Welcome to AG Mednet’s Judi II Imaging

Welcome to AG Mednet, the world’s largest diagnostic-imaging network. Our web-based network lets you transmit diagnostic-image exams electronically and securely to clinical trials.

The Desktop Agent (DA) ensures that all clinical trial image data is properly de-identified to satisfy regulatory compliance. It applies advanced, automated quality-assurance processes to ensure the data is protocol-compliant and readily usable. Finally, it delivers the data reliably and securely to one or more trial repositories.

About the DA

DA Overview

The AG Mednet Desktop Agent (DA) is a desktop application that handles searching for and importing imaging exams from local sources such as DICOM1 peer devices (modalities), file systems, or PACS.2 It helps you comply with the protocols of clinical trials by de-identifying records and images. Then it transmits the exams to the appropriate trial through the AG Mednet network.

The DA can be configured to help you locate images in a large repository. See Querying and Retrieving a DICOM Exam and How to Perform a DICOM Query in Appendix B.

Also, it is convenient to have imaging exams sent directly to the DA from the PACS or from a DICOM Modality (a DICOM image-acquisition device). See Adding the DA Information to a Sending PACS/DICOM Modality in Appendix B.

Getting Started

These are the steps that first-time users need to perform.

Software Requirements

Setting up the DA is a simple process that does not require administrative rights or local IT support. There are three steps to setting up the DA.

Launching the DA from the Portal

To run the DA for the first time, log in to the AG Mednet Portal.

If you have access to an active JUDI trial, the default login screen is the JUDI // Adjudication page. Click My Account at the top right to access the home page.

The home page shows recent notifications (if any) to your account.

To run the DA for the first time, click Submit Exams in the upper right of the Portal. This downloads and runs the DA. Depending on network bandwidth, the DA might need a minute or two to get started.

Depending on your desktop security settings, the DA might not start automatically. You can run the downloaded .jnlp file manually, or call Technical Support for assistance.

You may be asked to confirm that you want to run the DA. (Note the checkbox that lets you hide this message, so you won’t see it every time you log in.)

Click on Run.

You already logged in to the Portal, but you’ll have to log in again each time you open the DA. Use the same credentials as your Portal login.

The DA welcomes you by showing its Getting Started screen. You are ready to begin working with the DA.

Launching the DA from the Desktop Icon

After the DA has been downloaded from the Portal and installed, it can be launched from a convenient desktop icon.

There is no need to pass through the Portal every time you need to run the DA.

Using the DA

The AG Mednet Desktop Agent (DA) helps you locate and prepare an imaging exam for submission to a trial protocol.

The DA uses a simple workflow:

• Import an imaging exam from a local image repository. • Assign the exam to a trial. • Complete the prescribed trial workflow, which may include de-identification, transmittal forms,

and other trial-specific tasks. • Fill out any remaining fields in the transmittal form, which tells the trial administrator who

submitted this exam. • Transmit the exam. The exam is routed directly to the destination. You receive an email

confirmation and/or a notification on the Portal.

Depending on the trial configuration, tasks in a trial workflow may automatically start without the user needing to initiate them.

“Getting Started” Tab

The Getting Started tab, which is illustrated in the previous chapter, is the DA’s initial landing page. The checkbox at the bottom of the page lets you open the DA on other tabs if you wish.

There is also a list of international telephone numbers where help may be obtained at any time.

“Locate Exam” Tab

The DA can import exams from a CD/DVD, from your computer’s file system, or from a network file system. The DA can also import DICOM exams directly from a PACS or Modality. The exams may be either DICOM or non-DICOM.

Click on the Locate Exam tab. The Look in menu shows all available locations for you to search. When you have located a desired file or directory, use the Import Exam button to load that data into the DA. The exam appears on the Exam List.

During an import, the DA displays a pop-up box showing import progress.

Importing from a DICOMDIR

Look for a DICOMDIR directory. Select it and click the Import Exam button.

Note: It is best to use the DICOMDIR file for import (if available) because it contains all components of the exam.

Importing Individual DICOM Files

On the Locate Exam tab, use the Look in menu to navigate to the individual file you wish to import. When you select the desired file, you may see an image preview on the right. Click the Import Exam button

Importing from a CD/DVD/Folder

On the Locate Exam tab, navigate to the desired drive and file folder. Click Import Exam to import all images in that folder.

