I. Call to Order
The meeting was called to order at 8:01 a.m., Thursday, May 16, 2019.
The following Board members were present: Quinn, Berrigan, Gillette, Gutierrez, Machesky, Preston and Smith
Quinn presided. Quinn welcomed members of the audience, explained the procedure for addressing the Board, and
stated the mission of the Board.

II.H. APRN/RN/LPN/CNA Investigative Report/Potential Summary Suspension Pursuant to
A.R.S. § 41-1092.11(B) and/or Acceptance of a Consent Agreement and/or Adoption of

II.H.1. Gregg Steven Faulkner RN063632 (Chambers)

Faulkner was not present.
Based upon the information presented at this meeting, Board members discussed whether a showing of extraordinary
circumstances or the possibility of irreparable harm has been made supporting the need for a Summary Suspension.
Quinn moved, Gutierrez seconded, and it was carried, based upon the information presented at this meeting and the
information contained in the investigative report, to accept and adopt the proposed preliminary Findings of Fact and
Conclusions of Law of the Findings set forth in the draft Order attached to the investigative report.

It was therefore ordered that the license of Gregg Steven Faulkner, RN063632 be suspended effective with the service
of the written Order, and that Gregg Steven Faulkner, RN063632 will immediately cease practice as a nurse. This
suspension shall remain in effect pending a public hearing and final decision by the Arizona State Board of Nursing
pursuant to A.R.S. § 32-1664.

Updates have been implemented but there have been errors and Board staff have developed work-arounds.
Board members discussed licensing process and stated they would like to continue to receive updates regarding

Ellis provided trending of licensing data from 2000-2019.
VII.A.10. Update on Personnel Changes: Dahn/Ellis/Ridenour
Ellis provided an update on AZBN personnel changes that included: Chris Arismendez to IT. Dahn advised that Irene
Barajas from Licensing is joining Investigations. John Capone joined Investigations and Diane Williams and Liz
Kleiser joined Customer Service. Ridenour announced that Cindy Mand has been promoted to the new Associate
Director for Complaints and Investigations.

abuse, and to include any additional testing deemed necessary by the evaluator, to be completed with 45 days and
then return to the Board. If the Interim Order is not completed, based upon the information in the investigative report,
issue Notice of Charges and include the factual allegations and alleged violations for failure to comply with the
Interim Order.
Quinn and Gillette amended the motion to allow 90 days to complete evaluation, and with this amendment, the vote

Program Investigative Report
Jan Kramer and Glen Tharp were present from Brookline College, along with counsel Bryan Gottfredson, and Jay
Heiler. Counsel addressed the Board and requested the Board continue to next meeting.
Malloch updated the Board about an offer of probation for 18 months with early termination for 12 months and also
advised the Board of the interpretation of Brookline’s NCLEX scores.
Board members discussed length of probation, and possibility of continuing to next meeting to allow time to negotiate.
Quinn moved, Hill-Mekoba seconded, and it was unanimously carried, to continue this case. Quinn provided direction
to Board staff to provide an update to Board members within 14 days.

resources and revised preceptor policy. Steinbinder advised that Board staff is still waiting on evidence of Arizona
licensing approvals for 3 faculty.
