THE DEPARTMENT OF HEALTH’ SERVICES’ REQUEST TO APPOINT APHYSICIAN MANAGEMENT POSITION IN THE EMERGENCY MEDICAL SERVICES(EMS) AGENCY
In accordance with the Board’s policy on management appointments, we received arequest from the Department of Health Services (DHS) to appoint William J. Koenig,M.D. as a Chief Physician II, M.D., Item Number 5480A, in the DHS Emergency MedicalServices (EMS) Agency. Dr. Koenig would be placed on Schedule Number M8, Step 13of the Management Physician Pay Plan, at an annual salary of $163,455.18($13,621 .265 monthly). This salary represents a 3 percent annual Physician AdditionalCompensation bonus of $4,381.20 ($365.10 monthly), a 5.5 percent annual BoardCertification bonus of $8,273.16 ($689.43 monthly), and a 3.25 percent annualPhysician Specialist bonus of $4,760.83 ($396135 monthly). This position is fullyfunded in the FY 2003-2004 budget and the Department would like to make thisappointment as soon as possible.
Dr. Koenig will serve as the Medical Director of the EMS Agency and will be responsiblefor providing medical direction and managing the EMS system. The Los AngelesCounty EMS Agency is the largest EMS system in the country. This multi-jurisdictionagency consists of four (4) paramedic provider agencies and eighty-two (82) 9-1-1receiving hospitals. Various responsibilities of the EMS Medical Director includecertifying and accrediting EMS personnel, maintaining a system wide qualityimprovement program, providing medical direction in disaster situations, and ensuringappropriate physician oversight and coverage at the Medical Alert Center, Additionalduties consist of planning, directing, and supervising EMS medical research andconsulting with public and private agencies regarding emergency medical services.

The Department of Health Services states that Dr. Koenig is highly qualified for thisposition based on his substantial experience in the field of emergency medicine.Dr. Koenig is Board certified in emergency medicine and has over 27 years ofexperience in this field and with EMS system leadership. This includes previousemployment with DHS as the EMS Director from 1987 to 1993 and experience as theMedical Director at various hospitals. Additionally, Dr. Koenig is on the Board ofDirectors for the EMS Best Practices Newsletter and the Prehospital Care ResearchForum. Respectively, these agencies are devoted to the promotion of best practices inemergency medical services through publication and recognition of model systems andof research in prehospital care. Furthermore, Dr. Koenig has been a clinical instructorat LAC+USC Medical Center’s Department of Emergency Medicine since 1984 and anexaminer for the American Board of Emergency Medicine since 1981.
Based on his experience in the field of emergency medicine and unique knowledge,Dr. Koenig appears to be an excellent candidate for this position. Therefore, we concurwith DHS’ request to appoint Dr. Koenig to the position of Chief Physician II, M.D. at thespecial step 13 placement with an annual salary of $163,455.18.

The EMS Medical Director receives administrative direction from the EMS Director and medicaldirection from DHS’ Director and Chief Medical Officer with advisory input from the Medical Council,which is comprised of medical representatives of all system constituents. Designated to provide foremergency prehospital medical care and disaster services, the Countywide EMS system is acoordinated composite with sub-elements of public education and information, communication,EMS reimbursement programs, emergency and non-emergency medical transportation, medicalcare, the trauma hospital system, coordination of the patient transfer system,certification/accreditation of prehospital personnel, paramedic services and training, emergencymedical training and approval of EMS training programs.
Efforts to select a qualified candidate have included placing position vacancy notices in applicablephysician journals and also mailing them to members of the California Chapter of the AmericanCollege of Emergency Physicians. Approximately one year ago a qualified candidate declined theposition because he stated that the salary was inadequate. Dr. Koenig is one of the two mostqualified candidates on the current certification list because he possesses “substantial experience inthe practice of emergency medicine” as required by Division 2.5 of the Health and Safety Code.Although the requested salary will be approximately 44% less than his current salary, Dr. Koenigwelcomes the opportunity to have a greater countywide impact on medical care provided in theprehospital care setting while benefiting from the County’s retirement plan.
Dr. Koenig is Board certified in Emergency Medicine and has over 27 years of experience inemergency medicine and EMS system leadership which includes both private practice and previousemployment with the Department of Health Services as the EMS Director from 1987 to 1993. Dr.Koenig is currently the Medical Director of the Emergency Department at Long Beach MemorialMedical Hospital, a Level 2 Trauma Center and the Medical Director for the Sierra SacramentoValley EMS Agency, which encompasses five counties. He is also the Medical Director for MercyAir Ambulance which provides emergency air ambulance coverage to Los Angeles, Riverside,Ventura, San Bernardino, Orange and San Diego Counties. Dr. Koenig is on the Board of Directorsfor the EMS Best Practices Newsletter, which is devoted to the promotion of best practices inemergency medical services through recognition and publication of model systems and on theBoard of Directors for the Prehospital Care Research Forum, which is devoted to the promotion ofresearch in prehospital care. Additionally, he has been a clinical instructor at LAC+USC MedicalCenter’s Department of Emergency Medicine since 1984 and an examiner for the American Boardof Emergency Medicine since 1981.
The salary requested for Dr. Koenig is commensurate with the scope and responsibilities of theChief Physician II, M.D. position. He is highly qualified and possesses the special knowledge, skills,and experience to successfully perform in this critical and highly visible position.
