Nanolevel modification of biomaterials has significantlyimproved their properties. With regard to implant dentistry,the application of “nano” to implants, abutments, and bonesubstitutes drastically changed the biologic response to them.Numerous in vitro, in vivo, and clinical studies proved thatthe submicron modification, normally impossible to detectwith our eyes, has enhanced osteogenesis to the biomaterial.An interesting fact is that one of the authors in this specialissue L. M. Svanborg has suggested in her thesis work thatnanotopography in fact exists on most of the commerciallyavailable implant surfaces.However, these structures aremostof the time the result of native titanium oxides formed dueto the exposure to air; thus, these structures are regarded asnonintended nanomodification.

An important aspect for the application of “nano” tobiomaterial surfaces is that the modification is “intended” or“strategically” performed. Cell responses differ depending onthe topography and chemistry, and controlling these param-eters could target specific biologic phenomena. Thus, studiesshow that not only the successful enhancement of osseointe-gration but also the enhancement of soft tissue responses toabutment surfaces.

This special issue is a comprehensive compendium of thestate of the art research directed towards implants dentistry.A. Alenezi et al. compared two different commercially avail-able implant surfaces, one with and one without nanostruc-tures. It was suggested that the two different surfaces presentdistinct bone healing kinetics, and the surfaces possessingnanostructures seemed to be effective in osteoconduction at

early healing periods. J. Loberg et al. have suggested thatthe nanostructure acts as a nucleation center for the earlyformation of hydroxyapatites, which supports the in vivoreports. Contradictory to these studies, the in vivo animalstudy performed by L. M. Svanborg et al. has shown noeffects in their histological and biomechanical study. Theyhave speculated that “nano” may not exert significant effectsin siteswith excellent bone quality. Furthermore, the presenceof microtopography, different implant macrogeometry, andcoarse evaluation techniques could be one of the causes,which could be disguising the effect of “nano.” K. Bredinget al. have shown in their study that the effect of the nano-levelmodification could not be detectedwhen removal torque testswere performed; however, gene expression analysis of theremoved implants clearly pointed out that osteogenic geneexpression had been upregulated for some genes. Based onthese studies, the destructive removal torquemethodmay notbe suitable to evaluate the effect of nano-level modificationand advanced techniques may be necessary for evaluation.

Nano in implant dentistry restricts itself not only toimplant surface modifications but also to other biomaterialsused in this field. C. Marin et al. have shown that nano-structured synthetic alloplasts fused with collagen promotedbone ingrowth compared to other materials, which couldbe a promising material to regenerate bone in severe bonedefects due to atrophy after tooth/teeth loss. E. Bressan et al.have shown that the antibacterial properties of silver nano-particles are effective towards targeted pathogens, whereas nocytotoxic effects were observed on human cells. This noble

nanocoating could be an effective method to apply antibacte-rial properties to biomaterials, such as abutment surfaces tolower the risk of biofilm formation.
