CONDUCTING A RISK ASSESSMENT The following factors are considered when conducting a risk assessment and ascertaining:
the likelihood of an identified hazard/risk occurring the consequence if the identified hazard/risk occurs
Likelihood Factors
Almost certain The hazard / risk is expected to occur in most circumstances.
Likely The hazard / risk will probably occur in most circumstances (more than 50% of time).
Possible The hazard / risk should occur at some time (less than 50% of time).
Unlikely The hazard / risk could occur at some time, usually in exceptional circumstances.
Rare The hazard / risk is possible but is not expected to occur.
Consequence Factors
Extreme Death; high financial loss; sustained national media coverage
Major Extreme / permanent Injuries (significant hospitalisation); major financial loss; major negative state media
Moderate Medical treatment required (may involve hospitalisation); moderate financial loss; some state media, sustained local media
Minor First Aid treatment on site; minor financial loss; one off digital media, TV or newspaper coverage
Insignificant No injuries or treatment; low or no financial loss; one off local media coverage
RISK ANALYSIS MATRIX The following risk analysis matrix is used to assign a residual risk rating to the identified hazard/risk. A residual risk rating considers the risk management strategies in place.

HIERARCHY OF CONTROLS The hierarchy of controls describes methods to eliminate the hazard/risk, or it this is not possible,
control the hazard/risk to the fullest extent possible by using an alternate control method.
Eliminate the hazard Remove the hazard from the workplace. Stop using it, buy different substances or materials which don’t create a hazard or change the activity so that the hazard is not used.
Substitute the hazard with a safer alternative
Replace the material, process, chemical or machine with a less hazardous one to lower the risk from the hazard.
Isolate the hazard Isolate the hazard from the person at risk such as guarding of equipment and machinery or locating noisy machinery away from the main work area.
Engineering controls to reduce the risk
Design and install equipment to counteract the hazard such as installing an extraction system to remove dangerous fumes or dust or installing lifting equipment for heavy loads.
Administrative controls to reduce the risk
Establish and document work procedures and safe work practices. Examples include supervision of students, behaviour management programs, instruction and training in safe work methods.
Personal Protective Equipment
Use appropriately designed and properly fitted equipment such as safety goggles, ear muffs in conjunction with other control measures identified from above.
