Mission Statement

Battenkill Valley Health Center, a community resource, provides compassionate, responsive, comprehensive health care through a committed team of professionals ensuring affordable access to all.

BVHC honored our local first responders during National Health Center Week for all

they do to keep our communities safe.

BVHC partnered with Fisher Elementary students to send their uneaten candy to

troops over seas - sweetening up their day!

BVHC staff donated over 50 Christmas gifts to local children in need through the all-volunteer Red Stocking Program.

BVHC assessed the oral health of 30 children at Happy Days Playschool and provided

parents with seasonal health information.

Our Board of Directors

BVHC?s Board of Directors is comprised of devoted individuals who volunteer their t ime and energy to provide the Center with ongoing guidance and vision. The majority of our Board members have been BVHC pat ients, so their leadership reflects the interests, issues, and concerns of the people we serve. The Board of Directors are vested with ult imate responsibility for the fiduciary management of BVHC and to ensure that the Battenkill Valley Health Center fulfills its mission.

Peter King, MD (2015) was the director of the Bennington VA Clinic from 1997-2015. Pract icing primary care, Dr. King has a special interest in long-term care of older adults.

Carol Kirkland (2017) is a ret ired instructor from Penn State University where she taught courses in stat ist ics and quantitat ive business methods.

Russ Leslie (2018) is the co-founder and ret ired associate director of Rensselaer Polytechnic Inst itute?s Light ing Research Center and Professor Emeritus of architecture at Rensselaer.

Jan Nolan (2016) has been a Procurement Specialist at Mack Molding since 2004. She brings her progressive and comprehensive experience in purchasing, negot iat ing, and cost management to the board.

Lee Spivey (2010) is a Principal CPA at Spivey, Lemonik, Swenor PC. He joined the firm in 1990 and later became a lead partner.

Lynn Williams (2010-2013, 2015) spent 29 years as a school nurse serving hundreds of children in Arlington and Sunderland.

MaryAnn Carlson (2010) is current ly a self-employed psychotherapist and Reiki Master Teacher and pract it ioner. Previously, she worked as a Clinical Psychologist for children and their families at United Counseling Services for 14 years.

Denise Colety (2010) has worked per diem for Dartmouth-Hitchcock Medical Center since 2003 as the 3D-Dartmouth Device Development Office Coordinator.

Charlie Cummings (2018) has been a member of the Fisher Elementary School faculty since 2008, serving the children and their families of the Arlington community.

Brian Cunningham, MD (2015) is a ret ired internist who spent 36 years at Southwestern Vermont Medical Center. He is a faculty member at the Department of Medicine Residence Program at Berkshire Medical Center in Pit tsfield, MA.

Bill Jerome (2017) is president of Off Road Solar, Ltd., a principal in Vermont Solar Farmers, a representat ive of Recovered Energy Technologies and Powerstar/Virtue.

Equal AccessSliding Fee Program

The Sliding Fee Program is a federally funded program that provides a discount to pat ients who are uninsured or under-insured. This program allows qualifying pat ients to receive primary medical care and/or qualifying dental services at BVHC at a discounted rate after any insurance, if applicable, has been processed. There is a minimum co-payment due at the t ime of service for all discounted services received.

Federal guidelines require BVHC to take household size and income into considerat ion when determining an applicant?s eligibility. Pat ients can apply anyt ime to understand if they qualify for the Sliding Fee Program. If approved for the program, pat ients are required to renew your applicat ion every six months. If a pat ient is not approved for the program, they are encouraged to contact BVHC if they have a significant change in income or family size to allow the Health Center to re-evaluate the information.

Services at BVHC

Primary & Prevent ive Care

Primary medical care is at the heart of what we do. If you are healthy, we can help you stay that way. If you are sick, we can treat you. It is our priority to provide a safe and comfortable environment for pat ients to talk about confident ial concerns.

General Dent ist ry

The dental team is thrilled to provide oral care in four state-of-the-art dental operatories. From cleanings to restorat ive care, BVHC provides general dent istry to pat ients start ing at age one. Zoom teeth whitening and Invisalign are also available.

Nut rit ional Counseling

Providing pat ients with valuable dietary skills to lead a healthy lifestyle, our nutrit ionist offers ideas to help pat ients make healthy food choices. Pat ients can learn hands-on how to cook delicious, healthy meals for themselves and their families in the on-site teaching kitchen.

Nurse Case Management

Nurse case managers oversee transit ional care for BVHC pat ients. Working as a team to ensure quality care post hospitalizat ion, nurse case managers ensure pat ients are following their care plan to help them meet their healthcare goals.

Counseling Services

We offer mental health, nutrit ional health, and substance use disorder counseling to children, adolescents, adults, and families in both individual and group sett ings.

