1. IMPLEMENTATION OF THE WORK PLAN ACTIVITY 1 – Contribute to PAHO’S project on « Human resources for health development to support the practice of primary health care, promote the implementation of medical curricula oriented towards primary health care, and foster the development of postgraduate training in Primary Health Care/Family Medicine PAHO PROJECT Last year, the Sherbrooke Collaborating Center responded to the invitation from PAHO in order to contribute to a bank on innovative practices in Medical Education with the purpose of disseminating the social commitment of medical schools, the substantive change process of the curriculum, and original experiences that are contributing to the reorientation of medical education. The Bank of good practices is available online. (http://www.observatoriorh.org/?q=buenas-practicas) September 10th to 12th, 2014 Manaus, Brazil Martine Morin from the Sherbrooke Collaborating Center participated to the technical meeting “The Social Mission of Medical Education to achieve Health Equity”, organized by PAHO, the Brazilian Government and the Universidad de Estado do Amazonas. The purpose of this meeting was to promote the commitment of the social mission of medical schools to strengthen Primary Health Care and reach Universal Health Coverage. This conference was a follow-up to the December 2012 Buenos Aires meeting. After oral presentations of the OPS, the Brazil government, and some medical and health sciences schools, small group workshops permitted to propose orientations and future actions to promote a common vision of social accountability and implementation of accountable socially programs. The report of this meeting is available online. (http://www.observatoriorh.org/sites/default/files/webfiles/fulltext/2014/reu_manaus_sept/informe_misionsocial.pdf) At the end of the technical meeting in September 2014, there was a special side meeting about next steps with Dr. Rosa Maria Borrell and Dr. Jose Francisco Gutierrez-Garcia of PAHO, 3 Latin American schools (Cusco, La Matanza Argentina, Montevideo) and the 3 Collaborating Centers (Sherbrooke, Rockford, New Mexico). Each participating institution form South and North America were interested to commit to a bi-directional role in sharing with and learning with partner institution. The proposal was to discuss the follow-up at the upcoming meeting of The Network - Towards Unity for Health (Network-TUFH) in Forteleza, Brazil in November 2015. November 19th to 23th, 2014 Fortaleza, Brazil During the conference in Fortaleza, a meeting with Martine Morin and Eric Lachance from Sherbrooke, Arthur Kaufman from New Mexico, Michael Glasser from Rockford and Jose Francisco Gutierrez-Garcia from PAHO confirmed the commitment of the 3 collaborating centers. This group agreed to meet with Latin American Schools during the Beyond Flexner meeting in Albuquerque in April 2015. April 13th to 15th, 2015 Albuquerque, USA The 3 collaborating centers and 4 schools of Medicine and Health Sciences from Latino America (Leon, Nicaragua/ Montevideo, Uruguay/ Cusco, Peru/ La Matanza, Buenos Aires-Argentina) jointly attended the conference and had a meeting the last afternoon (April 15th, 2015) with Dr Rosa Maria Borrell, Dr Jose Francisco Gutierrez and Dr Fernando Menezes Da Silva from PAHO. The proposal was to form a consortium of schools on social accountability for the Americas and to commit to assess the social accountability of each of our Faculties and to work together to promote the social accountability in the Americas. Since that time the meeting report is awaited from PAHO and the signal to move forward in joint actions. This opportunity respond to the desire of our Faculty to explore in collaboration with PAHO, the establishment of a close partnership with one or more individual

faculties of Medicine in Latin and South America in supplement to our present collaboration with the Facultad de Medicina de la Universidad de la Republica (FM UdelaR), Montevideo, Uruguay (See activities #3 and #4). The collaboration with these faculties is done with the participation of FM UdelaR and will focus on Primary Health Care and social accountability. June 25th to 27th, 2015 Tucuman, Argentina Martine Morin from the Collaborating Center was invited to the 4th Congress of the “Foro de Escuelas y Facultades Publicas de Medicina de Argentina” in Tucuman. She had the opportunity to give the following lecture: “La extensión universitaria como escenario para la formación de equipos de salud que el país necesita - Experiencia de Sherbrooke”, and to better understand the context of medical education in Argentine. She meet with some Argentine schools of medicine with the objective to collaborate in medical Education. She visit the Universidad Nacional de la Matanza in Buenos Aires who also collaborate with PAHO in the consortium for social accountability. Planned actions for 2015-2016 November 2nd to 7th, 2015 Santa Fe, Argentine Invitation from the Universidad Nacional de Litoral to participate to the “Seminario Internacional de APS” on November 3rd, 2015 and to meet with teachers of the university to assess the feasibility and objectives of a future collaboration. This also reflects our desire to expand our collaborations with Latin and South America. Latino America’s Consortium on Social Accountability The instructions of the PAHO to move forward with this Consortium are still expected, but the process to assess our FMHS social accountability with the criteria of ASPIRE from AMEE or with THEnet Social Accountability Framework.

