Valid Proof of Residence:

Option # 1 Current (July or August) utility bill (water, gas or electric, only) displaying parent/legal guardian name and current address.

Disconnect notices will not be accepted as a valid proof of residence.

Option #2 A copy of current rental agreement/lease page showing parent/legal guardian name, address and signatures of tenant/property manager.

New Students to HEB ISD

• Proof of residence (see above) • Original Birth Certificate* • Immunization record* • Social Security card (if available) • Parent/Guardian driver’s license or

current photo I.D. • Withdrawal form or report card from

previous school

New Students to Campus from another HEB ISD school

• Proof of residence (see above) • Withdrawal form or report card from

previous school

*Students transferring from another Texas public school have 30 days to provide original birth certificate and immunization records