Warning: If you are importing DICOM files that do not have a DICOMDIR, take extra caution to ensure you import the entire exam. Select the top-level file directory associated with the exam before importing. Additionally, once the exam is in your worklist, confirm that the number of series and images matches the total you expected to see.

Non-DICOM Import Handling

The DA automatically determines whether you are importing DICOM exams or non-DICOM files based on your selections. You may select one or more files or folders.

The DA imports the following based on your selections:

You Selected The DA Imports As a DICOM/Non-DICOM exam

DICOMDIR file The DA imports all images in the DICOMDIR.

DICOM

Files The DA imports the selected DICOM files, if any, while ignoring non-DICOM files that were also selected.

DICOM

If there are no DICOM files in the selection, the DA imports the selected non-DICOM files except for executable (.exe) and library (.dll) files.

Non-DICOM

Entire CD, DVD, or Folder If the DA finds any DICOM files in the location, it imports them. It ignores non-DICOM files in this situation.

DICOM

If the DA does not find DICOM files in the location, then it imports all of the non-DICOM files except executable (.exe) and library (.dll) files.

“Exam List“ Tab

The Exam List is where a user can process an exam according to a trial’s specific protocols. The list shows all exams that you have imported into the DA. When you complete the workflow of an exam, it moves from the exam list to the audit log.

An exam can contain multiple images. Each exam initially appears as a single line. To “drill down” to the series level, click on the "+" symbol beside the exam.

Assigning an Exam to a Trial

Up to this point, we have been importing exams into the DA. The next step is to assign each exam to a specific trial.

Assign an exam to a trial by clicking the Assign Exam to Trial button in the lower left-hand corner of the screen. Then select the appropriate trial from the drop-down menu in the Assign Trial pop-up box:

You must assign the exam to a trial before you can proceed with any additional steps.

As soon as you assign the exam to a trial, a task list for the trial workflow appears in the lower-left corner of the screen. The DA leads you through these tasks in top-down order. (If there are optional tasks, you may select them manually and click on the Do Task button.)

Clinical Trials Module

The clinical trials module lets you extract trial-related fields from an integrated Electronic Data Capture system and add then to the exam. The trial’s core lab determines which fields are available to you. Enter values for the Site, Subject, and Timepoint. Some entries are dynamic. For example, you must select a Site to view its list of Subjects.

Complete the Clinical Trials module by clicking Save. The DA updates all the images in the test to contain this additional information.

De-Identification

AG Mednet provides an industry-leading de-identification tool, allowing you to anonymize an exam before sending it outside your institution. Most trials require de-identification.

When you reach the de-identification step, the DA presents a screen listing the DICOM header fields that require changes. Select a DICOM element by clicking the Deidentified Value cell.

Follow the directions in the information field on the lower portion of the screen. An icon in the right column shows whether your entry is valid or not:

Must be de-identified.

The item has been correctly de-identified.

Information should be changed, but is not required.

Read-only validation field: Cannot be changed but must conform to a protocol requirement.

When you correctly de-identify an item, the check mark icon appears. When you have de-identified all of the required fields, the Deidentify button becomes active.

Note: You must complete all required fields before exiting this screen. When you exit in mid-process, your changes are not saved.

Note: If no entry is required by the user, the DA will automatically complete and move on to the next module.

Pixel De-Identification

Pixel de-identification is an exam task that removes patient-identifying information from images. Use the mouse to erase a specific region of the image.

Mark the area to mask by clicking and dragging your cursor/mouse over the image. Click Deidentify Images. The DA prompts you about applying this mask to all images in the series, but this is not always appropriate. If the exam includes mixed image formats, you may need to use Pixel De-Identification in different locations on different images.

Completing a Transmittal Form

AG Mednet provides advanced forms capabilities that let you complete a transmittal form electronically. This form is similar to the paper forms used to send an exam to the core lab. There are several form characteristics you can expect to see:

• Required and optional fields. Required fields are bordered in red. If you attempt to save the form without completing one of these fields, the DA will point out the missing field to you.