SOCIAL ACCOUNTABILITY OF MEDICAL SCHOOLS Sherbrooke Collaborating Center is still heavily involved in the International Project of Francophone Medical Schools around the world regarding Social Accountability. This project is under the responsibility of CIDMEF (Conférence international des doyens et facultés de médecine d’expression française) which regroups 139 faculties of medicine worldwide, and SIFEM (Société international francophone d’éducation médicale). The project, initiated in 2011, includes 45 participating schools. Its objectives is to promote social accountability and support the implementation of concrete actions regarding social accountability in the participating schools. The project (http://www.responsabilite-sociale-francophone.org/) has recently confirmed its strategic plan for 2015 to 2020. The main actions accomplished by Sherbrooke members regarding this project are the following: Participation of two representatives (Pr. Paul Grand’Maison and Pr. Tewfik Nawar) to the

International Executive Secretary of the project: regular phone conferences, contribution to the

management of the project, participation to annual meeting and other meetings, contribution to the

strategic plan process.

The project strategic plan has been confirmed in spring 2015. Paul Grand’Maison has played a

significant leadership role in order to develop the plan, obtain advices on the plan, confirm the plan

and present it in written documents. Worth noting is the participation of Paul Grand’Maison and

Tewfik Nawar to the specific 2-day workshop of the strategic plan held in Paris in February 2015.

of the meeting; leadership for the consultation on the strategic plan of the project; two scientific

presentation.

Sherbrooke representatives contributed to the dissemination of the concept of social accountability

among medical schools and of the international francophone project through activities at the

following meetings:

o September 2014 : 19e Journées universitaires francophones de pédagogie des sciences de la

santé de la Conférence international des doyens et facultés de médecine d’expression française

(CIDMEF). Cluj, Romania. 1 workshop, 1 oral presentation

o October 2014: Participation to the annual meeting of the Network Towards Unity for Health

(Network-TUFH) Fortaleza, Brazil. 4 posters, 1 symposium

o April 2015 : Congrès international francophone de pédagogie des sciences de la santé.

Our Faculty has accomplished significant steps regarding its project for the integration of Global

Health in programs of Medicine, Nursing Sciences, Physical Therapy and Occupational Therapy.

This specific project is identified as the Sherbrooke Faculty of Medicine and Health Sciences

(FMHS) concrete project to be included in this international collaboration.

Planned actions for 2015-2016

Continuous contribution of Paul Grand’Maison to the International Executive Secretariat of the project. Sherbrooke Medical Schools, in collaboration with the other Quebec medical schools will be the host of the annual meeting of this project on social accountability in May 2016 in Quebec, Canada Contribution to educational workshops on social accountability Scientific dissemination: presentations and publications Collaboration for the project of the establishment of a Consortium of Medical Schools from Americas on Social Accountability (see activity 1)

ACCREDITATION OF MEDICAL SCHOOLS

Pr. Tewfik Nawar from Sherbrooke is still Co-president of the CIDMEF. He has been involved in a few evaluations this year. He still is a consultant for medical education regarding accreditation with World Federation for Medical Education (WFME), other organization in various countries including Tunisia and Haïti. Pr. Paul Grand’Maison is a member of the same committee.

Planned actions for 2015-2016 Continuous involvement of Pr. Tewfik Nawar as Co-president of the Committee and Pr. Paul Grand’Maison as a member. INTEGRATION OF POPULATION/PUBLIC HEALTH TO INDIVIDUAL HEALTH

Pre Denise Donovan is still the leader of a working group of Network-TUFH on the integration of public health in medicine.

