1. Scope

This Guideline covers the requirements associated with the entry into, and performance of work in, confined spaces such as tanks, vessels, pits, ditches, pipes, ducts, sewers, drains, tunnels, etc. The guideline addresses the assessment of hazards, authorisation for entry, preparation for entry, entry into the confined space, stand-by person's duties, work in the confined space (including hot work), rescue and first aid and the closure of confined spaces. Please Note This WHSE Guideline does not cover requirements which apply to designers or manufacturers of plant which includes confined spaces. These are covered in specific legislative requirements (refer Section 8 below).

2. Objectives

To ensure that all hazards associated with the entry into a confined space are assessed and controlled and risks are minimised and, where possible, eliminated.

3. Definitions

Confined Space An enclosed or partially enclosed space that is not intended or designed primarily for human occupancy, within which there is a risk of one or more of the following:

• An oxygen concentration outside the oxygen range;

• A concentration of airborne contaminant that may cause impairment, loss of consciousness or asphyxiation;

• A concentration of flammable airborne contaminant that may cause injury from fire or explosion;

• Engulfment in a stored free-flowing solid or rising level of liquid that may cause suffocation or drowning.

Entry (to a confined space) Entry into a confined space is when a person’s head or upper body is within the boundary of the confined space.

4. Issues to be Addressed

Please Note This section of the WHSE Guideline needs to be expanded and further developed by users into a full operational procedure. This is done by incorporating details to reflect your company’s structure, work process and local circumstances; legislative and other requirements; and technical details from publications and guidance notes. Assistance with developing procedures is available from Management Systems Australia. The major issues addressed in this guideline are as follows:




4.1. The Need for Confined Space Entry

In accordance with the requirements of the Work Health and Safety Regulation 2011, arrangements shall be in place in relation to the duty of designers, manufacturers, importers, suppliers, installers and constructors of plant and structures that include confined spaces. These arrangements shall be aimed at ensuring that:

• the need for any person to enter the confined space and the risk of a person inadvertently entering it are, so far as is reasonably practicable, eliminated; or

• if it is not reasonably practicable to eliminate the need to enter the confined space or the risk of a person inadvertently entering the space, ensuring that:

• the need or risk are minimised;

• the confined space is designed with a safe means of entry and exit; and

• risks to health and safety are eliminated or minimised so far as is reasonably practicable.

4.2. Construction Work In or Near a Confined Space

In accordance with the requirements of the Work Health and Safety Regulation 2011, any construction work carried out in or near a confined space shall be considered as High Risk Construction Work and arrangements shall be in place to ensure:

• the preparation and content of the Safe Work Method Statements;

• compliance with the Safe Work Method Statements;

• the provision of a copy of the Safe Work Method Statement to the principal contractor;

• the review of the Safe Work Method Statements; and

• the keeping of the Safe Work Method Statements.

4.3. Identification and Registration

All confined spaces which Company personnel may need to enter shall be identified. The identification shall be based on the detailed definitions of confined spaces, as per the relevant regulations and Australian Standards (refer Section 8 below). All confined spaces shall be registered in the Confined Spaces Register

4.4. Confined Spaces Review

A review shall be conducted of all confined spaces on site to determine whether the need to enter them can be eliminated altogether or alternatively, the frequency may be reduced. Specialists shall participate in this review, as required. Such a review shall also be conducted during the design phase of all new facilities which include confined spaces (refer WHSE Guideline WHSE-CG-030).

4.5. Warning Signs and Protective Barriers

All entry points into all confined spaces shall be sign-posted with a danger sign forbidding entry to unauthorised personnel (refer WHSE Guideline WHSE-CG-032). In addition, protective barriers shall be erected to prevent unauthorised entry.

