Actual Accide n t

At 2:45 a.m. o n au gust 5, 1994, employee #1, the operator, a nd two coworkers wereloadi ng a milli ng machi ne o n to a lowboy trailer. The ri ght rear track of the machi neslipped off the trailer's ed ge a nd the machi ne rotated 180 de grees, crushi ng employee #1 to death.

Actual Accide n t At approximately 6:30 p.m. o n march 27, 1996, Employee #1, a forema n a nd truck driver,was prepari ng to move i n to positio n a lowboy trailer pulled by a ford tractor so ahydraulic excavator could be loaded o n to it. The tractor trailer, which had bee n parked earlier i n the day by a nother driver, was faci ng dow nhill on a public roadway.Employee #1 received a beep o n his pa ger. He got out of the tractor a nd we n t in tothe house to make a pho ne call, leavi ng the truck idli ng . W he n employee #1 retur ned,he saw the tractor trailer rolli ng dow nhill a nd ra n toward the tractor i n a n attempt tostop it. Employee #1 fell a nd was ru n over by the rear ta ndem wheels o n the driver'sside. He was pro nou nced dead at the sce ne.
