IIndia is experiencing a surge in its aesthetic industry. Close to

700,000 cosmetic surgical procedures are performed every

year the country, putting it in the top four powerhouses for

plastic surgery, alongside the USA, Brazil and China.

While India’s per capita rate of plastic surgery is currently lower

than in East or Southeast Asia, as one of the world’s fastest

growing major economies this is forecast to change. India’s

burgeoning middle class population is expected to increase to

more than half a billion people by 2030, meaning more and more

people will have the means to spend on luxury services.

We spoke to some of India’s leading aesthetic practitioners to

find out how these statistics are reflected in the industry and

what current trends indicate about the market.

• Dr Satish Arolkar has been in Aesthetic Plastic and

Reconstructive Surgery for 33 years and is a Trustee of the

Indian Association of Aesthetic Plastic Surgeons

• Dr Ashish Davalbhakta is Chief Consultant Cosmetic Surgeon

and Managing Director of Aesthetics Medispa

• Dr Chiranjiv Chhabra is Director and Consultant Dermatologist

of Skin Alive Clinics

• Dr Rashmi Shetty is an aesthetic dermatologist and author of

AgeErase

• Dr Kuldeep Singh is the Senior Consultant for Plastic,

Reconstructive & Aesthetic Surgery at the Indraprastha Apollo

Hospitals & Apollo cosmetic clinics New Delhi, India.

BOOM FACTORS

The doctors spoke of India’s flourishing economy, which has

touched all of the country’s industries, as a significant factor in

contributing to the aesthetic boom. As Dr Chhabra states, with

the rising disposable incomes, “the gap between aspiration

levels and purchasing power has narrowed” and the aesthetic

industry is no longer seen as belonging exclusively to the elite.

However, the industry’s growing popularity is not only due to a

financial change, but also a cultural shift. The doctors say that

the expansion in aesthetic procedures has been propelled by

the popularity of international beauty pageants; the growth in the

Bollywood film industry with its attractive heroes and heroines;

and the power of social media and its “selfie generation”, which

has a particular impact in India with its very young population.

Looking good is now paramount to success.

The popularity of aesthetic procedures also means that the

latest technologies, training and treatments are finding their

way to India. Dr Arolkar observes that there has been an influx of

multinational companies selling previously unavailable devices,

therapies and technologies. Dr Singh has also seen a similar

change: “Gradually, I think the thrust came from the pharma

industry, as they made products available in the country, along

with training programs by international faculty. The need was

there all along, and everything slotted quickly. Add to this the

fact that the purchasing power of the middle class increased,

their awareness increased, and most important of all, care

providers could provide safe, predictable and high level of

treatments which really sent the demand in a spiral.”

In short, Dr Singh says the most significant factors contributing to

India’s aesthetic boom are threefold: “availability, awareness and

affordability – in that order”.

TRENDS

The doctors have seen a general rise across all segments of

the industry.

Dr Chhabara recalls how the initial trend was for reconstructive

surgeries, but says consumers are now mostly going for

aesthetic procedures, while Dr Singh says that people now are

asking for procedures “to satisfy a greater range of needs but

also safe, reliable, affordable and quality care. In other words,

value for money.”

There is also a notable trend in the number of people in India

opting for non-invasive, non-surgical procedures. Dr Arolkar

names skin peels, lasers, radio-frequency devices, hifu, thread

suspension, hair grafting and fillers, as areas of growth.

Part of this surge in interest could also be ascribed to a

breaking down of taboos. The public now sees aesthetic

treatments in a more positive light, and consumers have

become willing to have treatment on any area they feel could

be improved.

Dr Chhabra explains that while the initial trend was for “face

only”, procedures, this has now expanded to the rest of the

body.

“Every year, getting ‘work done’ becomes more accessible and

more acceptable,” Dr Chhabra says, adding that procedures

have become “akin to a regular visit to a salon”.

In Dr Shetty’s practice, the aesthetic boom means that patients

are now more knowledgeable about aesthetic tools, services

and drugs and are open to intervention. “It takes less to explain

the need of combined approaches and need of regular

maintaining,” she observes.

According to Dr Chhabra, who has seen the industry evolve

significantly over the last 10 years, the changing trends also

extend to the origin of the people coming for treatment.