• Pre-populated fields sometimes display data from the DICOM header of the exam. De-identification values might appear here.

• Data validation. Some fields may require specific formats, such as a value that has one letter followed by eight numbers. An alert may appear if the enter is not in the correct format.

• Dynamic fields. The form can change in response to your input. For instance, when a trial has two exam types, choosing one or the other can change the following fields on the form.

If your trial protocol requires a form, you can open it by clicking on the form task of the exam list. When you have completed all the required fields, click Save.

Note: When you attempt to complete a form without meeting the form specifications, the DA points out the incorrect fields. You must correct the values before proceeding.

Uploading the Exam to the AG Mednet Network

The final step is to upload the exam and associated data to the AG Mednet Network. The upload procedure depends on the settings of your specific trial. For some trials, you must select the Upload Exam task and click the Do Task button. Other trials will pop up a message saying “Are you ready to upload?” with a confirmation button. Some trials initiate the upload automatically when you complete the list of tasks.

Watch the upload progress bar. When the upload is complete, the exam disappears from your exam list. You may receive a delivery notification email when the exam reaches its destination, depending on the trial protocol.

Warning: Do not exit the DA while an upload is in process. An exam is uploading when the message “Exam Upload in Progress” displays in the lower-left screen, and the progress bar is still incrementing. If you exit while a transmission is in progress, transmission stalls until you log in again.

The DA can send multiple exams at one time while you proceed to other tasks. The DA records each upload event (see Advanced DA CommandsThe Portal).

Confirming Delivery

You can confirm delivery to the destination by visiting the AG Mednet Portal and viewing your submissions worklist. Additionally, you can set up email notifications confirming that AG Mednet has delivered an exam to its final destination. See Appendix C: The Portal, for more information.

Exiting the DA

Before exiting, verify that no exams are still uploading, as indicated by the exam’s Progress status bar. When an exam has completed uploading, the exam drops off your exam list.

Warning: Do not exit the DA while the Progress bar is still active. If you exit while a transmission is in progress, the transmission stalls until you log in again.

To exit the DA, use the File/Exit command or click on the X in the upper-right corner of the window.

Restarting the DA

To start the DA, click on the AG Mednet Icon on your desktop, or go to the AG Mednet Portal and click the Submit Exams button.

Appendix A: Virtual Histology Import Tool The Virtual Histology Import Tool is a DA module that attaches Virtual Histology (VH) raw image files to the associated intravascular ultrasound (IVUS) exam. If your trial includes the Virtual Histology Import Tool, you may select the IVUS-VH Import task to import non-DICOM Virtual Histology files for the Volcano IVUS modality. Imported VH files are added to the DICOM version of the exam as a new series.

The Virtual Histology Import Tool is specific for DICOM types:

The Virtual Histology module supports importing DICOM files located on CD, DVD, USB or the file system. After you import the corresponding IVUS exam and assign it to a trial, the IVUS-VH workflow task appears in the "Available Tasks" list:

The IVUS-VH Import task opens another file browser window. Navigate this browser back to the DICOMDIR file associated with this exam.

For example, if the DICOM IVUS exam was located at D:\, then navigate back to the same location, select the file D:\DICOMDIR, and choose Open.

The Virtual Histology tool imports all associated non-DICOM VH files for this exam. A status window displays the progress of the import.

When the import is complete, the IVUS-VH Import task displays a check mark. Also, the VH files imported appear as an additional series added to the exam:

Appendix B: Advanced DA Commands The “Advanced DA Commands” are actions outside of the usual task flow. They are available from the File menu.

Manual Delete

The DA lets you manually delete entire exams, selected series, and selected instances. This is not normally required, but if you have imported an exam in error you may wish to delete it manually.

Note: The DA never deletes the original exam files (on the source media). These commands delete the copies of the exams, series, and instances that you imported into the DA.

To manually delete an exam from the worklist, highlight the exam and select File/Delete Study. You can also manually delete a series or instance by first selecting it on the Exam List.

The DA asks for confirmation before performing a manual deletion.

Warning: You cannot Undo a manual delete. If you delete an exam, series or instance accidentally, you must re-import the entire exam.

