Accessing the Final Evaluation
• When you open a teacher’s plan, select the “Final Evaluation” tab on the right

General Overview/Demographics
• Employee information• Initial Planning Sheet submission and
conference• Observation dates and durations• Conference dates

The Final Evaluation Form will pre-populate with information and data that you entered during the year.

Observation dates will pre-populate if you entered them in the scheduling activities
IPS dates will pre-populate if you entered them in the IPS sign-off activity

• Planning and Preparation, Classroom Environment, and Delivery of Instruction
–Formal Observations–Informal Growth Plan Visits–Supplemental Observations

populate on the evaluation screen, based on the administrator’s ratings entered for formal or supplemental observations

What happens if the Final Overall Rating is Below Standard Performance?
• Notify Staff Relations Field Directors ASAP
• Follow detailed steps in Final Evaluation User’s Guide
• Teacher must be scheduled for evaluation next year
