey means a list of questions you can answer.

You might not want to do the survey if
● you do not have time
● you did not like seeing a dietitian in the past
and the survey makes you feel bad.
If you feel bad you can
● tell the researchers
● call your doctor
● call Lifeline on 13 11 14.

About the online survey
If you say yes to the study we will ask you to
do a survey.
The survey will be online so you will need to
have access to a computer or phone.
You can ask someone to help you do the survey.
The survey will take about 20 minutes to finish.
What questions are in the survey?
We will ask questions about
● your age
● where you live
● your gender
– for example, male or female or other.

We will also ask
● how you found a dietitian
● where you saw a dietitian
● what was good about the dietitian
● what could be done better.
At the end of the survey you can choose
to give more information at an online focus group
or interview.
A focus group is a meeting with a small group
of people and a researcher.
An interview is a meeting with you and someone
doing the study.
The online meeting will go for about 1 hour and
30 minutes and will be recorded.

If you say yes to the meeting you must give your
contact information so we can book the meeting.
You might win a $200 prize if you give us your
contact information at the end of the online survey.
The prize will be drawn on 1 June 2021.
You will be paid $77.30 per hour to come to
the meeting.
What will happen to your information?
We might use your information to
● teach dietitians how to work well with people
with disability and their carers
● write articles for experts to read
● write reports for the NDIA.

Your information is confidential.
Confidential means your name will not be on any
of your information.
Your information will be saved in a locked
computer file at Dietitians Australia for 5 years.
Only researchers from this study can look at
your information.
You can contact us to read any articles or reports
we write after the study.

What if you change your mind about the study?
You do not have to say yes to this study.
You can change your mind and stop the survey
at any time.
Nothing bad will happen if you stop the survey.
If you click submit we can not delete
your information.