4.6. Confined Space Hazards

The hazards associated with each confined space shall be identified and documented (refer WHSE Guideline WHSE-CG-028). Hazards to be considered prior to entry into a confined space shall include, but are not limited to, the following:

• Suffocation

• Poisoning

• Moving parts

• Fire

• Explosion

• Drowning

• Burial under solids

• Electrocution

• Radiation

• Burning and scalding

• Any other hazard, specific to a particular confined space.

4.7. Confined Space Work Permit

A Confined Space Work Permit must be issued for any entry into a confined space or for any work in or on the outside of a confined space (refer WHSE Guideline WHSE-CG-036). The issue of permits for work on the outside surface of a confined space shall apply if:

• There is any risk to the persons carrying out the work; or

• If there is a risk of fire or explosion.

Persons authorised to issue these certificates must be trained in confined spaces hazards (refer WHSE Guideline WHSE-CG-003). Before issuing the permit, the issuer shall consider whether the work can be carried out without entering the confined space. The Work Permit shall specify preparations for entry such as:

• Physical isolation including the isolation of hydraulic and pneumatic systems (refer WHSE Guideline WHSE-CG-037)

• Electrical isolation (refer WHSE Guideline WHSE-CG-038)

• Cleaning and purging

• Decontamination (refer WHSE Guideline WHSE-CG-039)

• Testing of the atmosphere

• The use of personal protective equipment (refer WHSE Guideline WHSE-CG-052)

• Briefing of the person(s) carrying out the work and the Stand-by Person(s).

The process for issuing the permit shall include a risk assessment (refer WHSE Guideline WHSE-CG-028) by a competent person or persons before work associated with the confined space is carried out. The assessment shall be in writing and take into account at least the following:

• the nature and inherent hazards of the confined space;

• the work required to be done, including the need to enter the confined space;

• the range of methods by which the work can be done;

• the hazards involved and associated risks involved with the actual method selected and equipment proposed to be used;

• emergency response procedures; and

• the competence of the persons to undertake the work.

The risk assessment shall be revised whenever there is evidence to indicate that it is no longer valid.

4.8. Work in the Confined Space

The concentration of oxygen in the atmosphere within a confined space and the increase in the level of flammable gases must be in accordance with Australian Standard AS 2865. The work in the confined space must be carried out as specified in the permit and all the required precautions must be strictly followed. The stand-by person shall follow the instructions for his duties and not leave his position while person(s) are in the confined space. In the event of an injury or collapse of the person in the confined space, the stand-by person's primary duty is to summon help and if possible, provide assistance, first-aid, resuscitation, etc. The Stand-by person must not enter the confined space.

4.9. Completion of the Work

At the completion of the work, the confined space shall be closed and the permit returned to the issuer and closed. Records of all confined spaces permits shall be kept.

4.10. Confined Space Training

Arrangements shall be in place to ensure appropriate confined space training is provided to workers who:

• enter or work in confined spaces;

• undertake hazard identification or risk assessment in relation to a confined space;

• implement risk control measures ;

• issue entry permits;

• act as a standby person or communicate with workers in a confined space;

• monitor conditions while work is being carried out;

• purchase equipment for confined space work;





• design or lay out a work area that includes a confined space;

• carry out any function in relation to work in a confined space or the emergency procedures but who are not required to enter the space; or

• carry out supervision of workers performing the above functions.

This training shall include, but not be limited to:

• the nature of all hazards associated with a confined space;

• the need for, and appropriate use of, risk control measures;

• the selection, use, fit, testing and storage of any personal protective equipment;

• the contents of any relevant confined space entry permit; and

• emergency procedures.

Re-training or refresher training shall be provided as appropriate. Records of all confined space training shall be kept as required by legislation (refer Sections 8.1 and 8.2 below).

5. Responsibilities of Personnel

The following personnel should assume the primary responsibility for the activities covered by this guideline:

• Maintenance Manager

• Line Managers (including supervisors)

• WHSE Manager

• Issuers of Confined Spaces Work Permit

• Personnel performing work in confined spaces

• Stand-by persons.
