WORKING PAPER INSTRUCTIONS

PLEASE READ INSTRUCTIONS:

• Parents complete Part 1 of this form. • If you had a physical with your family doctor you must take this form

to your doctor and have it completed. • If you saw the school doctor for your physical the form wil be

compelted by the school nurse. The completed form should be taken top your home school district (usually the high school you would attend if you went to public school). That school will issue your working papers.

• You must bring proof of your residency, date of birth and social security card with you. You must also bring in a Photo ID to receive your Working Papers.
