Household monthly income per person must be $1,800 and below OR Annual Value (AV)* of residence must be $21,000 and below for households with no income.

CHAS Criteria
* AV is the estimated annual rent of your residence if it is rented out. An AV of up to $21,000 will cover all HDB flats and some lower-value private residences.
Pick up a CHAS application form at any Community Centre or Club (CC), Community Development Council (CDC), Public Hospital or Polyclinic or visit www.chas.sg to download the form.
Only one application form per household is needed. All eligible family members in your household will receive a Health Assist card if your application is successful.

CHAS Subsidies
There are two types of Health Assist cards:

Integrated Screening Programme (ISP) under Health Promotion Board (HPB)
Cardholders who receive HPBs invitation** will receive subsidies for the recommended screening tests under ISP. They will also enjoy subsidies for their doctors consultation charges (up to $18.50 per visit)
for screening and related follow-ups, up to two times per calendar year.
About CHASThe Community Health Assist Scheme (CHAS) enables Singapore Citizens from lower- and middle-income households to receive subsidies for medical and dental care at participating General Practitioners (GPs) and dental clinics near their homes. To enjoy subsidies at CHAS clinics, simply produce your Health Assist card with your NRIC during registration. For cardholders below 15 years old, please present your Health Assist card and birth certificate. CHAS clinics can be easily identified by the CHAS sticker displayed.