National Health Center Week (NHCW), celebrated every August, is a fun t ime for BVHC to honor the pat ients we serve. This past NHCW, BVHC was recognized by the U.S. Department of Health and Human Services as a National Quality Leader and was awarded $76,110 to

cont inue its successful quality improvement efforts.

Medical Providers at BVHC

Battenkill Valley Health Center medical providers know and care about the patients they serve. Whether a patient is seen once a year or once a week, they are valued and involved in their health care plan. Providing care on a daily basis, our medical team is committed to our patients - helping to create a healthier community for us all. We are excited for the road ahead, and we look forward to delivering care to patients of all ages, throughout all stages of life.

Dr. Anje Van Berckelaer (Dr. Van) provides primary health care services to pat ients of all ages and has served as BVHC's Clinical Director since March, 2017. Dr. Van received her early educat ion at Université Paul Valéry in Montpellier, France and then received a B.A. in Anthropology from Yale University. After earning her Doctorate of Medicine from Harvard Medical School in 2004, Dr. Van spent t ime as a Physician Supervisor with Doctors Without Borders in Maradi, Niger and the Central African Republic providing direct pediatric and adult inpat ient care. In 2010, Dr. Van went on to complete her Masters in Health Policy Research from the University of Pennsylvania. Prior to moving to the area with her husband to be closer to her in-laws, she worked in Philadelphia at several health centers. Dr. Van speaks English, French, Spanish, and Dutch.

Anje Van Berckelaer, MD

Dr. Michael Welther, who pract iced medicine at 9 Church Street for over 28 years, has spent the past three years at the Pownal Campus of Southwestern Vermont Medical Center. On February 19th, 2019, Dr. Welther will return to his Arlington roots and BVHC and the community is thrilled! Dr. Welther received both his undergraduate educat ion and his medical degree from the University of Wisconsin, Madison, and is board cert ified in family medicine. "As a family medicine physician, I look forward to ut ilizing the support of my future colleagues at BVHC who deliver mental health and substance abuse counseling, general dental care, nurse case management, and nutrit ion counseling all under one roof."

Michael Welther, MD

Following her graduation from Southern Vermont College (SVC) with an Associates degree in Science of Nursing, Jamie Rogers, Family Nurse Pract it ioner, began her nursing career in 2010 at the Eddy Heritage House in Troy, NY. While working there, she went on to pursue her Bachelors in Science of Nursing in 2011, again from SVC. Continuing her educat ion while working for VNA & Hospice and Southwestern Vermont Medical Center, Ms. Rogers went on to receive her Master's in Science of Nursing in Leadership/Management, and in 2016 she completed her post graduate cert ificat ion as a Family Nurse Pract it ioner from Maryville University. Ms. Rogers joined BVHC in January, 2018. Jamie Rogers, FNP

Expanded Services in 2018

Battenkill Valley Health Center is committed to promoting mental health as a crit ical part of overall wellness. To meet the growing needs of our community, we increased mental health services with the addit ion of Amy Breese, LICSW.

Amy Breese, Licensed Independent Clinical Social Worker, graduated with a Bachelor's degree in Childcare Management from Southern Vermont College. After pursuing a long career as a postpartum doula, Ms. Breese realized that her favorite part of the job was listening to new mothers and assist ing in their emotional growth. She then went on to earn a Master's degree from the State University of New York - Albany in Clinical Social Work. Ms. Breese has held posit ions at Big Brothers, Big Sisters of the Capital Region in Albany, NY as a Match Support Specialist and most recent ly as an Outpat ient Clinician at United Counseling Service in Bennington, VT. Ms. Breese joined the team at Battenkill Valley Health Center in July,

2018 and assisted in developing the behavioral health program. She enjoys working with clients aged five years and older, and specializes in perinatal mood disorders, anxiety, depression, and trauma.

Where compassion and healing come together.

Mental Health

Specializing in perinatal mood disorders, anxiety, t rauma, and depression, our clinician works closely with pat ients to gain control of their emotional and behavioral needs. Short-term and long-term counseling provides pat ients with a plan that works for them and their family.

Nut rit ional Health

Providing pat ients with valuable dietary skills to lead a healthy lifestyle, our diet it ian offers ideas to help pat ients make healthy food choices. Pat ients can learn hands-on how to cook delicious, healthy meals for themselves and their families in the on-site teaching kitchen.

Substance Use Disorder

We provide Medicat ion Assisted Treatment (Suboxone) for individuals dependent on opioids. Pat ients are provided support, educat ion, and consultat ion. Individual and/or group therapy assists pat ients in rebuilding lives that have been affected by an opioid disorder.

Arlington Food Shelf - Resource for those in need; staff members donate t ime and food to this important organizat ion.

Arlington Community Public Health Nursing Services - Provided BVHC with grant funding to use as needed; cont inues to be a resource for pat ients in need of financial assistance in regards to their health and well-being.