PARTICIPATION TO THE NETWORK- TUFH

Participation of Pre Denise Donovan on the executive of the Network-TUFH since 2007 Four members of Sherbrooke medical school participated to the annual meeting of the Network TUFH held in November 2014 in Brazil.

Planned actions for 2015-2016

Scientific dissemination: presentations and publications

Contribution to the organization of an international conference on Social Accountability planned to be held in Tunis, Tunisia in spring 2017. This conference will be organized in conjunction with the annual conference of the Network-TUFH and with the collaboration of numerous international organizations such as the World Health Organization, Faimer, AMEE, AFMC, etc. Sherbrooke should become an official faculty of medicine collaborating and contributing to the Training for Health Equity Network (THEnet)

ACTIVITY 2 – In Mali, Africa, support the development of postgraduate and continuing medical education in Family Medicine and the improvement of Primary Health Care services offered in community health centers.

FMHS IN MALI FMHS-UdeS is active in Mali since 1997, first with an International Health clinical training for Canadian residents and with various continuous medical education activities for Malian health care professionals. These activities have led to a broader commitment to Primary care in Mali and to the development of a partnership supporting Family and Community medicine in the country. In June 2010, the DECLIC program for the Development of Community based, clinical training of Health care professionals in Mali was launched. DECLIC is supporting 1) the implementation of a Department of Family Medicine/Community Medicine at the Faculty of Medicine and Dentistry (FMD) of Bamako and the National training institute for health sciences (nursing etc.). This includes 2) the elaboration of a curriculum adapted for Mali, 3) the establishment of a network of five academically upgraded Community Health Centers for community based clinical training, including 4) interdisciplinary work with nurses and midwives and 5) faculty development support (main activity: a 100-hour faculty development program offered over a 2-year period with face to face sessions mainly in Mali and personal work) DECLIC is an 18, 6 million dollars, 8 years [July 2010-June 2018] program funded by the Canadian Department of Foreign Affairs, Trade and Development. Although elective trainings for Canadian residents have been suspended in 2012 for security concerns, DECLIC activities have never stopped, thanks to a strong technical assistance team in Bamako and a robust partnership. September 2014: 7 professors from the Department of Family Medicine/Community Medicine at the FMD of Bamako and 3 Academic Community Health Centers (A-CHC) clinical teachers came to Canada for training purposes. They visited a family medicine unit from the University of Sherbrooke, participated in practical and theoretical training sessions, and met with curriculum and pedagogy specialists. November 2014: Dr M. Maiga and Pr F. Couturier attended the Canadien Family Forum, as well as the Sadok Besrour conference, where they held a presentation relating the evolution of the DECLIC project. January 2015: Official inauguration of the Academic Community Health Center Sanouboug II (Sikasso). The inauguration took place in presence of the Minister of health and public hygiene, delegates from the Canadian embassy, the FMD, National training institute for health sciences, as well as several other partners. March 2015: Emergency support organized in the Academic Community Health Centers (A-CHC) following the Ebola outbreak: distribution of equipment and training courses in environmental management, hygiene and sanitation. C.-A. Cormier visited our partners in Mali to establish a communication charter and gather information, as well as testimonies for a future promotion website. April 2015: Prs J. Achim, M. Terradas and M. Benoit conducted a data collection and teaching mission in Mali. This initial mission for an Integrating Mental health in primary care was comprised of participant observation, group discussions with health professionals (residents, doctors, nurses, midwives, as well as community workers and traditional therapists), as well as a 2-day workshop. Representatives from the FMD and the National training institute for health sciences participated at the international francophone conference for pedagogy of health sciences in Brussels, Belgium. A presentation about DECLIC was held by our colleagues from the National training institute for health sciences.

May 2015: M. Rietmann conducted several discussion groups with residents Department of Family Medicine/Community Medicine to develop knowledge about participatory research, as well as discussion groups with the Associations of Community Health Centers to evaluation possible effects of the project. June 2015: A. Aubin, a consultant, travelled to Mali to follow up on monitoring processes and approrpation mecanismes developed and implemented during her first visit in 2014. Throughout the year: Multiple peers’ supervision sessions in Academic Community Health Centers, as well as follow up missions with the Associations of Community Health Centers. Completion of the structural upgrading works at the Academic-Community Health Centers of Konobougou (Ségou) and Sanobougou 2 (Sikasso).

