Key failings of the Beaumont Leys accident…
• Unsafe working practice allowed to develop unchallenged for a prolonged period of time
• Training for team members in relation to the safe use of roll cages was inadequate
• The store specific Risk Assessments were not adequate • Store Manager failed to address the key risks arising from the use of
roll cages in the warehouse • Record keeping was poor, with many of the documents inconsistent
in format and content • Initial steps following the incident to manage the Leicester City
Council investigation lacked rigor and were not effective
Life changing consequences…
• Snapped her spine
• Wheel chair bound with a 1% chance of walking again
• Reliant on family and carers for the rest of her life
• Various additional health problems
• Significant impact on quality of life & shortened life span