Querying and Retrieving a DICOM Exam

You can query and retrieve an exam from any DICOM peer device on your network. Before using this feature, you must first add a destination to query.

Adding a PACS or DICOM Modality Query Destination

To use the DA’s query/retrieve capability, you must add a DICOM destination to the DA’s list of DICOM devices. To set up this capability, select File/Preferences.

On the DICOM Devices tab, you can add destinations to query. Click the New button and type in the AE Title, description, host name, and port, and then click Save. To make this your default query destination, select the Default Device box. Typically your radiology department can provide this information to you. You can edit this information at any time by highlighting the destination then editing the information fields and re-saving.

Note: You must restart the DA for this setting to take effect; you will not be able to Query/ Retrieve until this step is done. Additionally, your PACS administrator may also have to enter in your AE Modality information into the PACS, depending on the local PACS settings. If the query feature does not yield results after set up, you institution’s PACS may have settings that prevent queries. Contact your PACS administrator.

How to Perform a DICOM Query

Begin by selecting Tools/DICOM Query. Next, input all available search criteria using the following fields:

• Patient Name • Patient ID • Study Description • Accession Number • Date of Birth • Study Date • Modality

Finally, select the device you would like to search in the upper-right corner of the screen. (You may search one device at a time). The device is likely to the Modality on which the exam was created or a PACS where exams at your institution reside.

The search results appear in the bottom half of the screen. You can resize columns as necessary, as well as sort the rows by clicking on the column title. If you do not see the exam you are looking for, check that your query criteria are correct and that you have selected the appropriate source. If the exam still doesn’t appear, try removing some of the query criteria. If the problem persists, contact your imaging department.

You can retrieve multiple exams from the query results by clicking the checkboxes in the leftmost column of the results table, and then click on the Retrieve Selected Exams button. Progress bars appear in the rightmost column. Once the import is complete, a pop-up box confirms completion.

Note: There is sometimes a small delay before the exam appears on your exam list.

Direct Send from DICOM Modality or PACS

The AG Mednet DA can receive a DICOM study directly from any DICOM Modality or PACS on your network. To do so, add the DA to the sending device or PACS as a destination. Your imaging department will need the AE Title, Port, and IP Address of the DA, which appear on the DICOM Listener tab of the File/Preferences dialog box.

Warning: The DA must be running, and you must log in before remote devices can send exams to the DA.

You do not need to take any action in the DA to receive exams-- just send the exam from the device/PACS to the DA. Received exams appear in the DA's exam list.

Adding the DA Information to a Sending PACS/DICOM Modality

To receive exams directly from a DICOM Modality, first configure an AE Title for the DA by clicking on the DICOM Listener tab. Enter an AE Title and Port, and then click Save.

To use AG Mednet’s recommended default, type in an AE title consisting of your initials (such as “ABC”), an underscore, and “Agent”. For example: ABC_Agent. Leave the host blank. Set the port to 104 and click Save.

Next, set the DA's AE Title, Port, and IP Address in the sending device or PACS as a destination. Use one of the IP addresses that is reachable from your device/PACS.

What is the difference between the DA and the Portal?

The AG Mednet Portal is a website used for reviewing exam transaction information. The portal is also where you create your AG Mednet account, and where you download and launch the DA for the first time. Portal: https://judi.agmednet.net

The DA is an application that lets you transmit diagnostic-image exams to clinical trials. Additionally, the Agent provides tools to de-identify exams, complete transmittal forms electronically, and perform other trial-specific tasks. You must be invited to join a trial before the Agent can transmit exams to it.

Where can I obtain an invitation or join code for a specific trial?

If you have not yet received an email inviting you to the trial, please contact the core lab project manager coordinating this study. If you are unable to reach the core lab project manager, please send an email to [email protected] to obtain a join code that can be entered into the portal.

By entering the join code on either the registration page (for new users) or under Home > My Account > Memberships > ”Ask to Join A Trial” on the portal (for registered users), an email will be sent to the trial administrators for the trial. A trial administrator will either invite you to the trial or will respond with any questions.

When I try to register, the Portal tells me “This email address is already used by another user. Please choose another.” What should I do?

Your email address is already active with another AG Mednet account. You can have many trials per account, so there is no need for multiple accounts.