Planned actions for 2015-2016

Advocacy and long term planning: Meeting with partners at the FMD, the Canadian Embassy, the Minister of Health of Mali and other stakeholders (planned for September 2015) External supervision of residents and A-CHC clinical teachers. (Planned for September 2015) Development of an Accident and Emergency; treatment stabilisation and safe referrals program using local resources. (Planned for September through November 2015) Dr Maiga will attend the annual 2-dayworkshops of the Department of family medicine of the University of Sherbrooke. He will also meet with various key actors at the university and project members. (Planned for September 2015) 3rd and 4th block of Faculty and professorial Development for 6 FMD professors and 3 A-CHC clinical teachers. (Planned for November 2015) Dean and Vice-Dean of Bamako FMD and Pr F. Couturier will attend the Canadian Family Forum, as well as the Besrour Conference. (Planned for November 2015)

ACTIVITY 3 – Support the implementation of the renewed (2009) MD curriculum at the Facultad de Medicina de la Universidad de la Republica (FM UdelaR) in Uruguay through the improvement of students’ acquisition of clinical skills, reinforcement of the clinical skills lab and increase the use of simulation including standardized patients for education and evaluation.

This collaboration in Uruguay has been in place since 2006 and has become more and more important throughout the years. See the 2013-2017 CC Redesignation document and previous annual reports for the general objectives of this collaboration. This activity has been supported by the TCC funds received from PAHO (Extension till march 2013) titled “Fortalecimiento de la Medicina Familiar y Comunitaria en Uruguay y Canadá, en el marco del Nuevo Plan de Estudios de la Facultad de Medicina de la Universidad de la Republica, 2010-2012 » A second technical cooperation among countries (TCC) grant of $160,000 (US dollar) titled: Mejorar la formación clínica de los estudiantes de grado de la carrera de Doctor en Medicina y de los residentes y postgrados de Medicina Familiar y Comunitaria con el propósito de alcanzar mayor calidad en los servicios de Atención Primaria de Salud en Uruguay” has been submitted to PAHO in January 2013 for a 2 year period and was expected to start in spring 2013. Unfortunately the grant was refused by PAHO. Despite the absence of external funding, exchanges are continuing and the 2 Faculties are often collaborating as a team. Both faculties were represented in the Manaus, Albuquerque and Tucuman meeting. (See activity 1 for more details) Sherbrooke welcome a 6th grade student from the FM UdelaR, for an internship of 4 month with the Emerging Leaders in the Americas Program (ELAP) scholarship. It was the first medical student from Latin America to come in Sherbrooke. The project for the development of a skills lab was abandoned by the UdelaR for lack of funds. Planned actions for 2015-2016 The collaboration will continue especially with the department of medical education in the information and communication technologies field. We also want to explore the possibility of a collaboration in research in education. The FM UdelaR has to renew her accreditation in 2018 and wants to work with the Sherbrooke CC to help to facilitate the organisation of the process.

ACTIVITY 4 – Sustain the development of the academic discipline of Family and Community Medicine in Uruguay through the reinforcing of the residency and post graduate program in Family and Community Medicine and the development of research and scholarship capacity among teachers and residents.

See in Activity 3 the history of the collaboration. Despite the absence of external funding, exchanges of documents and information through, e-mails, Skype sessions, phone calls, etc. on the various subjects of the collaboration were implemented.

COLLABORATION WITH THE FAMILY AND COMMUNITY MEDICINE EDUCATIONAL PROGRAM

Teachers from the two departments of Family Medicine were involved in 2 panels and a workshop during the 4th “Congreso Iberoamericano de Medicina Familiar” in Montevideo (March 18th to 21th, 2015).

RESEARCH IN FAMILY MEDICINE Due to lack of funding, no collaborative research project is planned for the moment.

Planned actions for 2015-2016 Members of the Family Medicine department of the Université de Sherbrooke (UdeS) and members from the Udelar Family and community medicine department expect to participate to the WONCA congress in Brasil in 2016. They will work together to produce a workshop. Other action will be implemented as needed.

ACTIVITY 5 – Supporting education and practice of health professionals at the primary level of care in Haiti.