She says that one of the trends she has observed is that the

patient population is no longer only local, with medical tourism

on the up. She cites data from the Ministry of Tourism that

shows 96,856 foreigners visited India on medical visas during

January to June 2016, compared to 56, 129 for the whole of

2013.

This trend has also been witnessed by Dr Davalbhakta, who

says that 40 percent of his practice comes from international

medical tourists. Dr Singh added that international patients are

coming from countries in Central Asia, Africa and the Middle

East.

While dermatologist Dr Shetty has not seen witnessed a

growth in foreign patients at her clinic, she notes that this could

happen in aesthetic surgery. For her the focus is more local:

“every Indian is a potential customer”.

Dr Singh also observes a change in the clinics themselves:

“Dedicated aesthetic clinics attached to hospitals are coming

up, which seem to provide safety, reliability and quality to the

people more visibly than standalone clinics.”

GEOGRAPHIC GROWTH

So far the growth of India’s aesthetic industry has been driven

by the metropolitan cities.

However, Dr Chhabra forecasts that going forward “the

aesthetics wave” will be driven not only by the larger tier 1

cities, where the penetration still significantly lags global

averages, but also by tier II cities, such as Pune and Surat, and

by rural growth.

“Led by comparison with their urban counterparts, females in

rural India are now awakening to the idea of beauty, wellness

and aesthetics,” she says.

The other doctors also observed that while the largest number

of clinics and consumers are in larger, metropolitan cities,

other areas are indeed catching up. Dr Davalbhakta noted

that this expansion will continue, “as large parts of rural India

become upwardly mobile”.

CHALLENGES

In spite of all this growth, there are some challenges for those

in the industry to overcome.

According to Dr Chhabra, these include price sensitivity of

Indian consumers; significant investment costs; a lack of skilled

manpower, which limits the industry’s ability to scale up; and a

lack of indigenous R&D.

For Dr Shetty the challenges include too many players in the

market as well as a lack of regulations for doctors as well as

equipment providers.

Such lack of regulations can lead to a question mark over

reliability of services for consumers. Dr Chhabra has observed

this and sees potential quality issues for the consumer,

extending to medical staff, facilities and infrastructure, and

equipment.

As Dr Singh points out, “mushrooming of care, as well as

facilities, always brings us to the question of quality. Regulation

of who can do procedures, adequate training of professionals

providing this care, accreditation of facilities, screening and

regulation of new products being pushed into the aesthetic

market, and continuing medical education and upgrading of

skills will remain, and should remain the priorities in the years

to come.”

Dr Arolka perceives a challenge for specific therapies, namely

PRP and stem cell therapies as they are both regulated by the

FDA and are of limited popularity.

While for Dr Davalbhakta, the issue to tackle is a global one:

overcoming people’s fear of surgery.

THE FUTURE

Looking positively to the future, there are many opportunities for

the aesthetic industry in India.

Dr Chhabra points out that “the market size is immense, scope is

comprehensive and growth is unparalleled”. She says that in this

under-penetrated market established service providers are able

to rapidly expand.

With the growing awareness of the industry, the economic surge

and the sheer size of India’s population, Dr Shetty says this could

be called “a golden period” for the aesthetic industry.

“This industry is not stopping soon,” she asserts.

The selection process of these abstracts derives from precision and expertise. To ensure unbiased presentations, each abstract is carefully studied and preceded by the approval of a strict Conflict of Interest policy (COI), which is constantly updated by IMCAS in order to uphold a standard imposed by international CME accreditation entities.

Benjamin Ascher, MD, is a board certified Plastic Surgeon, lecturer and clinical assistant, from Paris Academy, and chairman and head of the Clinique of Aesthetic Surgery IENA in Paris – France.He is a member of the French, American, and International Societies of Plastic, Reconstructive and Aesthetic Surgery (SOFCPRE, SOFCEP, ASPS, ISAPS). He is section editor of the Aesthetic Plastic Surgery Journal and member of the Editorial Board for the Journal of Cosmetic and Laser Therapy, as well as author of 130 original articles published in referenced journals, and review articles in updated aesthetic surgery, non surgical procedures and recently, editor of “Injections treatments in Cosmetic Surgery” Informa, 2008. He is the founder and scientific director of IMCAS congresses (Paris, Cancun, Bali, and Mumbai), world leader in medical conferences dealing with the interface between Plastic Surgery, Cosmetic Dermatology, and Aesthetic Medicine.
