YOUR PATIENT-CENTERED MEDICAL HOME The National Committee for Quality Assurance, or NCQA, describes the Patient-Centered Medical Home as a model for care provided by physician practices that seek to strengthen the physician-patient relationship. UF Health primary care practices in Northeast Florida have been designated Patient-Centered Medical Homes by the NCQA. UF Health primary care practices are the only Patient-Centered Medical Homes in the region. As your designated medical home, our call center operators and referral coordinators provide support services to help coordinate your care across our health system. YOUR MEDICAL HOME CARE TEAM Your medical home care team includes call center associates, care associates and referral coordinators who offer extended support services across multiple practices. PRACTICE LOCATIONSUF Health physicians and providers are committed to you and your family’s health.

Most of our practices are open Monday through Friday from8 a.m. to 5 p.m., with some locations offering extended hours.* Visit UFJaxPrimaryCare.org for the practice hours at the location nearest you.

Patients may call to request an appointment or to speak with the office manager to answer any questions. Appointment requests may also be made online through your MyUFHealth patient portal account on your computer or mobile device. MyUFHealth allows you to message your provider, request medication refills, view test results and more.

YOUR FIRST APPOINTMENTOur practices are part of the region’s leading academic health center. All services are offered by a team of physicians, residents and medical students.u Please arrive 15 minutes before your scheduled appointment time to fill out any paperwork or update your medical records. u Bring your current health insurance card or insurance information and a photo ID. u Bring a list of all current medications.u Bring copies of any medical records you may have.u Your first visit may last longer than usual so we can obtain a complete medical history and perform a comprehensive examination.

OUR EXPECTATIONTo provide you with the best care possible, we need your medical history, including current medications, recent test results, information about visits to other doctors and health care providers, hospitalizations and emergency room visits.

FEES AND INSURANCEOur fees are based on the services we provide. We accept most types of insurance, as well as Medicaid and Medicare. If your insurance covers all or a portion of your claim, we will file it for you and bill your insurance company. If you are responsible for all or a portion of your health care costs, payment is expected at the time of your visit.