CONTINUED SUPPORT OF HAITIAN MEDICAL SCHOOLS.

Under the leadership of Lucie Brazeau-Lamontagne, Sherbrooke medical school continued to support medical education in Haiti, mainly through a strong collaboration with the Faculty of Health Sciences at Université de Quisqueya (FSSA-UniQ). During 2014-2015 period, numerous Sherbrooke medical teachers participated to 7 educational missions in Haïti (2 or 3 Sherbrooke faculty members for each missions). These missions covered program development, support to the governance of Haitian medical schools, faculty development workshops, contribution to specific teaching in the neurology unit (June 2015) at FSSA-UniQ with the participation of two residents in neurology. All the participants to these activities did it as volunteer; furthermore, some of them had to pay all or a part of their travel and living expenses. Mrs. Lise Couture, an administrative professional support person from FMHS – UdeS took a leave of absence from January to end of June 2015 in order to support the leadership (the Dean and her close collaborators) at FSSA-UniQ. She did it has a volunteer; her living expenses were partly covered by a NGO and by the Royal College of Physicians and Surgeons of Canada (RCPSC). The RCPSC has progressively become a partner of FMHS – UdeS in this endeavour to support Haitian medical schools. A major project is actually being planned for implementation by October 2015 by FSSA-UniQ and Faculty of Medicine – Université Notre-Dame (FM UND) in order to reform the internship offer by these two schools at Hôpital Justinien, Cap-Haïtien. FMHS – UdeS and the RCPSC are partnering with those schools in this project, seen as a pilot project (2015-2018) in order to improve the educational quality of the internship for all future interns in Haiti. The overall goal of the project is to develop better training for interns in Haiti, in order for them to be better-prepared to offer primary health care services during their social service year (the 1-year period following the completion of the internship) or during their future practice as general practitioners for those who will not complete a specialty residency program. The reform will include a revision and reorganization of:

A proposal for external significant funding was deposited to Quebec government in January 2015 but wasn’t successful. The project will continue in the future years with the collaboration of Sherbrooke medical schools and the Royal College of Physicians and Surgeons of Canada. Dr. Yannick Simon from FSSA-UniQ started in May 2015 a Master of Science program in biology (under the supervision of Dr. Ghassan Bkaily) at FMHS-UdeS Dr. Lucie Brazeau-Lamontagne from Sherbrooke participated to the “Ve Conférence Charles Mérieux” held in Port-au-Prince (February 23th, 2015) and titled “Global Health: Haitian Issues”. Dr. Brazeau-Lamontagne contributed as a resource person for an educational workshop.

During all the missions in Haiti, numerous activities were done in order to meet leading people at the level of the University, the Ministry of Health, the Ministry of Education, the Hospitals, etc. The objective is to work in collaboration with all these stakeholders in order to improve medical education in Haiti.

COLLABORATION IN REHABILITATION In 2014-2015, the collaboration in rehabilitation has been pursued. Three rotations (6 week long) including 4 students in occupational therapy or physical therapy (total number=11) with 2 supervisors per rotations (total number=6) occurred during the year. Due to external circumstances, the previous site (Hopital de la Communauté Haïtienne) where the rotations took place had to be changed during this period. After a rigorous process, the decision was made to move on rotations to Foyer Saint-Camille at Port-au-Prince

CLINICAL ROTATIONS IN INTERNATIONAL HEALTH IN FAMILY MEDICINE 18 students (9 resident in Family Medicine, 9 residents in other specialties, 5 clinical nurses and 2 practicing physicians) participated to 4-week rotations (7 rotations during the year) offered in Labrousse area, Haiti. Students were locally supervised by Canadian teachers (12 of them contributed to these supervisions during the year). Students and supervisors offered clinical services in Primary Health Care, community-based activities and capacity building activities for the personal working in this area. This rotation is a complementary activity to an advance study program in International Health that has been offered in our faculty for the last 10 years.

TRAINING FUTURE PHYSICIANS IN PRIMARY HEALTH CARE 2 projects were progressively develop during 2014-2015 regarding this objective:

Project of a reform of the internship alluded previously.