Battenkill Valley Supervisory Union - BVHC providers have presented at faculty meetings and visited classrooms to offer assistance on health related topics.

Happy Days Playschool - BVHC dental team has provided our youngest cit izens with oral assessments and act ivit ies to encourage oral health and wellness.

Mack Molding - BVHC dental team has provided Mack Molding employees with oral assessments and educat ion about their dental benefits; expanded hours to meet the needs of working pat ients.

Pickering's Greenhouse - Created a Good Food Bucks program which supplies pat ients with organic, nutrient dense food as part of their health improvement plan.

Southwestern Vermont Medical Center - We have a referral agreement in place for medical services, and SVMC provides dental peer review and coverage.

Addit ional Community Collaborat ions ~ St rengthening our communit ies by improving the health of all of our neighbors.

During Mayfest, we have the chance to meet with families from all over Bennington County, providing parents with health educat ion and creat ing teaching opportunit ies through act ivit ies with their children.

Eagle Pride: 10 of our 24 employees are graduates of AMHS or current ly have children in

the Arlington School System.

In 2016 BVHC had 119 pediatric pat ients. Today, 520 pediatric pat ients are seen by our team of

providers - a 337% increase!

Staff members have met with parents at local daycares, answering quest ions about their

child's health and wellness.

Oral health is key to your overall health and wellness. The dental team at BVHC is thrilled to provide oral care in four state-of-the-art dental operatories. From cleanings to restorat ive care, BVHC provides general dent istry to pat ients start ing at age one.

Dr. Stephen Phillips provides general dent istry to pat ients of all ages and has served as BVHC's Dental Director since April, 2017. Having received his Doctorate in Dental Surgery from Georgetown University School of Dentistry, Dr. Phillips has provided oral care for over 35 years.

With the understanding that oral health is key to overall health and wellness, Dr. Phillips and his team provide general dent istry to pat ients start ing at age one, from cleanings to restorat ive care.

As part of their t raining, fourth year student dent ists from the University of New England College of Dental Medicine spend t ime working and learning from pract icing dent ists. These students have been chosen as they have met all school guidelines

proving they are clinically ready to be providing care to pat ients in outpat ient sett ings. Dr. Phillips has been chosen to teach dental students because of his excellent dental and teaching skills and his commitment to training students.

The opportunity to experience a dental pract ice in local communit ies is an important part of a dental student?s training. As a direct result of this experience, some of these students may eventually locate their pract ices in Vermont/Maine/New York/New Hampshire/Connect icut. When this happens, the dental needs of all residents are better met.

Programs Available to Patients

The Vermont Blueprint for Health is a state-led, nat ionally recognized init iat ive that helps health care providers meet the medical and social needs of people in their communit ies.

Medical Homes are the foundation of the Blueprint which helps to cont inuously improve pat ient care and outcomes. Community Health Teams expand the services available to include free care coordinat ion, counseling, substance abuse support, health coaching and more. The Blueprint also helps design innovat ive intervent ions, like Support and Services at Home for elders and Hub & Spoke Medicat ion Assisted Therapy for individuals batt ling opioid addict ion. All Blueprint work is closely integrated with health and human service organizat ions through Community Collaborat ives that guide care delivery and payment reforms at the local level.

The Blueprint's aim is constant: Better care, better health, and better

control over health care costs.

Jessica Bachiochi, RN, BSN - Nurse Case Manager

Jessica Bachiochi oversees transit ional care for BVHC pat ients and those with chronic condit ions including diabetes and high blood pressure.

Ms. Bachiochi works with BVHC transit ional care pat ients, primary care providers, their family members and a wide

range of community resources to ensure quality care post-hospitalizat ion. She follows up with high-risk chronic care pat ients to ensure they are following their care plan and ut ilizing available Blueprint services, as well as community services, to help them meet their healthcare goals.

Rachel Rodney, MS, RD, CDE - Diet it ian

Rachel Rodney provides pat ients with valuable dietary skills to lead a healthy lifestyle.

During a pat ient visit , Ms. Rodney learns about the pat ient 's current diet and offers ideas to help them make healthy food choices moving forward. Ms. Rodney ut ilizes

the community teaching kitchen where pat ients learn hands-on how to prepare and cook delicious, healthy meals for themselves and their families.

Jill Egbert , LCSW - Behavioral Health

Jill Egbert provides on-site, short-term behavioral health and substance abuse counseling to BVHC pat ients during stressful

t imes. During their visit with Ms. Egbert, pat ients begin to gain control of their emotional and behavioral needs, and they determine together the need for further assistance.

Long-term care needs are met through referrals to our full-t ime pract it ioner or one of the many organizat ions like United Counseling Services, which BVHC partners with to ensure the highest quality care possible for our pat ients.