If you do not remember the username or password for your account, you can attempt to reset your password by clicking the “Forgotten your password?” link on the portal. If that does not work, or if you believe that you have never registered with AG Mednet before, please contact us at [email protected] for assistance.

What should I do if I forget my login credentials?

To obtain your username, please contact AG Mednet Support.

To reset your password, click on “Have You Forgotten Your Password?” on the Portal or Desktop Agent Login Screen. Enter your registered email address and click on “Request Password Reset”. You will receive a password reset email. Follow the instructions in the email to reset your password.

What should I do if my trial is not in the “Assign Trial” drop-down menu?

If you have not yet received an email inviting you to the trial, please contact the core lab project manager coordinating this study. If you are unable to reach the core lab project manager, please contact [email protected]. We will help you locate the project manager.

I received an invitation, but still do not see my trial.

Your AG Mednet account and your clinical trial invitations must all use the same email address. If the project manager sent the invitation to a different address, you must arrange for a new invitation using the address of your AG Mednet account.

The option to “Submit Exams” is not available on the portal. What should I do?

If you have registered on the Portal but have not yet received an email inviting you to a trial, or you cannot successfully log into the DA, please contact the core lab project manager coordinating your study. What should I do if I receive an error when running “launch.jnlp”?

This occurs when launch.jnlp file is not correctly associated with Java Web Start.

To associate launch.jnlp with the correct file, look in the downloads directory used by your web browser. Right click on launch.jnlp, select Properties > Opens with > Change > Browse > “c:\Program Files\Java\jre1.x.x_xxx\bin\javaws.exe”. If you continue to receive errors, please contact the AG Mednet Technical Support Team.

What should I do if my IT Department doesn’t allow me to download Java or the DA?

Please have your IT Representative contact the AG Mednet Technical Support Team. Our support engineers are available to address any concerns about downloading Java or the DA.

What should I do if my IT Department claims that firewall restrictions prohibit me from using the DA?

The Desktop Agent will need to send and receive HTTPS (port 443) traffic to and from www.agmednet.net, judi.agmednet.net, judi-auth.agmednet.net, portal.agmednet.net and blue.agmednet.net. We recommend allowing access for the entire domain, *.agmednet.net to allow for infrastructural changes. Also, AG Mednet uses a third-party certificate authority, Digicert. Please whitelist Digicert as a supported certificate authority: AGMednet, Inc. (DigiCert SHA2 Assured ID Code Signing CA)

What happens if I close the DA before the exam upload is complete?

The DA is very forgiving of interrupted uploads. The upload picks up where it left off the next time you log in. There is no need to redo anything.

How long does it take to upload an exam?

The size of the exam and the network bandwidth determine the time required to complete an upload. Medical imaging exams can be quite large, sometimes requiring minutes to complete.

You can continue to work on other exams in the DA while an upload is in progress. The DA queues upload requests, completing them sequentially as rapidly as possible.

Does the 15-minute DA inactivity lockout interrupt my exam upload?

DA uploads continue uninterrupted during an inactivity lockout.

Is a temporary folder saved to my computer when I use the DA? Does this mean that I could potentially send PII (patient identifying information) to the core lab?

The DA cannot transmit an exam to a core lab until you complete all of the tasks required by that project. One of these tasks is de-identification. There is no risk of sending PII by accident.

When you import an exam, the DA creates a temporary copy of the exam on your local computer. This copy contains patient information until you complete the de-identification step. The Agent deletes this copy automatically after uploading it.

If my CD contains more than one exam, how do I import them into the DA?

If there is one DICOMDIR, simply highlight this and click on “Import Exam.” The DA locates all of the exams in that directory and loads them into your Exam List.

How do I import my exam if it is too large to fit on one CD and requires 2 CDs?

Import the exam from the first CD. Then insert the second CD and import the exam as if it is a new exam. The DA recognizes the exam ID and merges the data into one entry in your Exam List.

Is the data that I send across the AG Mednet network secure and encrypted?

The DA encrypts your exams for uploading. They remain encrypted until they arrive at their destination. AG Mednet is also HIPAA and 21 CFR Part 11 compliant.