A collaborative group including representatives in Global Health development coming from UdeS,

Université of Toronto (UofT), College of Family Physicians of Canada (CFPC), Partner in Health-

Canada (PIH-Canada) and Partner in Health-Zanmi Lasanté (PIH-ZL) was established in spring

2015 in order to develop a comprehensive project to support the practice and education of Family

Medicine in Haiti through direct collaboration with the residency program in Family Medicine offered

by PIH-ZL at Saint-Marc, Haiti. The group resulted from exchanges among collaborators that were

held during the 2 previous years. Initial meetings were held and a comprehensive project is

progressively being developed. Collaboration with the Ministry of Health in Haiti as well with some

Haitian medical schools should be confirmed during 2015-2106. The collaborative group is looking

for funds ($100,000 to $150,000) in order to implement the initial need assessments and planning

phase of this project in 2015-2016 with the objective to present a grant proposal to a funding

agency for a project that would go to 2016 to 2021. The areas of focus of this project are :

o Development of national framework for training and practice in Family Medicine;

o Comprehensive strengthening of Family Medicine residency at Saint-Marc;

o Incorporation of Family Medicine within the social service student experience and in

undergraduate training;

o Continuing professional development in Primary Health Care and Family Medicine

MATERNAL AND NEONATAL HEALTH Some colleagues from Sherbrooke have continued to collaborate with Society of Obstetrics and Gynecology of Canada for the improvement of practice and education in Obstetrics and Gynecology in Haiti. Sherbrooke school has been the leader for a grant proposal deposited to Ministry of Foreign Affairs, Trade and Development in January 2015 titled: “SOURIRE-HAITI (Soins Obstétricaux d'Urgence, Renforcement des Institutions, Renforcement de l'Enseignement)”. This project was not funded. During 2014-2015, Pr. Jean-Charles Pasquier has pursued his research (funded by Grand Challenges Canada) to improve the utilisation of pessaries in obstetrical care in Haiti

Maintenance and consolidation of clinical rotations in international health in Family Medicine

Maintenance and consolidation of clinical rotations physical and occupational therapy

Continuing improvement in activities with SHOG (Société Haitienne des obstétriciens-gynécologues) and other

stakeholders in order to improve the management of pregnancy and delivery, in order to decrease maternal and

neonatal death.

Participation of Haitian faculty members to training session in Sherbrooke

Missions of Sherbrooke faculty members to support Haitian medical schools and improve medical education.

Significant involvement in the project to improve internship for students coming from FSSA-UNIQ and FM-UND

of Université Notre-Dame

Development of the project “Primary Care for a Healthier Haiti: A Canada-Haiti partnership for strengthening

Family Medicine”, a collaboration between UdeS, UofT, CFPC, PIH-Canada, PIH-ZL, Ministry of Health in Haiti,

2. COLLABORATION BETWEEN SHERBROOKE COLLABORATIVE CENTRE AND WHO

Collaboration through Activity 1 Sherbrooke Collaborating Center participated to the PAHO meetings. See Activity 1 for the details.

Collaboration for the management of the Ebola Virus Disease outbreaks in West Africa Pr François Lamontagne a Critical Care/Intensive Care specialist from Sherbrooke spent 3 weeks in September 2014 at the Foya treatment Center in Liberia. He works with Medicine Sans Frontiers (MSF) and other partners to ensure best and evidence-informed clinical management with the goal of reducing mortality. Planned actions for 2015-2016 Pr Lamontagne will pursue his collaboration with WHO and MSF if needed.

3. COLLABORATION WITH OTHER WHO COLLABORATING CENTRES

See in Activity 1 the collaboration with 2 Collaborating Centers for a Consortium on Social Accountability. Dr Giorgio Cometto, technical officer of the Global Health Workforce Alliance-WHO, invite the Sherbrooke CC to collaborate with the WHO Collaborating Centre on Health Workforce Planning and Research at Dalhousie University in Canada. The Dalhousie CC have been asked to develop a Global Strategy on Human Resources for Health by synthesizing available evidence on Human Resources for Health requirements in high-income OECD countries. Pr Gail Tomblin Murphy director of WHO/PAHO Collaborating Centre invite us to participate in this work as a member of the Advisory Group. The purpose of this group is to ensure that the review is as comprehensive as possible and that the appropriate stakeholders are engaged throughout its development.
